National Rural Health Resource Center

Technical Assistance and Services Center (TASC) supports CAHs with Quality Improvement: Provides Information, tools, and education for CAHs and state Flex Programs on the TASC website. TASC supports quality, financial, and operational improvement, including MBQIP; health system development; and community engagement. Quality Health Indicators (QHi)

2015-2016 WY Flex Program Activities. QHi – is a user driven program specifically designed to facilitate benchmarking for small rural facilities. Participants trend data and benchmark against self-defined peer groups to learn from the best practices of other organizations in order to adopt new processes in four categories: clinical quality, financial and operational, employee contribution, and patient satisfaction. QHi includes a set of over 250 indicators developed by participating hospitals. All MBQIP measures can be reported and tracked in QHi. Reports,

graphic displays, and data uploads from CART are some of the key features along with customized dashboards, best practices reports, and a report scheduler to automatically email customized reports to staff and board members. QHi is made available to all CAHs in Wyoming through the Flex Program at no charge. Contact Rochelle Spinarski at [email protected] with questions.

Population Health Tool—Portal. The National Rural Health Resource Center (The Center) created a Popula-tion Health Portal in cooperation with the Federal Office of Rural Health Policy (FORHP). The Population

Health Portal is designed to help CAHs, Flex Coordinators and rural health networks navigate the journey towards improved population health. About the Portal—This Population Health Portal adopts a systems-based approach modeled after the Baldrige Framework for Perfor-mance Excellence in managing the complexities of transitioning to value-based care and pay-ment. This comprehensive approach, which includes the ability to measure and show value, can also help rural health organizations frame the essential components for adoption of pop-

ulation health as an integral strategy. The Portal features a readiness assessment for hospitals and networks and connects tools and resources to the organization’s unique strengths and needs for transitioning towards population health.
