Chapter 1

Assign Initial Bed

OverviewThe Assign Initial Bed menu function is used to assign a bed to a patient, based on an admit bed request. Admit bed requests are generated when a visit is created. This menu function encompasses many of the bed management and census features of the Bed Control menu function. The Assign Initial Bed menu function provides a de-centralized approach to bed assignment.

Function Attributes

DescriptionOnce the Assign Initial Bed menu function is accessed, the identification of a patient and a visit must be entered. After the visit is identified, the System checks the current bed requests assigned to the bed control queue defined in the push parameters. If an admit bed request exists for the defined patient, the Bed Request screen displays. If no request exists, an error message displays.

Bed Request ScreenThe Bed Request screen displays information that is needed to determine the most appropriate bed to assign the patient. A bed control service/unit can be entered or the registration and/or visit screen can be edited. For a complete description of the Bed Request screen, see Chapter 3, Bed Control.

Bed Assignment ScreenOnce a bed control service/unit is specified, the System displays the Bed Assignment screen. The Bed Assignment screen lists all the available beds for the specified service. An individual unit can be selected or all units chosen. For a complete description of this screen, see Chapter 3, Bed Control.

Bed Assignment Accept ScreenOnce an available bed is selected from the Bed Assignment screen, the Bed Assignment Accept screen displays. This screen displays detailed bed and roommate information about the selected bed. Accepting the bed assignment causes the patient to be assigned to the bed. For a complete description of this screen, see Chapter 3, Bed Control.

Push Parameters

Bed Control Queue

Determines the beds that are available for assignment. A bed control queue can be selected or the default area can be chosen. Bed Control Queue options are defined in the Bed Control Queue Table, ^rt("bcq").

When the default area is chosen, the System defines the bed control queue through the terminal location. By using the location of the terminal, the System looks up the default bed control queue. Terminal locations are defined in the Device Table, ^ad. The default bed control queue for each area is defined in the Area Table, ^afa.

Registration Screen Sequence

Defines the registration screen sequence that displays when the edit registration option is selected from this menu function. Options are defined in the Registration Screen Sequence Table, ^rt("rs").

Visit Screen Sequence

Defines the visit screen sequence, for each of the visit types that displays when the edit visit option is selected in this menu function. Options are defined in the Visit Type Table, ^rt("vt").

Available Bed Summary

OverviewThe Available Bed Summary menu function is used to display a summary list by unit of current statistics concerning the number of beds that are available or about to become available.

Function Attributes

DescriptionOnce the Available Bed Summary menu function is accessed, the QuadraMed Computerized Patient Records (QuadraMed CPR) System displays the word Processing. While the System is processing, it scans for the current status of all beds defined in the Area Table, ^afa. This scan generally takes less than five seconds.

When the scan is finished, the QuadraMed CPR System displays the Current Available Beds Screen. This screen displays either as a double column or as a single column (refer to the figures on the next page), depending upon how the Display Type push parameter is defined.

Vacant (Vct) Displays the total number of unoccupied beds. Both licensed and unlicensed beds are included in this total.

Pending (Pnd) Displays the number of beds that are currently occupied by patients who have a pending discharge or transfer.

Total (Tot) Displays the total number of vacant beds plus the number of pending beds.

Reserved (Rsv) Displays the number of beds that have been assigned to patients. Reserved beds can be occupied by one patient and reserved for another. Reserved beds are not included in the Total column.

Bed Control

OverviewThe Bed Control menu function is used to track available beds, place patients into beds, and to verify and accurately record the patient's location. Through the use of this menu function, the following processes can also be performed:

Assign bedsChange financial bed types Create and assign overflow bedsCreate and maintain wait lists De-assign bedsEdit registration and visit data View currently available beds

In the QuadraMed CPR System, a bed is a resource that needs to be managed efficiently. In order to efficiently manage a bed, the need for that bed must be communicated as soon as it is known. This method of communication is known as a bed request. The bed request contains information that is used to determine the most appropriate bed to assign to the patient. This information includes the admission type, bed request or medical service, level and type of care required, special precautions and/or restrictions, and the patient's sex, age, diagnosis, and preferences.

A bed request is created when a patient event that requires a bed (i.e., visit processing, order entry, discharge reversal) is defined. Only inpatient visits and those visit types defined as allowing bed assignments can create bed requests. A visit type can be defined to allow bed assignments in the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, Facility-specific level, Bed Control Parameters Screen, Bed Control Information field. After a bed request is generated, it is routed to the bed control queue(s) defined for the patient's bed control service.

A bed control service must be defined as part of the visit in order for the System to generate a bed request.

In order to access the list of available beds that satisfy the bed request, a bed control service or unit must be specified. Bed control services represent beds with similar characteristics. These characteristics relate to the type of care that is provided to a patient occupying a bed in that service. Hospitals use bed control services to establish bed assignment priorities and guidelines (e.g., physician specialty and/or unit specialty). By designating beds to a particular service, hospitals can efficiently allocate similar resources (e.g., bed, equipment, and/or nursing care). Bed control services can be defined to include one or more nursing units, and one nursing unit can belong to more than one bed control service. Bed control services are defined in the Bed Control Service Table, ^rt("bcs").

When a bed request is made for a particular service, it is routed to the bed control queue(s) defined for the service. Bed control queues are designed to prioritize the issuance of beds. For instance, in most hospitals, transfer requests are handled first, followed by emergent admissions, then urgent and elective admissions. This priority helps to ensure that beds are first given to the patients who need them the most. Bed control queues are defined in the Bed Control Queue Table, ^rt("bcq"), and linked to a bed control service in the Bed Control Service Table, ^rt("bcs"), Bed Control Queue(s) field.

Transfers/LOA/INTDisplays the bed requests generated by a transfer, leave of absence, or visit interrupt. When this queue is selected, the System displays the bed requests sorted by home location. The areas display in the order defined in the Area Table, ^afa, Bed Dsply Ordr field. Patients who are not currently checked into an area are listed under the In Transfer heading. Patients who are occupying beds that do not have the Area Table, ^afa, Bed Dsply Ordr field defined are listed under the Other Areas heading (see the figure below).

Emergent AdmissionsDisplays the bed requests generated by a new admission when the admission sub-type is defined as emergent. Admission sub-types are defined in the Visit Type Table, ^rt("vt"), Visit Sub-Type subfile. A visit displays in this queue when the field created from the Visit Type Table, ^rt("vt"), Visit Sub-Type data element is defined with an emergent sub-type. A sub-type is defined as emergent when the Visit Type Table, ^rt("vt"), Visit Sub-Type subfile, Bed Control Queue Type field is defined as emergent admission. When this queue is selected, the System displays the bed requests sorted by time. Only those bed requests for the current 24-hour period (i.e., midnight to midnight) display. When this queue is selected, the System displays a list of all transfer, leave of absence, interrupt, emergent, urgent, and elective bed requests, sorted by time (see the figure on the next page). Only the bed requests for the current 24-hour period (i.e., midnight to midnight) display. Pressing the EXPD key displays the requests that are not in the current 24-hour period. A patient can be selected or one of the following processing options can be chosen:

Review current bed assignments – Displays the list of beds that have patient assignments (i.e., reservations), but the patient has not been checked into the bed. For a complete description of this processing option, see Review current bed assignments on page 14.Hold bed – Displays the list of beds that are currently being held and allows a bed to be edited, added, or deleted. When this processing option is chosen, the System prompts for a bed control service/unit to be entered. Once a service is entered/selected, the System displays all available beds for the specified bed control service/unit. When the selected bed is in a room with more than one bed, the System displays roommate information for each occupied bed.

Wait List RequestsDisplays a list of all patients who have been assigned and/or checked into a bed, but are awaiting a bed that meets all the requested preferences. The System displays the patients in alphabetical order. For a complete description of the wait list, see Wait List on page 29.

A queue type can be selected from the Bed Control Queue Type Summary Screen or one of the following processing options can be chosen:

Review current bed assignmentsDisplays the list of beds that have patient assignments (i.e., reservations), but the patient has not been checked into the bed. The System prompts to select a bed to de-assign. Only assignments for beds that are linked to the Bed Control Queue push parameter of this menu function display.

Since the Transfers/LOA/INT queue contains all unresolved transfer requests and the All Bed Requests queue contains only those bed requests for the current 24-hour period, the number of bed requests in the All Bed Requests queue may not reflect the total number of bed requests.

The priority of the bed request displays with the bed assignment. LOA and wait list default priorities are defined in the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, System-specific level, Bed Control Information Screen, LOA Bed Request Priority and Wait List Bed Request Priority fields. A request priority is also defined for each visit type that creates admit events in the Visit Type Table, ^rt("vt"), Visit Sub-Type subfile, Bed Request Priority field. Priorities are defined in the Priority Table, ^nt("p").

Once a bed is selected to de-assign, the System displays the Bed Request Screen and prompts for the removal of the bed assignment to be accepted. When the patient is currently in transfer, the System displays a NOTE: Patient currently in transfer! message. For a complete description of the Bed Request Screen, see Bed Request Screen on page 16.

Hold bedAllows a held bed to be edited, added, or deleted. When this processing option is chosen, the System displays a list of beds currently being held, along with the patient's sex and a comment(s) related to the hold. The following processing options are available:

Edit – When a held bed is selected, the System displays the selected bed at the top of the screen and prompts for the sex of the patient for which to hold the bed (i.e., male, female, or unknown). After the sex is defined, the System allows the hold comment to be edited.

Add – Allows a bed to be placed on hold. When this processing option is chosen, the System prompts for a bed control service/unit to be entered. Pressing the EXPD key displays the bed control services defined in the Bed Control Service Table, ^rt("bcs"). Once a bed control service/unit is selected/entered, the System displays the Bed Assignment Screen and prompts for the bed to hold. Only those beds that are currently available (i.e., vacant, pending discharge, or pending transfer) can be held. Once the bed is selected, the sex of the patient for which to hold the bed can be entered (i.e., male, female, or unknown). When the bed is located in a room with more than one bed, the System displays roommate information and prompts for a hold comment to be entered. Once a comment has been entered, the hold can be accepted.

Held beds display in dim text on the Bed Assignment Screen with the message bed held (<patient sex>): hold comment>. For a complete description of the Bed Assignment Screen, see Bed Assignment Screen on page 23.

Held beds also display when the view available beds processing option is chosen. When this processing option is chosen, the System displays the Bed Assignment Screen with the message bed held: <hold comment>. The patient's sex does not display.

Delete – Allows a held bed to be unheld. A hold can be removed during bed assignment or through this processing option. When a held bed(s) is selected to delete, the System prompts for the deletion to be verified. Once the hold is removed, the bed can be assigned.

Placing a hold on a bed does not remove any current assignments for that bed and no new assignments can be made against the bed.

When a held bed is selected from the Bed Assignment Screen, the System displays a NOTE: Bed is currently being held message and prompts to verify the removal of the current bed on hold. The System defaults to no. When the removal is verified, the System deletes the held bed.

View available bedsAllows a list of all available beds for a specified bed control service to display. When this processing option is chosen, the System prompts for a bed control service/unit to be entered. Pressing the EXPD key displays the bed control services defined in the Bed Control Service Table, ^rt("bcs"). Once a service is entered/selected, the System displays all available beds for the specified bed control service. When the selected bed is in a room with more than one bed, the System displays roommate information for each occupied bed.

Bed Request ScreenWhen a patient is selected from within a bed control queue, the System displays the Bed Request Screen (see the figure on the next page).

An inappropriate transfer is detected when the transfer warning group defined in the Area Table, ^afa, Area level, Trfr Warn Grp field for the patient's current location does not match the transfer warning group defined for the requested bed control service in the Bed Control Service Table, ^rt("bcs"), Transfer Warning Group field. Transfer warning groups are defined in the Transfer Warning Group Table, ^rt("twg").

The Bed Request Screen contains information that is used to determine the most appropriate bed to assign to the patient. This screen is divided into the following sections:

Bed Request InformationDisplays information pertaining to the requested bed (e.g., request type, request date/time, and bed type). The following fields can display within this section:

When a bed request is selected from the Transfers/LOA/INT queue, and an inappropriate transfer is detected, the System displays a warning message indicating the transfer is illegal, and the patient must be discharged and readmitted. The user can override the warning when the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, Facility-specific level, Bed Control Parameters Screen, Allow Transfer Warning Override? field is defined as yes. When the Allow Transfer Warning Override? field is defined as no, the warning message displays but the user cannot override the warning. In this case, the patient must be discharged and readmitted or another transfer location must be specified.

Request Type – Displays the process that caused the bed request to be generated. The following request types can display:

Request Date/Time – Displays the date/time the bed is needed. The admit request date/time is generated from the scheduled visit date and time. The transfer request date/time is defined in the transfer order. The LOA and visit interrupt return request date/times can be entered in the order and/or during documentation of the event. The discharge reversal date/time is generated from the discharge reversal event time.

Bed Requested – Displays the name of the requested bed control service.

Bed Type(s) – Displays the bed type(s) of the room or bed that is needed. Admit and discharge reversal bed types are defined during visit processing, transfer bed types are defined during order entry, and LOA and visit interrupt return bed types are defined during the check out process. Bed Type Options are defined in the Bed Type Table, ^rt("bt").

Priority – Displays the order of importance in which this request should be handled. Admit request priorities are defined in the Visit Type Table, ^rt("vt"), Visit Sub-Type subfile, Bed Request Priority field for each visit sub-type option. Transfer bed request priorities are defined in the order information. LOA and visit interrupt return priorities are defined in the System Parameter Table, ̂ %z, Reg/ADT System Parameters subfile, System-specific level, Bed Control Information Screen, LOA Bed Request Priority field.

Diagnosis – Displays the patient's diagnosis, if defined.

Comment – Displays pertinent non-clinical information about the patient. When a comment is entered as part of the bed request during visit processing or order entry, it displays in this field.

(De-)Assigned By – Displays who assigned or de-assigned the patient's bed (i.e., an employee or the System). When system displays, the bed de-assignment was due to an automatic cancellation or closure, based on the parameters defined in the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, Facility-specific level, Auto Close/Discharge Parameters (IP,EP,OP) and Auto Close/Discharge Parameters (CP,RP,SP) Screens. The System also de-assigns beds when the patient's visit time is changed to a time when the reserved bed is not available (e.g., still occupied).

Patient Preference/StatusIndicates the type of bed needed for the patient. The following fields can display within this section:

Preference(s) – Displays the patient's preference for bed/room features (e.g., by a window, with television, near nursing station, etc.). Preferences are defined during visit processing in the field created from the Visit Type Table, ^rt("vt"), Bed Preferences data element.

Smoking – Displays the patient's preference for a non-smoking or smoking room. Smoking preference is defined as part of registration, visit, or order information. When searching for available beds for the patient, the System considers only those beds as a match that have a roommate(s) with the same smoking preference.

Other – Displays additional information related to the type of bed needed for the patient. This information is generated from the patient's visit information. This field can include the following information:

Bed Checkin HistoryDisplays a list of beds, with the checkin date/time, the patient has occupied during the course of a visit.

Pending EventsDisplays a list of outstanding bed request events for the patient (i.e., admit, checkin, transfer, and discharge). When the event is currently in-progress (e.g., the patient has been transferred out of a bed, but has not yet been checked into a new bed), current displays next to the bed request event, instead of a date and time.

Enter bed control service/unitA bed control service/unit can be entered or the default bed control service can be accepted by pressing the ENTER key. Pressing the EXPD key displays the bed control services defined in the Bed Control Service Table, ^rt("bcs"). The System defaults the bed control service/unit from one of the following:

Employee/Provider Table, ^ae – When an inpatient visit is created for a patient, the System defaults the bed control service defined for the attending physician. A default bed control service can be defined for each specialty(s). The physician's default bed control service is defined in the Employee/Provider Table, ^ae, Facility-specific level, Specialty(s) field.

System Parameter Table, ^%z – For visit types other than inpatient, the System defaults the bed control service defined for each visit type. The default bed control service is defined for a visit type in the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, Facility-specific level, Bed Control Parameters Screen, Bed Control Information field.

Bed Request Bed Control Service data element – When a bed control service is not defined in the Employee/Provider Table, ^ae, System Parameter Table, ^%z, or Physician Service/Department/Specialty Table, ^aet("ps"), the System defaults the information defined in the field created from the Visit Type Table, ^rt("vt"), Bed Request Bed Control Service data element. This data element should be included on all visit screens for visit types that allow patients to be assigned to beds.

Bed Ctrl Service/Unit data element – When the bed control service is entered through order entry or result entry, and a bed control service is not defined in the Employee/Provider Table, ^ae, System Parameter Table, ^%z, or Physician Service/Department/Specialty Table, ^aet("ps"), the System defaults the information defined in the field created from the Order Entry Profile Table, ^nt("op"), or Result Entry Profile Table, ^nt("rp"), Bed Ctrl Service/Unit data element. This data element should be included on all relevant order and result entry profiles.

Once the bed control service/unit is selected/entered, the System displays either the Medical Service Summary Screen or the Bed Assignment Screen. For a complete description of the Medical Service Summary Screen, see page 21. For a complete description of the Bed Assignment Screen, see page 23.

Edit registrationAllows the patient's registration information to be edited. When this processing option is chosen, the System displays the Registration Screen Options included in the registration screen sequence defined in the Registration Screen Sequence push parameter of this menu function. Patient registration information can be entered or edited, and new registration reports can be generated.

Edit visitAllows the patient's visit information to be edited. When this processing option is chosen, the System displays the Visit Screen Options included in the visit screen sequence defined in the Visit Screen Sequence(s) push parameter of this menu function. Medical Service Summary ScreenWhen a facility uses the medical service as the basis for allocating available beds, the Medical Service Summary Screen displays when a bed control service/unit is entered from the Bed Request Screen (see the figure on the next page). This screen displays only when the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, System-specific level, Bed Control Information Screen, Bed Control By Service? field is defined as yes.

When the Bed Control By Service? field is defined as yes, the System retains current statistics for each medical service and displays those statistics on the Medical Service Summary Screen. These statistics determine whether there are beds available or whether a bed(s) should be borrowed from another medical service. The Medical Service Summary Screen provides the ability to review the available beds by unit for the requested service. This screen contains two separate summary counts: one for the medical specialty associated with the patient's attending physician and one for all units included in the bed control service.

The following columns display for the medical service summary:

COLUMN DESCRIPTION

Beds Displays the total number of beds allotted to the specialty. This number is defined in the Area Table, ^afa, Area level, Dept/Spec Def field for each specialty associated with the area.

When OV displays in this column, the specialty is considered to be in an overflow situation. An overflow situation occurs when the number of beds defined for the specialty are all occupied or when there are no beds defined for the specialty.

Occ Displays the number of beds on the unit that are occupied by patients assigned to the medical service specialty.

Out Displays the number of beds on the unit that are occupied by patients belonging to the medical service specialty who currently have a pending discharge or transfer.

In Displays the number of beds on the unit that have been assigned, but not occupied by patients belonging to the medical service specialty.

Avail Displays the number of beds on the unit that are available for the specialty. The System calculates the number of available beds by adding the total number of occupied beds (i.e., Occ column) and assigned beds (i.e., In column), and subtracting that total from the total number of beds allotted to the specialty (i.e., Beds column).

NOTE: The numbers that display in these columns reflect only those beds that are defined in the Area Table, ^afa. Overflow beds that have been created for the individual units and/or medical services are not included in these counts.

COLUMN DESCRIPTION

Beds Displays the total number of beds defined for the unit. Both licensed and unlicensed beds are included.

Occ Displays the number of beds on the unit that are currently occupied.

Out Displays the number of beds on the unit that are occupied by patients who currently have a pending discharge or outgoing transfer.

In Displays the number of beds on the unit that have been assigned, but not occupied by patients, including incoming transfers, admits, and LOA/visit interrupt returns.

Avail Displays the number of available beds on the unit. The System calculates the number of available beds by adding the total number of occupied beds (i.e., Occ column) and assigned beds (i.e., In column), and subtracting that total from the total number of beds defined for the unit (i.e., Beds column).

Bed BorrowingWhen the EXPD key is pressed from the Medical Service Summary Screen, the System displays a list of medical services. The following information displays for each medical service:

The System prompts for a medical service to be selected. The System scans the specialties defined in the Physician Service/Department/Specialty Table, ^aet("ps"), and displays the specialty(s) that has the Bed Control For This Specialty? field defined as yes.

Bed borrowing occurs when a patient is assigned to a bed defined for a service that is different from the patient's medical service. When a medical service is selected that is different from the requested medical service, the System displays a Medical service selected is different from patient's medical service! message and prompts for verification to continue the bed assignment. The System defaults to no. When the continuation is verified, the System prompts for an overflow medical service comment to be entered. This comment, along with the overflow medical service, displays as part of the bed checkin event.

Bed Assignment ScreenWhen a unit(s) is selected from the Medical Service Summary Screen, or when the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, System-specific level, Bed Control Information Screen, Bed Control By Service? field is defined as no, the System displays the Bed Assignment Screen (see the figure on the next page). This screen displays all available beds for the bed control service that meet the criteria specified in the bed request. An available bed is any bed that is currently vacant or is scheduled to be vacant on or before the date/time of the bed request.

COLUMN DESCRIPTION

Total Displays the total number of beds allotted to the service. When OV displays in this column, the specialty is considered to be in an overflow situation. An overflow situation occurs when the number of beds defined for the specialty are all occupied or when there are no beds defined for the specialty.

Curr Displays the number of patients currently in beds for the medical service. This number includes all assigned and overflow beds for the service.

Admit Displays the number of patients with pending admissions for the medical service who have an assigned bed, but have not yet been checked into the bed.

Disch Displays the number of patients checked into beds belonging to this service who currently have a discharge order.

Avail Displays the number of beds currently available for this service.

Since this screen contains all the available beds that meet the specified criteria, the System displays Match in a bright box in the upper right corner of the screen. Beds that have been assigned to other patients or held display in dim text. Beds that have no assignments or holds against them display in bright text.

The following information displays for each bed on the Bed Assignment Screen:

Room/BedDisplays the location of the room/bed.

StatDisplays the current status of the bed. The following status codes can display:

HK – Indicates the bed is in the housekeeping queue and has not been cleaned. Assigning a patient to a bed that is statused as HK does not delete the entry from the housekeeping queue.HLD – Indicates the bed is currently held for a patient. Information concerning the held bed displays in dim text on the line below the bed being held. A held bed can be assigned by choosing the hold bed processing option from either the Bed Control Queue Type Summary Screen or the All Bed Requests queue, and deleting the hold, or by selecting the held bed when making an assignment. Deleting the hold makes the bed available for assignment when processing a bed request.

PD – Indicates the patient currently occupying the bed has a pending discharge order. Although the bed is available for assignment, a patient cannot be checked into the bed until the patient occupying the bed has been discharged or checked out of the bed.PL – Indicates the patient currently occupying the bed has a pending leave of absence order. Although the bed is available for assignment, a patient cannot be checked into the bed until the patient occupying the bed has been discharged or checked out of the bed. PI – Indicates the patient currently occupying the bed has a pending visit interrupt order. Although the bed is available for assignment, a patient cannot be checked into the bed until the patient occupying the bed has been discharged or checked out of the bed.

Bed TypeDisplays the bed type(s) defined for each bed in the Area Table, ^afa, Sub-area or Sub-sub-area level, Bed Type(s) field. Bed types are used to specify the physical characteristics of each bed (e.g., bassinet), the type of specialized equipment associated with the bed (e.g., telemetry), and the room in which the bed is located (e.g., semi-private). Bed Type Options are defined in the Bed Type Table, ^rt("bt").

Roommate InformationDisplays information about the current occupant(s) of the room (e.g., sex, attending physician, age or age range, etc.). This information determines whether the current occupant(s) is acceptable with the patient being assigned to the bed. Any pending orders (e.g., transfer, discharge, etc.) for the roommate(s) display on the line below the available bed information. The roommate information that displays is maintained only when the Area Table, ^afa, Area or Sub-area level, Roommate Info? field is defined as yes.

The System indicates whether a bed meets all the patient preferences by displaying an asterisk (*) before the bed type. When no patient preferences are defined, all beds are preceded by an asterisk (*).

Pressing the EXPD key displays all available beds for the selected bed control service. The System displays No Match in a bright box in the upper right corner of the screen.

A bed can be selected or an overflow bed can be created. When a bed is selected, the System displays the Bed Assignment Accept Screen. For a complete description of this screen, see Bed Assignment Accept Screen on page 26.

Overflow Beds Overflow beds are associated with a unit or specific area of the hospital and are created on anas needed basis. They are used as extra accommodations when a particular service or unit is full, when no predefined beds exist (such as in the nursery), or when an infant is placed in the mother's room.

In the QuadraMed CPR System, once the overflow bed is created, it exists as long as the patient is assigned to it. Once the patient is de-assigned, transferred, or discharged out of the bed, the overflow bed is automatically deleted.

When the create overflow bed processing option is chosen from the Bed Assignment Screen, the System prompts for an overflow bed location name to be entered. An area, sub-area, or sub-sub-area look-up name can be entered or the EXPD key can be pressed to display a list of Location Options. The areas that display are defined in the Area Table, ^afa.

After an overflow location is specified, the System displays a list of Overflow Bed Type Options and prompts for one to be selected. Overflow bed types are defined in the Overflow Bed Type Table, ^rt("bo"). After an overflow bed type is selected, the System displays the Overflow Bed Accept Screen (see the figure on the next page).

The Overflow Bed Accept Screen displays information about the overflow bed (i.e., bed name, bed short name, bed look-up name(s), and roommate information). Once the overflow bed is accepted, the overflow bed information displays on all unit displays. The roommate information that displays is maintained only when the Area Table, ^afa, Area or Sub-area level, Roommate Info? field is defined as yes.

Bed Assignment Accept ScreenWhen a bed is selected from the Bed Assignment Screen, the System displays the Bed Assignment Accept Screen (see the figure on the next page). This screen displays detailed bed and roommate information, and prompts for the bed assignment to be accepted.

Financial bed typeDisplays the default financial bed type of the selected bed. Default financial bed types can be defined for each bed in the Area Table, ^afa, Sub-area or Sub-sub-area level, Default Fin Bed Type field. This information displays only when the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, System-specific level, Bed Control Information Screen, Use Financial Bed Types? field is defined as yes.

Request/statusThe System displays whether the service, sex, bed type(s), and/or bed preference(s) match the patient's requests. When the selected bed matches the bed request, the System displays OK, in dim text, in the Status column. When the bed does not match the bed request, the System displays mismatch, in bright text, in the Status column.

When the selected bed displays in dim text (i.e., the bed has been held for another patient), the System displays a message indicating the bed is currently being held, and prompts for verification of the removal of the current hold on the bed. The System defaults to no. When the current hold is verified, the System deletes the bed reservation for the previous patient and makes the bed available for a new assignment. Once this process is complete, the System displays the Bed Assignment Accept Screen.

Roommate informationWhen the selected bed is in a room with more than one bed, the System can display roommate information for each occupied bed. The roommate information that displays is maintained only when the Area Table, ^afa, Area or Sub-area level, Roommate Info? field is defined as yes.

The following processing options can display on the Bed Assignment Accept Screen:

Accept bed assignmentChoosing to accept the bed assignment causes the System to assign the selected bed to the patient and delete the request from the bed control queue.

When a mismatch is detected between the selected bed and the patient's requests, the System prompts whether to place the patient on the wait list. The System defaults to no. For a complete description of the wait list see Wait List on page 29.

Change financial bed typeAllows the financial bed type of the selected bed to be changed. Financial bed types are used to record and track the charge information for a bed. The financial bed type might be changed if the patient requested a specific financial bed type, but was placed in an alternate financial bed type because the requested financial bed type was not available for the unit. For example, when a patient requested a semi-private bed, but is placed in a private bed, the patient is charged the semi-private rate when the financial bed type is changed to semi-private.

When this processing option is chosen, the System displays a list of Financial Bed Type Options defined for the room/bed and prompts for one to be selected. The System defaults the financial bed type defined in the Area Table, ^afa, Sub-area or Sub-sub-area level, Default Fin Bed Type field. Financial bed types are defined in the Financial Bed Type Table, ^rt("bf"), and linked to each bed in the Area Table, ^afa, Sub-area or Sub-sub-area level, Financial Bed Types field.

This processing option displays only when the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, System-specific level, Bed Control Information Screen, Use Financial Bed Types? field is defined as yes.

Edit registrationAllows the patient's registration information to be edited. When this processing option is chosen, the System displays the Registration Screen Options included in the registration screen sequence defined in the Registration Screen Sequence push parameter of this menu function. Patient registration information can be entered or edited, and new registration reports can be generated.

The System automatically reverts the bed back to its original financial bed type when the patient is checked out of the bed.

Edit visitAllows the patient's visit information to be edited. When this processing option is chosen, the System displays the Visit Screen Options included in the visit screen sequence defined in the Visit Screen Sequence(s) push parameter of this menu function. Patient visit information can be entered or edited, and new visit reports can be generated.

Wait ListDuring the bed assignment process when a mismatch is detected between the patient's requests and the assigned bed, the patient can be placed on a wait list. A mismatch occurs when a patient is assigned to a bed that does not meet all of his/her requests. Mismatches can occur based on sex, bed control service, bed type, and/or bed preference. The wait list can be accessed from the Wait List Requests queue or the All Bed Requests queue, wait list processing option. These queues can be accessed from the Bed Control Queue Type Summary Screen. When the wait list is accessed, the System displays the list of patients awaiting beds (see the figure below). The user can select a wait list entry to process or choose to find matching beds.

When the user chooses to find matching beds, the System displays a Finding matching beds! message. During this time, the System searches the available beds for a bed(s) that matches the bed request for all patients on the wait list. After the search is completed, the wait list redisplays with an additional column, Matching Beds. This column displays the found beds. When no bed matches are found, the column is blank.

Room/Bed Displays the bed to which the patient is currently assigned.

Type Displays the type of bed request that generated the wait list entry (i.e., transfer, admit, LOA return, visit interrupt return, or discharge reversal).

When the patient has a location defined (i.e., checked into another bed), the System displays current, in bright text, in this column. When the wait list entry is selected to process, the System displays the Bed Request Screen with the current type of bed request in the Request Type field.

Mismatch Displays the mismatch(s) that occurred (i.e., sex, bed type(s), service, and/or bed preference).

Select wait list entryWhen a wait list entry is selected to process, the System displays the Bed Request Screen. This screen displays information about the original bed request. When a new bed is assigned, the bed assignment that caused the wait list entry to be created is deleted. For a complete description of the Bed Request Screen, see Bed Request Screen on page 16.

ReviewAllows a wait list entry to be reviewed. When this processing option is chosen, and a wait list entry is selected, the System displays the Bed Assignment Mismatch Screen (see the figure below). This screen displays the following information about the bed assigned to the patient:

Financial bed type – Displays the default financial bed type of the selected bed. Default financial bed types can be defined for each bed in the Area Table, ^afa, Sub-area or Sub-sub area level, Default Fin Bed Type field. This information displays only when the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, System-specific level, Bed Control Information Screen, Use Financial Bed Types? field is defined as yes. Request/status – The System displays whether the service sex, bed type(s), and bed preference(s) match the patient's requests. When the selected bed matches the patient's requests, the System displays OK, in dim text, in the Status column. When a mismatch is detected, the System displays mismatch, in bright text, in the Status column.

Roommate information – When the selected bed is in a room with more than one bed, the System can display roommate information for each occupied bed.

DeleteAllows a wait list entry to be deleted. When this processing option is chosen, the System prompts for a wait list entry to be selected. Once an entry is selected, the System displays the Bed Assignment Mismatch Screen, and prompts for the deletion to be accepted. Accepting the deletion removes the entry from the wait list, but does not remove the original bed assignment.

Interface HooksWhen a bed request is processed, the System can automatically generate several interface transactions from this menu function. An interface transaction(s) is generated only when one or more of the following hooks are defined:

Bed Control Queue

Purpose Defines the bed control queue that determines the beds that are available for assignment in this menu function.

Input Bed Control Queue Options are defined in the Bed Control Queue Table, ^rt("bcq"). A bed control queue can be selected or the area default queue can be chosen. The System defaults to area default.

System Use When the area default processing option is chosen, the System defines the bed control queue based on the terminal location. The terminal location is defined in the Device Table, ^ad, Location field. The default bed control queue for each area is defined in the Area Table, ^afa, Area level, Bed Ctrl Queue field.

When the bed control queue defined in this push parameter is not linked to the bed control service defined for the patient, the bed request cannot be processed. A bed control queue(s) is defined for a bed control service in the Bed Control Service Table, ^rt("bcs"), Bed Control Queue(s) field.

Bed History by Bed Display

OverviewThe Bed History by Bed Display menu function is a display only menu function that is used to view events associated with a bed (e.g., admission, transfer, discharge).

Function Attributes

DescriptionOnce the Bed History by Bed Display menu function is accessed, the QuadraMed CPR System prompts for a review start date to be entered. The System defaults to today.

Review DateWhen a date prior to today is entered, the System prompts for a review direction, either forward or reverse, to be chosen. The review direction determines the events that display and the order in which they display.

The forward direction uses the review date as a start date and displays all events that have occurred on and/or since that date, up to today's date, in chronological order. The System defaults to forward.

The reverse direction uses the review date as a start date and displays all events that occurred prior to and/or on that date, in reverse chronological order. For example, when the entered date is 10 days in the past (i.e., t-10) and the direction is reverse, only those events that occurred on and/or before that date display.

When the default of today is accepted, the System automatically defines the direction as reverse.

Bed LocationAfter the review start date and direction are specified, the System prompts for a bed location to be entered. A location lookup name can be entered. Location lookup names are defined in the Area Table, ^afa, Sub-area or Sub-sub-area level, Look-up Name(s) field.

Pressing the EXPD key displays a list of area locations that have beds defined. When an area is selected, the System displays a list of defined bed locations for the area. Only locations that have the Area Table, ^afa, Sub-area or Sub-sub-area level, Lookup Flags field, Bed? subfield defined as yes display for selection.

Overflow beds can display on the list of area locations when they are occupied. Overflow beds created during visit processing have a bed history, which lasts as long as the bed is active on the unit. When the overflow bed is no longer needed (i.e., the patient is transferred or discharged), the overflow bed and the bed history are automatically deleted.

Bed History by Location ScreenOnce a bed location is specified, the System displays the Bed History by Location Screen (see the figure on the next page). For each event relating to the bed, the following information displays:

Date/Time Displays the date/time the event occurred.

Event TypeDisplays an activity relative to the specified bed. The following bed history events can display:

OverviewThe Bed History by Patient Display menu function is used to view the list of beds a patient has occupied during the course of a specific visit. This menu function is view-only.

Bed Swap

OverviewThe Bed Swap menu function is used to simultaneously exchange the beds of two patients. This type of transfer is referred to as a bed swap.

Function Attributes

DescriptionOnce the Bed Swap menu function is accessed, two patients must be identified. After the first patient is identified, the QuadraMed CPR System displays the patient's name at the top of the screen and prompts for the second patient to be identified. Only active inpatients with a current home location can be selected. Patients checked into overflow beds cannot be selected from this menu function. After both patients are identified, the System displays the Bed Swap Screen (see the figure on the next page).

Bed Swap ScreenThe Bed Swap Screen displays, in a side-by-side display, pertinent information about the selected patients. This information displays for verification of the correct patients to swap.

Since the Bed Swap menu function is a Reg/ADT event, a bed swap transfer procedure must be defined in the Procedure Table, ^t Procedure Identification Screen. This procedure is linked to this menu function through the System Parameter Table, ^%z, Reg/ADT System Parameters subfile, Facility-specific, Order Comm Procedure Links Screen, Bed Swap Transfer Procedure field.

The fields that display on the Bed Swap Screen are determined by the result profile linked to the bed swap transfer procedure in the Procedure Table, ^t, Documentation Information Screen, Result Profile/Link Information field, Result Profile subfield. The result profile field(s) allows documentation to be entered relating to the transfer (e.g., the transport method and the disposition of the patient's valuables).

Bed SwapAfter all the result profile fields have been edited, reviewed and/or marked, the System displays the two beds being swapped and prompts for the acceptance of the bed swap (see the figure on the next page).

When the bed swap is accepted, the System performs the transfers and generates an unscheduled transfer event for each patient. Census Display

OverviewThe Census Display menu function is used to calculate and display the current statistics regarding the status of all beds within a facility. This menu function includes a breakdown of licensed, transition (i.e., non-licensed), and overflow beds for each unit and provides totals of each bed type by unit as well as the entire facility. Depending on how the Census Display Variation push parameter of this menu function is defined, this menu function can also calculate the percentage of bed occupancy by unit and also the entire facility. Licensed beds are beds that have been approved for the institution by the Department of Public Health. Whether a bed is licensed is determined by how the Area Table, ^afa, Sub-area or Sub-sub-area level, Licensed Bed? field is defined. For example, if the institution is licensed for 50 beds, but has a total of 75, only 50 of those beds should have the Licensed Bed? field defined as yes. Beds for which the Area Table, ^afa, Sub-area or Sub-sub-area level, Licensed Bed? field is defined as no are called transition beds.

DescriptionOnce the Census Display menu function is accessed, the QuadraMed CPR System displays the word Processing. While the System is processing, it scans for the current status of all beds defined for the facility in the Area Table, ^afa. This scan generally takes less than five seconds.

When the scan is finished, the QuadraMed CPR System displays the Census Screen. This screen is a view-only screen.

Census ScreenThe Census Screen lists all the areas containing beds and the statistics calculated for each area. The screen that displays varies depending on the screen variation defined in the Census Display Variation push parameter of this menu function. The following variations are available:

Licensed, Out of Service, Overflow, Total, % Occupancy

Licensed Transition, Overflow, Total The licensed, transition, overflow, total variation displays a census screen with statistics regarding licensed, transition, and overflow beds (see the figure on the next page). The Census Screen for this variation contains the following totals for each unit:

Change Financial Bed Type

OverviewThe Change Financial Bed Type menu function is used to change the financial classification associated with a bed type for a patient. Financial bed types indicate, for billing purposes, a class or type of service required by a patient. Financial systems use the financial bed type to calculate the amount a patient will be charged for the use of a bed and/or for the service associated with that bed.

Function Attributes

DescriptionOnce the Change Financial Bed Type menu function is accessed, the QuadraMed CPR System prompts for a bed location name to be entered. The bed location name that is entered must be the name of an occupied bed. The EXPD key can be pressed to display a list of Location Options. The System only displays those Location Options that have the Area Table, ^afa, Sub-area or Sub-sub-area level, Lookup Flags field, Bed? subfield defined as yes.

Change Date and TimeAfter the bed location name is entered/selected, the System prompts for the date and time of change to be entered. The System defaults to now. A date and time of up to one minute in the future can be entered. The System also accepts the entry of a date and time back to the date and time of the checkin event to the bed.

Financial Bed TypesOnce a date/time is entered, the System displays the Change Financial Bed Type Screen (see the figure below). This screen contains the financial bed type for the patient currently occupying the bed and all the allowable financial bed types defined in the Area Table, ^afa, Sub-area or Sub-sub-area level, Financial Bed Type(s) field for that bed. The Financial Bed Type Options that display are defined in the Financial Bed Type Table, ^rt("bf"). The patient's current financial bed type is starred out (***), making it inaccessible. The System prompts for a new financial bed type to be selected.

After the new financial bed type is selected and accepted, the System updates the patient's record and generates interface records for the financial system.

Interface HooksWhen a patient's financial bed type is changed, the System can generate an interface transaction from this menu function. The interface hook used to create the transaction is the Visit Financial Bed Type Update (V5).

Housekeeping Queue ScreenWhen a bed(s) is vacated due to patient discharge or transfer, the bed is automatically sent to the housekeeping queue and displays on the Housekeeping Queue Screen. Once the Housekeeping Queue Screen displays, the System prompts to select a bed to process. The EXPD key can be pressed to list all the room/beds for all areas that have beds in housekeeping. The area(s) that displays on this screen depends on the definition of the Area Selection push parameter of this menu function.

The following information displays for each area:

Reason Displays the reason the bed/room is needed, as determined by the type of procedure ordered. For example, when the bed is reserved for a patient being admitted, the reason is admit. When the bed is reserved for a patient being transferred, the reason is transfer.

Housekeeping StatusWhen a bed has been vacated, the bed status is automatically changed to HK (i.e., housekeeping). This status indicates that housekeeping services are required. When Housekeeping is finished with a room/bed, it is selected from the housekeeping queue and accepted as complete. After the room/bed is accepted, the bed status changes to VAC (i.e., vacant), and the entry is removed from the screen. The housekeeping status can be seen in the bed control process. For a complete description of the housekeeping status, see Chapter 3, Bed Control.

Push ParametersArea Selection

Purpose Defines the area(s) that displays when this menu function is accessed.

Input The following options can be specified:

System Use Ask user for area – Indicates that the System prompts the user to select an area to view. When an area is selected, the System displays a list of beds in that area that need to be cleaned. Areas are defined in the Area Table, ^afa.

Patient Area LocationWhen the location processing option is chosen, the System prompts for a patient area location to be entered. When the EXPD key is pressed, the System displays a list of Inpatient Location Options defined for the facility in the Area Table, ^afa. After a location is entered/selected, the System displays the Information Desk Summary Screen (see the figure on the next page).

Information Desk Summary ScreenThe Information Desk Summary Screen displays a list of patients assigned to the location. The System displays the following information for each patient:

Patient's home locationPatient's current locationUnit phone (i.e., telephone number of the location)Bed phone (i.e., the home location telephone number)

When a patient has expired, the System displays the last home location for the patient. When no last home location exists (i.e., the patient was never checked into a bed or expired during transfer), the word Expired displays in the Location column.

The location telephone number only displays when the Check Telephone Flag? push parameter of this menu function is defined as yes, and the field created from the Visit Type Table, ^rt("vt"), Flag, Telephone Request data element is also defined as yes.

After a patient is selected, the System displays either the Information Desk Detail Screen or a list of Screen Options. Whether the screen or options that display is determined by the number of Information Desk Screens defined in the Face Sheet Table, ^nt("fs"), Information Desk Screen Sequence subfile.

OverviewThe Out of Service Beds Processor menu function is used to remove beds from service and place new beds into service. A bed that is removed from service is no longer available for selection when assigning or checking a patient into bed.

Function Attributes

DescriptionOnce the Out of Service Beds Processor menu function is accessed, the QuadraMed CPR System displays the areas that are linked in the push parameters of this menu function (see the first figure on the next page). When an area is selected, the System displays the beds that have a status of out (see the second figure on the next page). When a bed has a status of out, it is considered to be out of service. At this point, a bed can be chosen to be placed into service, removed from service or the information about who removed a bed from service can be reviewed.

Place in ServiceChoosing to place a bed into service causes the System to prompt for a bed(s) to be selected and verified. After the bed is verified, it is removed from the out of service beds list, the status is changed to vac and the bed is returned to service. When a bed has a status of vac, it is considered to be vacant. Once placed into service, the bed becomes available for all bed control functions.

Remove from ServiceChoosing to remove a bed from service causes the System to list all the active beds defined for that area (see the figure below). For those beds that are occupied, the System lists the date and time the bed will become available. Selecting a bed(s) from this list causes the System to remove it from service. An effective date and out of service reason must be entered to complete the process (see the figure on the next page).

Effective dateDefines the date the bed is to be removed from service. The System allows a future date to be entered, but will not accept a past date. For beds that are occupied on the effective date, the System waits until the bed becomes vacant before changing the status to out. An out status indicates that the bed is no longer available.

Out of Service ReasonsDefines the reason a bed is taken out of service. The System displays a list of reason options, which are defined in the Out of Service Bed Reason Table, ^rt("bco"). An option can be selected or free-text can be entered.

ReviewChoosing to review an out of service bed causes the System to display the date and identification of the person who removed the bed from service as well as the effective date and out of service reason.

The name of the patient can be specified using standard patient identification methods (e.g., partial name match, soundex, patient number, etc.). Once the patient is identified, the Current Patient Location Screen displays (see the figure on the next page).

A location can be identified using standard lookup methods. Only locations designated as patient care areas (i.e., locations in the Area Table, ^afa, that contain beds) can be identified.

Choosing to identify the patient by location causes the System to prompt for a patient area location name. Pressing the EXPD key causes the System to display a list of Location Options. Selecting an option or entering a location name causes the Current Patient Location Screen to display (see the figure on the next page).

The following information displays on the Current Patient Location Screen:

INFORMATION DESCRIPTION

Location Displays the patients location. The patients location can be one of the following:

For any patient visit type that is checked into a bed, the patient's room number is displayed (e.g., 5102-A).

When the patient is not checked into a bed, the System displays the visit type (e.g., Emergency).

When the patient's visit type is one that cannot be checked into a bed, the patient's status and visit type display (e.g., DIS-OP).

Name Displays the name of the patient.

Number Displays the patient's number.

Destination Displays the patient location when the patient is not currently at their home location (i.e., current location).

Left Displays the time the patient was checked out to be transported to the destination.

Arrived Displays the time the patient arrived at the destination. Once the patient is checked in at the destination, the Left column is cleared and the arrived date/time display.
