ICD-10 is quickly approaching
October 2015
Here are a few reminders for you... • Include the specific information you already know about the
conditions/diseases you are diagnosing in the narrative of your note. (ie: type of diabetes and the manifestations/complications of it; specific site of the cancer and the metastatic sites as well; the acuity and laterality of the disease)
• While picking a very specific ICD-10 code for billing ispreferable, remember that ICD-10 specificity is first aboutdocumentation and second about code selection. A specificICD-10 code without the specifics of that code documented inthe narrative of the note will not stand up against the scrutinyof auditing agencies. The body of the record needs to reflect thespecificity of the code that is selected.
• Please check your in-basket for any queries that the Codingor Clinical Documentation Team may have sent for ICD-10specificity and documentation clarifications.
Mercy welcomes Claudia S. Zavala, MD, Pediatric Hospitalist and Medical Director of Pediatrics
Dr. Zavala received her medical degree from the University of California, San Diego in 1997. Her pediatric internship and residency training was at Kaiser Permanente Oakland Medical Center in Oakland, Calif. Dr. Zavala has been a board-certified pediatrician since 2001. Previously, she practiced as an attending pediatrician in the inpatient and outpatient setting, the emergency department, and the newborn nursery in Bettendorf and Muscatine for 14 years.

2016 Mercy medical staff officers – please vote!The slate of medical staff officers for 2016 recommended by the Nominating Committee is: Gary Schweiger, MD, President; Stan Eilers, MD, Vice-President/President-Elect; and Nicholas Hodgman, MD, Secretary/Treasurer.
The Medical Staff Bylaws state the current year’s vice-president shall automatically succeed the president without the requirement of a vote of the medical staff. Incoming vice-president/president-elect and secretary/treasurer positions require an election by a majority of the eligible medical staff.
Any additional nominations must be received in the Medical Staff Office by Nov. 1, 2015, and must be supported in writing by two medical staff members eligible to vote. Ballots will be sent out to the members of the medical staff eligible to vote by Oct.30, 2015 ..

The Medical Staff Office sends Ongoing Professional Practice Evaluation (OPPE) Reports to active and courtesy medical staff members twice a year. Be on the lookout for your next report to arrive near the end of October or early November. Influenza vaccinations
With the upcoming influenza season upon us, we would like to share with you two new practices Mercy Medical Center has implemented.
1. Increase our inpatient vaccination rates.During the 2014-2015 flu season, we vaccinated 92percent of our patients but we think we can do better.There are ways you can help! When patients are seen forpre-operative visits (either by the primary care provider,specialty provider or by the surgeon) please considerarranging for an influenza vaccine prior to admission.This will reduce the number of patients we need toassess and vaccinate during the busy inpatient stay. Inaddition, please be sure vaccinations are given prior topatients being discharged.
2. Enhance our employee vaccination policy by makingcertain vaccines, including influenza, mandatory except formedical exemptions.We would appreciate your help encouraging staff to bevaccinated and to lead by example by getting vaccinatedyourself. Vaccination will again be provided by Mercy.Please watch for upcoming announcements on dates,times and locations.

Sepsis Reportable Measure (Starting Oct.1, 2015)
Severe Sepsis Reportable Measures (Patients 18 years and older)1. Measure lactate level within 3 hours of time and presentation2. Obtain blood cultures prior to antibiotics within 3 hours of time
of presentation3. Administer broad spectrum antibiotics within 3 hours of time of
presentation4. Re-measure lactate on arrival to ICC5. Required provider documentation within 6 hours of presentation
to the ED (utilize the smartphase "sepsisexam" in your documentation)
Septic Shock Reportable Measures (Patients 18 years and older)1. All of the above criteria from the severe sepsis reportable
measures section AND both of the following:2. Minimum of 30 mL/kg of normal saline required within 3 hours
of presentation3. Applly vasopressors (for hypotension that does not respond to initial
fluid resuscitation to maintain a mean arterial pressure >= 65). Within 6 hours of septic shock patients
