Upon your fulfillment of the requirements below, Tenneco will send you a MasterCard® Card equal to the purchase price of the lowest priced unit, up to our current suggested list price.

2) You must submit the following items (postmarked by November 30, 2015) to receive your Monroe® “SHOCKTOBER®” Consumer Offer MasterCard® Card:

a. Authentic dated sales receipt (copy or original) with business and address clearly identified along with product purchased and price circled. The receipt must verify the date of purchase and that the purchase is paid in full. The name and address information on the sales receipt must match the name and address on this claim form. Altered, forged or otherwise fraudulent proof of purchase documentation will not be honoured. PayPal™ receipts are not accepted. Tenneco has the right to substantiate proof of purchase. The Monroe® or Rancho® part numbers must be identified on the sales receipt. Offer does not apply to transactions when product is being replaced under warranty.

b. The original UPC bar code with part number from the carton or packaging of all qualifying product, if the qualifying purchase is made online. No photocopies accepted.

c. This form, filled out completely and legibly, or go to www.tenneco-canadaoffers.com to submit purchase information. If you submit purchase information online, print your confirmation page and mail with required proof-of-purchase (dated sales receipt and original UPC bar code with the part number from the carton or packaging of all qualifying product, if required) to the address indicated below.

3) Offer submission must be postmarked by November 30, 2015. Tenneco is not responsible for late, lost, postage due or misdirected mail, and all proofs-of-purchase become the property of Tenneco and will not be returned. Please retain copies for your files.

4) Offer valid only in Canada. Offer open to individuals; requests from groups, clubs or organizations will not be honoured. This is an end user offer; dealers, distributors and other resellers are not eligible for this offer. Purchases made on eBay or other similar online auction websites do not qualify for this offer.
