Re: ADDENDA #2 – ITEM #3
Addenda Items being collected in the folder will be formally issued on 9 OCTOBER 2014 by notice to all
registered GCs and Plan Rooms. Parties with access to the Plans and Specifications through registration
may use this material at this time.
A.) Section 01100.1.3.F indicates that the Owner will be utilizing a sitework, concrete, and grading
subcontractor that is already present on site to deliver that portion of the work. Please clarify.
a. To clarify the expectation as to form of agreement and why Construction Manager at
RISK is being considered in conjunction with AIA Document A102 Form of Agreement
Revisions have been made to Specification Section 01100. Please utilize this revised
Specification as a basis for your bid. It is included herein. The primary reason and
directive for this approach is:
i. The Owner has a Concrete Sub -Contractor on site who was going to put in all
new concrete parking lot. This Sub has a contract engaged with the Hospital and
was about to be engaged on working where the building pad is proposed. The
Owner asked that we try to get some kind of a basis for a credit to the portion of
the parking lot that will not be done where the building pad is located so he can
gain this back from the Sub. This Sub may be part of the building project but we
will still want this credit applied. So we are trying to include all utility trenching
shown on the AS sheet, the building pad fill in the Allowance at 1300CY which
should include any excavation, the immediate sidewalk system around the
building, each into the project. So the concrete work per plans and spec is to the
outside curb edge of sidewalks shown on the building plans. Because of the
extensive commitment of the Hospital to the Parking Sub no Civil was engaged.
The Utility extensions are in the project - the parking lot has not been worked
on yet we were able to halt that until this project starts. That is what we have in
this project bid.

Re: ADDENDA #2 – ITEM #4
Addenda Items being collected in the folder will be formally issued on 9 OCTOBER 2014 by notice to all
registered GCs and Plan Rooms. Parties with access to the Plans and Specifications through registration
may use this material at this time.
A.) Should the removal of the existing asphalt, concrete, or other ground cover be included in the
bid amount?
a. Our Bid price contract extends only to include extent of drawings shown. Sitework on
the Architectural Site Plan for Utilities is included. Other sitework beyond the sidewalk
curb edges shown on the drawings are NOT part of the project. See AS1.01 to determine
sidewalk and ramps included, and utility excavation and routing.
B.) Should the amount of excavation for the building foundation be calculated based on the 1300
CY of fill material specified in Allowance No. 3, or based on the minimum bottom of footing
depth indicated on S3.01; Foundation Note 3.
a. Yes. See Bid Doc #2 revised.
C.) Who will be responsible for patching of asphalt, new parking lay-out and restriping?
a. Current Contract outside the scope of this project. It is excluded from the bid.

registered GCs and Plan Rooms. Parties with access to the Plans and Specifications through registration
may use this material at this time.
A.) 2/S2.01: Note 2 indicates no masonry control joints within 2’-0” of openings. However,
architectural plans show all control joints adjacent to windows. This would have a negative
effect on the masonry bond beams above the windows? Please clarify.
a. Yes, this would have a negative effect on the masonry bond beams above the windows.
Therefore, move control joints as shown on ASD-2.0 and ASD-2.1.
B.) Should vertical control joints be utilized at the transition of Split Faced block to Smooth Faced?
a. See above (Answer A)
C.) 8/S2.01; Is the indication of 12” CMU being used above finish floor an error?
a. 12” CMU converts to 8” above footing.
D.) 02/A6.24; 08/A6.62; and 08/S2.01 all indicate the use of CMU Form Block at edge of slab
termination. Is it the intent that Form Blocks be used only at the column locations or could they
be used around the entire perimeter?
a. CMU Form Block at edge of slab termination are to be used as shown on S1.01.
E.) Are the indicated ‘small flush CMU” actually 8 x 4 x 16 split faced block?

smooth face finished. See ASD-2.2.
F.) Please clarify if the 4” batt wall insulation is faced or unfaced?
a. The 4” batt wall insulation is faced.
G.) PEMB Wall girts (purlins) will most likely be about 8”. Drawings indicate the 4” insulation to be
installed between girts. Please clarify how the insulation should be held in place in the 8” cavity.
a. The 4” insulation will be held in place by attaching hexagon chicken wire to the wall
girts.
H.) Please clarify that there is no pre-manufactured metal canopy above Door #139A.
a. Correct, there is NO pre-manufactured metal canopy above Door #139A.

L.) Please clarify if the RTU Mezzanine is to be wood framed as indicated on 04/M2.01, or metal
framed as per 6&9/S2.02.
a. The RTU Mezzanine is to be metal framed as indicated on 6&9/S2.02.
M.) Other than restroom signage, will other interior room signage be required?
a. No, there will not be other interior room signage required.
N.) Regarding the exterior signage shown on Sheet A1.51, there is a conflict. The plans indicate
Gemini as the manufacturer and Minnesota letters as the style, which are injection molded
plastic. It then lists the size and front as 10.5 Helvetica and Architectural Bevel Face. Please
clarify if the letters are to be injection molded plastic or brushed aluminum, and Helvitca or
Architectural font.

Re: ADDENDA #2 – ITEM #6
Addenda Items being collected in the folder will be formally issued on 9 OCTOBER 2014 by notice to all
registered GCs and Plan Rooms. Parties with access to the Plans and Specifications through registration
may use this material at this time.
A.) Several questions have surfaced with regard to the Pre-Engineered Metal Building (PEMB). With
the criteria for the PEMB included in the original construction documents and the addenda, the
structural drawings reflect our best assumptions for the PEMB layout, requirements and scope
of work to be performed. Until the reactions and final PEMB layout is provided by the
manufacturer, including the architectural elements for review, there will be some areas where
assumptions are required to complete your bid. Please do so in conjunction with your PEMB
vendor of choice.
B.) SHEET S3.01 - Special Inspections and Testing: Who is responsible for hiring and paying for an
independent testing company?
a. The Owner is responsible per the IBC.
C.) SHEET S3.01 - Foundation Note 1: Who is responsible for hiring and paying a Geotechnical
Engineer to prepare the soils report?
a. The Owner or Contractor can engage the Geotechnical Engineer.
D.) SHEET S3.01 - Reinforced Concrete Note 3: There are no Civil Drawings. Should a PSI of 3,000 be
used for site concrete?
a. Yes, 3,000 psi should be used for site concrete.
E.) SHEET S3.01 - Reinforced Concrete Note 15: Please clarify where the lightweight concrete and
composite deck are to be installed.
a. LW concrete is not included in this project (it was left over from the mechanical
mezzanine options
F.) SHEET S3.01 - PEMB Note 2: Refers to Detail 15/S2 for Deflection criteria, however, I believe this
to be Detail 01/S2.02. The criteria given assumes all deflections are for members supporting
plaster ceilings, which these ceilings are not. Please clarify.
a. Correct on the detail reference. The second line should read “Non-pilaster ceilings and
include L/240.
G.) Will specifications for the PEMB be provided?
a. The structural portion of the PEMB specifications are included on sheet S3.01. As the
non-structural portion of a PEMB specification to be included in the construction
manual are so complex and inclusive, we typically do not furnish those components
H.) Architectural plans indicate that the exterior PEMB wall panels are 2" thick. Please clarify panel
type and profile.
a. Per the architectural requirements
I.) Please provide details of how pre-manufactured metal canopy is to be attached and anchored
when installed on PEMB metal wall panels.
a. This should be provided by the PEMB manufacturer.

A. The project specifications detail this project pricing as a “Guaranteed Maximum” but Kelly
Turnage in my office who spoke to you about this understood that it is going to be a
“Construction Manager” with a fixed fee. Please advise which is correct.
a. This has been addressed in Addenda # 1.
B. Detail 03/A501 shows the north wall of the new building to be metal building, however, detail
01/A6.24 wall detail has CMU and metal building. Please advise which is correct.
a. If you look at the elevations you will notice it is a combination of materials. The building
to the far right on 3/ A5.01 is an existing clinic building and the finishes are not installed
work. There is a small connector (reference the plans) and it will have a door cut into
the existing building. If you look at the NOTED PLAN A1.51 you will see where
detail 1/A6.24 is cut and at that window it is as shown. See ASD 3.0.
C. There is no specifications for the metal building section 13000. Are you going to provide one?
a. Not Applicable.
