GENERAL NOTES

This project is to remove the temporary relocatable classrooms from the site of Enola MaxwellElementary School that were used as interim housing for Daniel Webster Elementary School, DSA App#01-114446. The removed portables consists of three (3) 24x40 re-locatable classrooms with bathroomsand one (1) 24x40 re-locatable classrooms without a bathroom that were installed under DSA APP #01-114732

Scope of work includes, but is not limited to, the following:

- Removal:built up asphalt landing pads,fencing behind leased buildings,electrical wires & associated conduits & associated conduit supports,all electrical devices,plumbing disconnect water and sanitary sewer & cap,remove wall/doors on 3rd floor of classroom bldg. (patch holes).

- New:extend fencing to create IT bldg and grounds parking lot,slurry asphlat seal on asphalt play yard (play yard side only)patch and repair/paint to match existing at all removed supports

CHAPTER 4 PART 1, TITLE 24 C.C.R.ADMINISTRATIVE REQUIREMENTS(PARTIAL LISTING ONLY)1. A copy of Parts 1 and 2, Title 24, C.C.R. shall be kept on the job site at all times2. Administration of Construction per Part I, Title 24, C.C.R..3. All change orders and addenda to be signed by the Architect and the Owner and approved by DSA. Change

orders are not valid until approved by DSA Section 4-338, Part 1, title 24.4. All tests to conform to the requirements of Section 4-335, Part 1, Title 24, and approved T & I sheet.5. Tests of materials and testing laboratory shall be in accordance with Section 4-335 of Part 1, Title 24 and the

District shall employ and pay the laboratory. Costs of re-test may be back charged to the Contractor.6. DSA shall be notified at the start of construction and prior to the placement of concrete per Section 4-331,

Part 1, Title 24.7. Inspector shall be approved by DSA. Inspector shall be in accordance with Section 4-333(b). The duty of the

Inspector shall be in accordance with Section 4-342, Part 1, Title 24.8. Supervision of Construction by DSA shall be in accordance with section 4-334, Part 1, Title 24.9. Contractor, Inspector, Architect, and Engineers shall submit verified reports(Form SSS-6) in accordance with

Section 4-336 and 4-343, Part 1, Title 24.10. The Architect and the Structural Engineer shall perform their duties in accordance with Section 4-333(a) and

4-341, Part 1, Title 24.11. Special inspection per Section 4-333(c).12. The Contractor shall perform his duties in accordance with Section 4-343, Part1, Title 24.13. The intent of the drawings and specifications is to construct the school building in accordance with Title 24.

C.C.R. Should any conditions develop not covered by the contract documents wherein the finished workwill not comply with Title 24, C.C.R., a change order detailing and specifying the required work shall besubmitted to and approved by the Division of the State Architect before proceeding with the work.

HAZMAT: Naturally occurring asbestos serpentine bedrock and soil has beenidentified to be present throughout the footprint of the site.  For bidding purposes,contractor shall assume asbestos concentrations are >1.0% asbestos in all bedrockand soils at the site.  Contractor shall comply with the requirements identified inCalifornia Code of Regulations, Title 8, Section 1529 for Class II Asbestos Work; CCR,Title 17, Section 93105 Asbestos Airborne Toxic Control Measure (ATCM); Bay AreaAir Quality Management District Regulatory Advisory; and in Section 02080, SoilSampling and Analysis Report and Geotechnical Soils Report located in the ContractDocuments when disturbing NOA soils / bedrock at the site.  Contractor shall disposeof all excess soils at a permitted Class II / III Waste Management Unit, granted that allnecessary and additional stockpiling and hazardous waste characterization results arebelow acceptable regulatory limits and waste management unit acceptance criteria.  Ifadditional sampling is required by the designated waste management unit for disposaland acceptance purposes, the contractor shall provide these additional requirementsin writing to the District’s Consultant and allow a minimum of 14 days for testing andreporting.  Contractor must provide a letter of acceptance or approved profile withauthorization number from the Class II/III Waste Management Unit to the District priorto removing any soils from the site.  Under no circumstances will the District allowexcess soils to be reused at another site or transported to a recycle facility for resaleand reuse.
