This Community Health Needs Assessment (CHNA)

is an overview of the health status of those who

live in the Avondale, North Center, Lakeview

and Lincoln Park neighborhoods of Chicago. A

collaborative team of community stakeholders—

from social service agencies to business leaders—

came together to gather and analyze health-related

information, identify the most pressing needs and

pinpoint ways to make improvements. Please turn

to page 15 to learn more about this process.

Access to care.

1 in 10 people use the ER as their primary source of health care.The majority of survey respondents say that

their immediate family goes to a doctor’s office

for a medical health problem, but almost 12% said

they go to the ER. Lincoln Park had the highest

percentage (20%) using the ER, while Lakeview

had the highest percentage (83%) who seek care

at a doctor’s office.

Chicago has a higher than average number of uninsured people at almost 20% compared to 15% nationwide.

In the Presence Saint Joseph Hospital service area, 19% are uninsured.

In Avondale, there is a shortage of primary care physicians.

All areas have a shortage of mental health services for low-income residents, consistent with most neighborhoods in Chicago.

What’s preventing people from receiving care?

Where does the community come up short?

1 in 5 felt that cost prevented them from seeing a doctor.

1 in 10 said that there were no providers available based on their insurance or lack thereof.

Lakeview, Lincoln Park & North Center have a much lower percentage than average enrolled in Medicaid while Avondale’s rate is significantly higher.

But unlike the rest of the country, diabetes

is among the top 5. Of note, in Avondale,

unintentional injury is one of the top five

causes, while lung cancer is not.

Cancer and heart disease are the top two causes of death, both here and in the U.S. Years of Potential Life Lost (YPLL) defines what

is killing residents prematurely and can help

determine prevention efforts. Homicide and liver

disease are leading causes in Avondale, while

suicide is among the top in other areas.

Lakeview has the highest suicide rate in the area—higher than the Chicago average.

Lakeview has a very high rate of annual HIV infection and prevalence of HIV/AIDS far above the city rate.

The annual rate indicates new cases per year and prevalence

shows how many individuals are living with HIV. With newer

treatments, people are living much longer, so a high prevalence

rate may indicate a high risk for infection, but may also signify

success, in that people are getting the treatment they need.

The teen birth rate has gone down in all areas, although Avondale’s is still above the Chicago average and Lincoln Park’s is especially low.

16.7% have been told they have asthma, more than the city, state and country.

All 4 communities had fewer asthma hospitalizations than the national rate.

Compared to the rest of Chicago, the Presence Saint Joseph Hospital service area has a relatively low crime rate.

Homelessness is a major challenge.Although exact numbers are difficult to confirm, the Chicago Coalition for the Homeless

estimates over 100,000 Chicagoans were homeless during the 2011-12 school year.

Barriers to health.

People in Avondale are more than twice as likely to experience food insecurity as others in the area.

This means that almost 20% of Avondale residents

either do not have enough to eat or don’t know

where their next meal is coming from.

Measuring our health.This Community Health Needs Assessment is the result

of a partnership between Presence Health, the Illinois

Public Health Institute and a diverse group of community

organizations and local leaders. In July 2012, a Steering

Committee was created to address the overall goal of

improving the health and well-being of the community

with a special focus on the needs of the most vulnerable.

The Committee examined a wide range of factors that

impact health, from the causes of disease and death

to income and unemployment rates. Statistics came

from sources like the U.S. Census and the City of Chicago.

Direct input from residents was also obtained through a

community survey that explored perceptions about quality

of life and barriers to health. Gaps in services were identified

and prioritized needs were determined (see page 2).

Taking action.Based on these priorities, Action Teams were formed to

develop goals and objectives that build from existing efforts

in the community. The result is targeted action plans that

help place resources where they are most needed and can

be the most effective. The Steering Committee will continue

to provide support and oversight of these plans as part of

a long term, comprehensive effort for improvement.

Residents who pay more than 30% of

their income on housing are considered cost

burdened. Renters were more likely to be

cost burdened in all areas except Avondale.

At least 1/3 of residents are cost burdened by housing, 1/2 in Avondale.
