Anyone that is uninsured without prescription coverage, and for people not eligible for Medicaid, Medicare Part D, veterans benefits, or any other government program. Household (HH) income is less than one of the following income guidelines:
HH of: HH Income less than: 1 $27,075 2 $36,425 3 $45,775 4 $55,125 5 $64,475 6 $73,825
There is no cost to qualify for free medications.
The annual registration fee is $25 per year, per person, and it is non-refundable. Medications are normally shipped to last 3 months and there is a $40 per month fee for all medications requested, no matter how many medications are needed.
Some pharmaceutical companies charge a co-payment on certain medications, notification will be provided at the point of application.
