Page 9: “Preparing Your Shop To Handle Pay For Performance”

Environmental Background

• May 2001 – Joint Commission announces the four initial core measurement areas for hospitals

• Measures are grouped in 4 categories (AMI, PN, CHF, and SCIP)

• February 2009 – ARRA adds meaningful use quality measures

• July 2011 – CMS VPB program goes into effect (Core Measures & HCAHPS)

• Regulatory & Quality reporting demands will grow

Page 10: “Preparing Your Shop To Handle Pay For Performance”

Observations

• Reporting environment is highly decentralized, no central accountability over enterprise reporting

• Multiple databases and reporting tools in use

• Data definitions are not well documented and multiple feeder systems require identification of ‘single source of truth’

• Not all data to meet required reports is captured electronically at the source

• No complete catalog of current report output

Page 11: “Preparing Your Shop To Handle Pay For Performance”

The Project – C&BI

• Visualize the desired end-state

• Take stock of existing systems & tools

– Create a comprehensive data dictionary with detailed field definitions

– Catalog current reporting from all systems and (re)evaluate and document report owner & purpose

• Identify technology partner(s)

• Establish governance structure

• Validate end-state vision

Page 12: “Preparing Your Shop To Handle Pay For Performance”

The Project – C&BI

• Identify & detail all reporting needs across enterprise (clinical and financial)

– Operational (reports, dashboards & scorecards)

– Strategic (ad hoc queries, trend analysis, modeling)

• Perform analysis to identify gaps in available data

• Develop remediation plan for identified data gaps

• Perform prioritization exercises to establish realistic roadmap to define timelines and manage customer expectations

Page 13: “Preparing Your Shop To Handle Pay For Performance”

Prioritization

• Value Based Purchasing (VBP)

– AMI, HF, PN, SCIP

• Hospital Acquired Conditions

– VTE, Pressure Ulcers

• Readmissions

• Selected operational dashboards

– looking for early success

• Resources are Limited – Prioritization is critical

Page 14: “Preparing Your Shop To Handle Pay For Performance”

Activities & Decisions

• Leverage existing investment in Siemens products including Soarian Clinicals, DSS and SQM

• DSS will be used as the data repository and will evolve with Siemens move to a comprehensive BI infrastructure

• Front end dashboard(s) are being evaluated – Usability will be a key driver

• Governance structure established, data mapping is underway

Page 15: “Preparing Your Shop To Handle Pay For Performance”

Final Thoughts

• Analyze benchmark data with process re-engineering utilizing tools like the workflow engine to ensure the right care, with the latest guidelines are prospectively being delivered to the right patients while ensuring the data is captured accurately at the point of care

• As the streams of clinical and financial data flow into an Enterprise Data Warehouse, the ability to then exploit the data for the right end users (Business Leaders) across the Enterprise – CEO, CFO, CMO, CxO…

• The path to empower end users to make business decision via actionable information
