Preface – A journey of 15 years Gram Seva Trust has completed fifteen years in the service of the poor in the Gandevi Taluka of Navsari district of South Gujarat. We feel extremely happy and proud to share highlights of activities of past 15 years along with annual report of 2008-09 with our esteemed donors, well wishers and friends who have been our co-travelers in this journey making substantial contributions all along.

GST took birth in 1994 with an aim to serve the poor and outcasts amongst the landless tribal communities in and around Kharel. It has since completed 15 years from that infant step. First 15 years represent childhood in the life of an individual when he or she is preparing to finish the school but for an organization, fifteen years show its journey towards stability in the field and a proof of its commitment towards the stated mission and goals. It is a long time, therefore, to introspect while celebrating this milestone. It is time to take an account of our shortcomings and incomplete missions and chart the future di-rections. Some projects like quality health care delivery, nutrition project to meet the chal-lenge of severe malnutrition amongst the children and building partnerships with various schemes in public health domain of state and central government have given us immense satisfaction.

We have taken great inspiration from the Gandhian thought where the village development is central to national progress and prosperity. A comprehensive approach is, therefore, necessary for alleviating poverty, promoting value based education and empow-ering the community ridden with caste prejudices. The community needs to awaken itself from the deep slumber of ignorance, illiteracy, addictions, superstitions and a fatalistic ap-proach to realize its full potential and gain self esteem and self confidence that has been a victim of years of exploitation.

Women health and empowerment have remained our natural goals for we believe that women, through education and financial emancipation, can lift the entire community to a different level with improved standard of living.

All through its inception we have received tremendous love and trust from many friends spread all over at home and abroad; whatever we have achieved so far is largely due to their unstinting support and generous donations. We remain grateful to all of them and seek their blessings and support as we continue the journey ahead. Their personal commitment to the betterment of health of poor has given us many excellent opportunities. Our achievements are also due to the dedication and teamwork of our doctors, paramedi-cal staff and trustees.

We are aware that challenges will be tough as expectations increase; we will strive hard to honor the faith reposed in us.

Introduction Gram Seva Trust (GST) started in 1994 at Kharel in a 30 bedded hospital run earlier by a local trust. The hospital was in a dilapidated condi-tion with several constraints. The mission was to cater to the health needs of the community in the surrounding vil-lages comprising of mainly Halpatis, who are lan-dless tribal population. At the same time, we were looking beyond health to uplift the social and eco-nomic condition of these people. Women and chil-dren have remained our special focus to promote health awareness and get a fair share of any economic activity initiated in the programmes of GST. Our New Initiatives for this mission are SNGO Project and Balawas( a Children’s Shelter Home)

Service NGO Program GST was selected as service NGO under the RCH-II (Reproductive Child Health) program of the Central Government. The organization has to pro-vide RCH services to the selected under-served and/or under-utilized areas with the objectives of providing antenatal care to all pregnant women, increasing institutional deliveries to 96% and de-creasing malnutrition in children under 3 years by 10%. Nine villages of Chikhli Block of Navsari District were selected for this project. The health workers work extensively to spot pregnant women, convince them for proper antenatal care and institu-tional delivery. They also take care of the new born

with positive impact on lowering maternal and neonatal morbidity and mortality. The organization has achieved the required objectives in this project within one year. This was recognized by The Health & Family Welfare Department of Government of Gujarat and Gram Seva Trust was felicitated with the “Best NGO award” on 07/04/2009. This award is also a reminder that the mission is not complete but it is now our duty to sustain and strengthen our work.

Education Education is central to the development of all human potential. Despite being a fundamental right through legislation since 2002, quality education has eluded many a children in disadvantaged areas where government schools are the leading providers of educa-tion. Moreover, poverty and search of livelihood result in large number of school drop-outs. Quality of edu-cation in government schools leaves much to be desired. 3 Tuition class-es were started in 2006-07 to fill the gap in quality where children come before or after their regular school hours. This proved popular with chil-dren and parents alike and now there are 14 such classes catering to 355

children. Constant efforts are made to innovate on methods of teaching by holding teacher’s training programs with aim to reduce the gap between learning and education by rote.

Bal-Awas One new initiative in providing education is Bal-Awas, a shelter home for deprived children. It was started on 31st July 2008 in a rented house to provide residential and educational facilities to children who are either orphans or whose parents are migrant workers and some who are brutally abused at home by alcoholic parents/relatives. At present there are 22 children, 9 boys and 13 girls. They all go to the regular school. The children have had poor learning skills so far with no great interest in stud-ies. There is a residential care-taker with one helper to tend to daily needs of these children. The wholesome balanced meals are sent from GST kitchen. There were some teething problems as children came from diverse backgrounds and some were deeply affected by the environment at home. It is heartening to see the wonderful change in these children where they look forward to everyday with hope and opti-mism. 10 day Diwali Vacation Camp was organized for these children. Dr. Kantibhai Desai and Rashmiben Desai , edu-cationists in Gandhian model , stayed with the children in Balawas and conducted various activities for the children like yoga, lifestyle education, action songs, story telling, dra-mas, drawing, paper craft, needlework, games, exhibitions, excursions, visiting local craftsman, science experiments, puppet show etc. Children thoroughly enjoyed this unique experience.

A look at our ongoing programmes –

Hospital Services General medicine along with Obstetrics and Gynecology were the initial departments that started working in the hospital. Over the years several new services were added along with basic

facilities of the laboratory, X-Ray, Sonography, op-eration theatres labour room and physiotherapy. There are general and specialty out patient clin-ics besides 50 bedded indoor patient facility with 4 general wards, Special and Deluxe Rooms, Inten-sive Care Unit and Neonatal ICU. Laboratory and operation theatres have been up-graded with sophisticated equipment.Thus the hospital provides basic primary and sec-ondary medical facilities to all the poor villagers in the surrounding area. Emergencies like accident patients and snake bite patients are also attended to. Patients of snake bite are given free anti snake

Sanjay and Meeta (Names changed) were children of an alcoholic father. Their mother had committed suicide in front of them. Nobody looked after these two children except for a handicapped paternal aunt. They were admitted in the Balawas. They were the most unruly children of Balawas. Everyday they had fights with one or the other children. They gave lots of bad words. They always wanted to do something different than others. When all the other children are saying their prayers with closed eyes these two will be the ones who are fidgeting, playing pranks on the others. If anybody scolded them, even in school, for not doing their homework they would try to run home. Thus this pair of brother and sister troubled all. It always made us think why these two behaved thus. Did they behave like this because they did not get any love and attention at home? Slowly as they settled in the congenial atmosphere of balawas, where, they received love, affection and caring and monitoring, with children of similar age and background, they mellowed down. There is a remarkable change in their behaviourThis makes us hopeful that by giv-ing the right environment we can try to make their lives happy and look towards a better future.

The organization also provides educa-tion regarding prevention and treatment of snakebite to pa-tient and relatives, through books, charts and exhibition of snakes. More and more snakebite patients are thus coming to the hospital for treatment. Obstetric & Gynecology services are most important in impacting the health of mother and child. The antenatal clinics run four times a week where poor expectant mothers are seen free of charge. Sonography is done every Tues-day afternoon to monitor the progress of pregnancy and fetal well being. These women are followed up in the village by the health workers and are given health education regarding safe and efficient motherhood. The Chiranjeevi Scheme has contributed to a substantial increase in hospital deliver-ies. This involved extensive mobilization and hard work by our health workers in educating the community about safe motherhood and child welfare ensuring healthy mother and baby. The hospital deliveries are impressive 96%. A full time pediatrician and neonatal ICU are at hand to take care of any emergency. We are happy to have with us Dr. Preeti Patel ( full time Paediatrician), Dr. Kanchan Dhadwe ( full time Gynae-cologist and Obstetrician) and Dr.Modi ( Surgeon), who are

dedicated and sincere people with empathy for the plight of the poor. Due to the hardwork of our team of such dedi-cated doctors, paramedicals, community workers and sup-port of government schemes like Cheeranjeevi Yojana, 108 emergency ambulance service etc, we were able to conduct the highest number of deliveries under Cheeran-jeevi Yojana in South Gujarat. Government of Gujarat has felicitated GST for this achievement in May 2008. GST is also recognized as an ICTC (Integrated Centre for Training and Counseling) and PPTCT (Pre-vention of parents to child transmission ) Centre for AIDS ( Vatsayan Kendra) giving pre-test and post-test counseling where required. Those willing and if found pos-itive, treatment is provided to the mother to prevent trans-mission of virus to the child.

Ma Annapurna Bhojanalya Ma Annapurna Bhojanalya or the Hospital Kitch-en provides free meals and milk to the patients and one at-tendant . Hospital staff, patients in private rooms gets food at subsidized rates. The kitchen caters for many training programs conducted at GST.51119 Free meals were provided during 2008-09.

Community Health, Education and Development The major cause of rural misery has been the grinding poverty and economic backwardness of the com-munity. Similarly the benefits of public health system, though well planned on paper, have not reached the poor. GST recognized this need and has been working towards improving the living conditions of the poor.

Child Health and Nutrition Project The mission is to monitor health care of children in 0-6 years age group, prevention of malnutrition and promotion of health for an optimum physical and mental growth.

Rekhaben had lost her previous two children. Her first delivery was preterm delivery in a private hospital in Navsari. She was then delivered by caesarean section because the baby was lying transversely. The newborn was kept in the NICU for 12 days then discharged. They did not go for any follow up visit to the doctor. Later when the baby was 3 months old it started convulsing. The doctor in Navsari advised them to go to Surat for further treatment, but due to monetary constraints they did not go and lost the baby.In her second delivery she gave birth to a stillborn. The baby had already died in the mother’s womb. This was her third and precious pregnancy. She had taken complete antenatal care at a nearby government health centre. When her delivery pains began she was taken to the same centre. There was no doctor or nurse at the centre and the hence she was taken to Navsari Civil Hospital by 108 Emergency Ambulance. There too the gynaecologist was unavailable. The 108 attendents then brought her to Kharel hospital. This time the baby was in breech position and already a foot had come out. She was immediately taken for caesarean section. Both the mother and child were fine after delivery. Thus Rekhaben’s precious child was saved just in time.

We monitor health and nutrition of about 3000 children less than 6 years of age. This is largely done through 83 anganwadis in 24 villages. A total of 522 checkups were conducted this year. These children are provided healthy and nutritious meals along with elementary education. The malnourished children are given high protein, high calorie powder (Hyderabad Mix) along with vitamins and iron supplements. We have been able to reduce malnutrition by 8% this year. The incidence of severe malnutrition (Grade IV, III) has also come down significantly except in children from migrant families. One Anganwadi in each village caters to popu-lation of 800-1,000; if the distance between Falias is more, additional Anganwadi is actively con-sidered. The activities include monthly monitoring of nutritional status, growth and development of children in 0-6 years age group besides imparting pre-school education by trained workers. Home visits by health workers, supervisors, interns and doctors to reinforce personal and social hygiene, good health practices and help in any other possible way. MMR and Hepatitis B vaccines are given to Anganwadi children as well as other children and adolescents since these vaccines are not covered under National Vaccination program.

Heart Disease Project Patients suspected to have heart dis-ease are seen by Dr. Ratna Magotra during her visits to Kharel and advised further investigations and/or treatment. A 2-D Echocardiography camp was held where Dr. Snehal Kulkarni of Wock-hardt hospital, Mumbai scanned 28 children, 7 amongst children had rheumatic heart disease while 4 were diagnosed with congenital heart dis-ease. We deeply appreciate Dr. Rajesh Desai of-fering the facility for 2-D echocardiography at his clinic in Navsari. Operated patients are regularly followed up at Kharel itself. Two children have successfully undergone open heart surgery this year for congenital heart disease.

Health Education Main thrust of Gram Seva Trust activities is on education and awareness amongst people about health. Hence we conduct various health education sessions in the community. Workshops for adolescents and newly married couple are also arranged.

Sneha studied in std.5. She lived in an interior village of Mahuwa Taluka with her parents who were poor farm laborers. She suffered from frequent body aches and pains. Initially she was treated by local doctors for these pains. Then she was diagnosed as patient of Sickle cell Disease in one of her school checkups. This is a genetically transferred disease of the red cells, mostly seen in tribals. Such patients need lifelong regular medications and care. Sometimes they suffer from crisis and need immediate treatment. Sneha was recently admitted to our hospital and given two bottles of Blood. She has received such blood transfusions 5 to 6 times. Today she is studying to become a primary school teacher and comes for regular follow up.

a) Health Education Sessions There were 68 night programs using posters, charts and audio-visuals. 106 antenatal sessions, 06 eye camps were also utilized for health educa-tion besides two sessions for the staff.b) Newly married couples workshopThis year two workshops were arranged for newly married couples. These workshops are arranged to bring awareness amongst couples regarding re-productive health and gender sensitization. This is done through different mediums like games, audio-visual aids and lectures.c) Adolescent Awareness Programs Adolescence is the age of change, confusion and

curiosity. There is a great need for education and counseling of adolescents regarding healthy life-styles, as many may agree. Gram Seva Trust strongly believes this and has always taken different initiatives to improve adolescent health like education, checkups and Hep.B and MMR vaccination. To be able to reach a large group of adolescents, this year we have trained few of our RCH workers in conducting health education sessions for adolescents. These workers conduct 2 hour ses-sions for adolescents for a minimum of 3 days. Topics covered in these sessions include personal and menstrual hygiene, age of marriage and child birth, reproductive tract infections, nutrition etc. This year 50 no of sessions were conducted and 515 no. of adolescents participated in them.

Community Health Workers Program We owe the success of the community programs to our field workers and Community Health Workers (CHW).

Barkha Kataria, a 14 year old 8 std. student, came for her school health checkup at Damdaccha High-school. All girls were anxious about the checkup. Barkha's eyes also showed the same anxiety. An abnormal cardiac sound was detected on her physical examination. She was hence referred for a car-diologist checkup and 2D-ECHO examination at Kharel. The cardiologist diagnosed Atrial Septal Defect (ASD) a congenital heart defect, which required surgery. This was a shock to her poor parents, who were unaware about this condition for so many years. Barkha also had not developed any symptoms yet. It was difficult for the parents to accept that their child had any defect. They did not turn up for any follow up. Repeated reminders to school authorities were of no use. We thought that we have lost this child as the parents would not bring her. Thus one and a half year passed by and suddenly one day Barkha visited our hospital with her father. They were now ready for the operation. After the required paperwork was completed, Barkha got admitted in a cardiac hospital in Mumbai and was operated successfully. Today she is preparing for SSC examinations.

It was evening time in Vanzana village, harijanwas falia, where about a 100 families of Halpatis lived. Most of the inhabitants were watching a film on TB. Recently an old man in the falia was diagnosed as patient of Tuberculosis. Hence our workers had arranged this night programme. The film explained the cause of TB, its effects, how it spreads, how it can be detected, what is its treatment, how it can be prevented and other facts about TB. The film had a profound effect on the villagers and they unanimously agreed that action should be taken to prevent the spread of this disease. The next day many parents came to Kharel with their children to test for TB. All those children who were positive were given treatment from the Primary Health Centre run by the government. The rest were followed regularly. All adults suffering from any symptoms were surveyed and a TB detection camp was organized. Those positive were also given treatment from the PHC. The patients have good compliance and are taking their treatment regularly.Thus a small film had such a great impact on the community and brought about great awareness in the people.

49 CHWs are involved at village level with registration of pregnancies, deliveries, new-born care and maintaining morbidity and mortal-ity records. They also follow 0-3 year olds closely to ensure good weaning practices and carry out health education programs for adolescent girls; they follow T.B patients and other specific groups as and when so identified.

Women Activities The women and adolescent girls of the surrounding villages are given training in tailoring and other handicrafts. They are also given train-ing in making shampoos, hair-oil, detergents and washing powders. The products made by these women are sold through two retail outlets and also in different fairs and exhibitions. Through this programme needy women can earn Rs.1000 to Rs.2000 per month. We have formed women micro credit groups in the villages to cultivate the habit of sav-ings in the village women. The groups are also a source of micro loans whenever needed as such micro loans are not provided by banks. This also saves the village women from the traps of money lenders.

Our laboratory assistant Prakashbhai met Rajubhai aged 17, a farm labourer educated up to 8 std, at a funeral. He looked very pale. On enquiry Raju admitted that he felt very weak, was breathless on exertion and poor appetite. Yet he went regularly to work and had not informed his parents. Pra-kashbhai advised his parents to bring him to Kharel hospital for further checkup. On investigation his haemoglobin level was only 3.2gm%. He was given two units of blood and iron tablets along with nutritional advise and then discharged.

There are 108 Self Help Groups (SHG) being run and managed independently largely by the women themselves. The total savings exceed Rs. 1,500,000 while internal lending is around Rs. 2,900,000. Several members of these SHGs have contributed actively in not only in the well being of the family but also in creating as-sets. It needs a special mention that encouraged by the women groups, five male groups have emulated them and started SHGs.

Satellite Centre Dang is a hilly, forest, tribal District of South Gujarat. The tribal people of Dang district do not have good health and transport facilities. At times they have to travel 100 Kms or more to get treatment for even minor

Kusumben Halpati aged 45 years developed a small ulcer on her tongue. She was referred to Surat by the PHC medical officer. Biopsy report in Surat showed that she suffered from cancer of tongue. Kusumben was not chewing Ghutka or any other tobacco product. She was perplexed as to how and why she should suffer from this disease. Again the PHC medical officers advised her to go to Surat Civil Hospital for further treatment. But her husband refused to take her to Surat. The poor lady attended various medical camps to get some treatment. Everywhere she was advised to go to Surat Civil Hospital. Her husband had now started a relationship with another women and completely refused to take her. She started some herbal therapy from nearby village. It too did not have any effect. Her condition slowly deteriorated and at last she succumbed to her illness 2 years after being diagnosed.

ailments. They ignore the illness in early stages for lack of money and seek treatment at an advanced stage. A satellite centre was started to reach these people near Vaghai in Dang district. A medical team of doctors and paramedical workers visit this centre every week to provide medical and labora-tory facilities to these tribal people at a cost they can afford or even free to some. 150 to 200 tribals benefit from this outreach programme every week.

15 year old Hitesh studing in std.5 was brought to Kharel hospital from a village 20 kms. from Kharel by 108 Emergency Ambulance. He suffered from severe abdominal pain since the previous night and by morning had nearly lost his consciousness. Some villagers advised his parents to take him to Kharel and thus he was brought here. His blood investigations showed that his sugar was vey high, more than 500m.g. and hence he was suffering from its complications. This was a serious condi-tion and could also lead to his death. He was given intensive care in the ICU. Slowly his condition improved and on complete recovery he was discharged. Juvenile Diabetics like Hitesh require daily insulin injections which his poor parents cannot afford. Today the organization provides these injec-tions at 50% charges. Such patients require 2 to 3 such vials every month, each vial costing Rs.140/-

Eye Camps Large number of patients suffering from cataract and other eye ailments benefit through the Shri Sukhabhai Govindji Patel Eye department of our hospital. 8 eye camps were held this year where 1270 patients were examined. 173 pa-tients were operated upon and 589 received spectacles free of cost.

AwardsIn consideration of our activities, the Health & family welfare department of Government of Gujarat felicitated Gram Seva Trust with the “Best NGO award” on 07/04/2009. The Award was given by the Gujarat State Health & Welfare department for best performance in Health services in year 2008-09 at state level to achieve the goals of National Health Programme in NGO cadre of Gujarat. The Award was conferred upon Gram Seva Trust by Honorable Health Commissioner Dr. Amarjeet Singh & the Secretary Dr. Ravi Saxsena. Both dignitaries con-

gratulated the trust board & the Team of the Gram Seva Trust. Gram Seva Trust was also felicitated with the award for maxi-mum number of deliveries under the Cheeranjeevi Yojana, in South Gujarat on 31st May 2009 by the Honorable Health Commissioner Dr. Amarjeet Singh. The Gram Seva Trust Team is delighted to receive these awards. These awards have helped to increase the energy of the team members in their participation in the progress of the trust & services for the rural population.
