    Postoperative care is the

    management of a patient aftersurgery.

    This includes care given during

    the immediate postoperative

    period, both in the operating room

    and postanesthesia care unit(PACU), as well as during the

    days following surgery.

    PURPOSE

    The goal of postoperative

    care is to preventcomplications such asinfection, to promote healing of the

    surgical incision, to return thepatient to a state of health.

    TRANSPORTING THE PATIENTFROM

    OPERATING ROOM

    In moving the patient from the operating

    table to the stretcher , the operating room

    personnel strive to avoid the following

    three problems:

    1. Exposure which predisposes the

    patient to respiratory infections and shock.

    2. Rough handling, which may place astrain upon the patients sutures.

    3. Hurried movements and rapid

    changes in positions, which

    predisposes the patient to hypotension.

    TRANSFER OFPATIENT TO

    POST ANESTHESIA CAREUNIT

    Communicates intraoperative information:

    Identifies patient by name.

    States type of surgery performed.

    Identifies type of anesthetic used.

    Reports patients response to surgical procedure

    and anesthesia.

    Describes intraoperative factors (e.g., insertion of

    drains or catheters, administration of blood,

    analgesic agents, or other medications duringsurgery, occurrence of unexpected events.

    Describes physical limitation.

    Reports patients preoperative level of

    consciousness

    RECOVERY ROOM

    The Recovery Room is a very safe place tobe directly postoperatively. Nurse will

    provide attention to the needs and assist

    patient in awakening from surgery safelyand comfortably.

    Because the immediate post anesthesia

    period is a critical time for the patient,

    close observation is important. The

    patients physical and physiological

    functioning must be supported until the

    effects of anesthesia have worn of.

    POSTOPERATIVEASSESSMENTRECOVERYAREA

    Determines patients immediate response tosurgical intervention.

    Monitor patients physiologic status.

    Assess patients pain level and administers

    appropriate pain relief measures.

    Maintains patients safety(airway, circulation,

    prevention of injury)

    Administer medication, fluid and bloodcomponent therapy, if prescribed.

    Assess patients readiness for transfer to

    inhospital unit or for discharge home based on

    institutional policy.

    DISCHARGE FROM THE RECOVERY ROOM

    The Physician either the surgeon or anesthesiologistgenerally discharges the patient from he recovery room.

    Common criteria used in the evaluation of patient

    discharge readiness are:

    1. The have recovered from the effects of the general

    anesthesia. This usually requires 2hours stay in therecovery room.

    2. The vital signs must be stabilized.

    3. There must be no excessive drainage from any site or

    body cavity.

    4. The physiologic effects of any narcotic medication must

    have stabilized.

    5. The patient must have gained consciousness, and the

    level of consciousness must be satisfactory.

    6. All essential orders must have been carried out

    completed by the recovery room personnel.7. Should the situation arise, provisions must be

    made for attendance by a private duty nurse on

    the clinical unit where the patient is to be moved.

    8. The urine output must be adequate (30cc/hr).

    The amount should be noted and recorded.

    9. The staff of the clinical unit to which the patient

    is to be transferred must be alerted and must beprepared for receiving the patient.

    Continues monitoring of patientsphysical and psychological responseto surgical intervention.

    Provides teaching to patient duringimmediate recovery period.

    Assist patient in recovery and

    preparation for discharge home.Determines patients psychological

    status.

    Assist with discharge planning.

    or clinic visit or by telephone contact.

    Reinforce previous teaching andanswer patients and family

    questions about surgery and follow-

    up care.

    Assess patients response to surgery

    and anesthesia and their effects on

    body image and function.
