FAMILY EDUCATIONAL RIGHTS & PRIVACY ACT (FERPA), RELEASE OF PUSD DIRECTORY INFORMATION – I permit the release of PUSD’s directory information regarding my student. PUSD directory information may include my student’s name, parent’s name, address, e-mail address, telephone number, major course of study, participation in officially recognized activities and sports, awards, and school most recently attended. This information may be released to requesting agencies with a legitimate educational interest, universities, other school districts, and the school’s PTA and Foundation. P.U.S.D. STUDENT ACCIDENT AND HEALTH INSURANCE - As parent/guardian of the named student, I understand that Poway Unified School District does not provide medical or dental insurance for student injuries but does make voluntary student insurance available for purchase. I also understand that State law requires my student to have health insurance in order to participate in school sports. I have read the Superi ntend en t’ s lett er regarding accident insurance.

Mt. Carmel High School STUDENT HANDBOOK ACKNOWLEDGEMENT – 2017 - 2018

The student handbook is a document regarding the attendance and discipline rules, and policies of Mt. Carmel High School. Prior notification will be given if any of these policies require modification.

By signing this page you agree to access the handbook, read it in its entirety, and become familiar with school and district policies and procedures.

The following violations are considered extremely serious and have been included on this page as an additional caution to students and parents/guardians. Suspensions, transfers, or expulsions will result on the FIRST OFFENSE and are not limited to the following acts:

1. Possessing, selling, or furnishing to others any firearms, knives, explosives (including firecrackers), or other dangerous objects such as, but not limited to, brass knuckles, razor blades, any pellet or pellet-type guns, paintball guns, lasers, pepper spray, or instruments expelling projectiles.

2. Possession of an imitation firearm.

3. Transferring, selling, distributing, offering, arranging, or negotiating to sell, or possessing quantities sufficient to suggest the intent to provide, give, or sell to other students substances which are, or are purported to be, alcohol, narcotics, dangerous drugs, unauthorized prescription medications, other controlled substances, or intoxicants of any kind, including over-the-counter medications which create an intoxicating effect.

4. Possessing, using, having consumed, or being under the influence of alcohol, narcotics, dangerous drugs, or other controlled substances or intoxicants of any kind.

5. Committing or attempting to commit a sexual assault, as defined in Section 261, 266, 286, 288, 288a, 243.4 of the Penal Code, or any successor statute.

6. Committing or attempting to commit robbery or extortion.

7. Actual, attempted, or threatened injury to another person.

As students prepare to transition from high school, it is necessary for them to send their official

high school transcript to the military, selected schools, colleges, universities and/or institutions as

part of the entrance requirements. However, student transcripts are confidential and may be

released only with the consent of the student, 18 years of age or older, or their parent/guardian.

Since students and parents sometimes make multiple requests as they consider options, the

process can be delayed while parent/guardian signatures are obtained. For your convenience, the

Registrar will keep this signed authorization on file. Subsequent requests for transcripts can be

handled without the need for the school to acquire parental authorization for each request.

The official high school transcript contains the following information:

Personal Information: social security number, grade level, gender, birth date, enrollment date.

Course Information: courses completed, grades earned, cumulative GPA, date of course

completion, credits earned, notations if courses were Honors/AP/College prep.

Transcripts contain confidential information that can only be released with the permission of an

adult, 18 years of age or older. Students younger than 18 require the permission of a

parent/guardian. Signature below will be kept on file and authorizes the school to release the

student’s transcript to any schools, colleges or universities the student or parent requests.

Cal Grant OPT OUT Form

Cal Grant Awards are free money provided by the State of California to help pay for the cost of college. If you are a graduating high school senior, meet academic, financial and eligibility requirements, you may qualify to receive a Cal Grant.

To be considered for a Cal Grant award, both of the following requirements must be completed:

1) A 2016-2017 Free Application for Federal Student Aid (FAFSA) must be completed by parent/guardian with the Federal processor at www.fafsa.ed.gov by the October 1, 2017 deadline.

2) High school must submit a certified Grade Point Average (GPA) to the California Student Aid Commission by October 1, 2017.

Cal Grant GPAs will be accepted only if certified by a school electronically. The fastest and most secure way for a student to submit a Cal Grant GPA is to have Mt. Carmel High School electronically upload the GPA directly to the California Student Aid Commission’s system.

If you wish to OPT OUT from your child’s electronic submission please fill out

the following and return it to Mrs. Meyer in the Records Office (located in the

Administration Building) during Registration. If not received, your information

will be uploaded.

Parent/Guardian Permission for Off-Roll

Dear Parent/Guardian,

You and your student have requested a shortened schedule during one or more terms for the 2017-18

school year. A shortened schedule is one that has your student enrolled in fewer than five courses.

Only students who are on track to graduate may request a reduction in their school schedule.

Not having a 5th period will result in a release from school prior to the scheduled end time.

Not having a 1st period will result in a start time that is later than the scheduled start time.

Not having period 2, 3 or 4, will result in students being unassigned to a class during the school day.
