Super Bowl excitement and high fashion collide in this unique and spectacular fundraising event. Mingle with current and former NFL superstars and celebrities while enjoying a delicious brunch, fashion show and awards ceremony. Silent and live auctions will feature exclusive items such as jewelry, sports memorabilia and spa packages. A portion of the proceeds from the event will benefit local charities in Super Bowl host city.

Featuring the wives of current and former NFL players working the catwalk as they showcase the latest fashions. This is an exclusive event – limited number of tickets will be sold.

16th Annual Players’ Wives Fashion ShowEvent Information

Join current and former NFL superstars and their families at the 16th Annual Players’ Wives Fashion Show! In accordance with our mission, proceeds from the event will benefit local charities. The purpose of the event is simple:

• To create a unique football-related signature fundraising event to be held in conjunction with the Super Bowl; • To honor and pay tribute to the leadership qualities of women in Super Bowl host cities who have made remarkable strides in improving the lives of women and children; • To raise money in support of local host city charities; • To develop enduring relationships and connections between Off the Field and the Super Bowl host cities

In addition to presenting a fun and fashionable runway show, the event offers a unique opportunity to acknowledge the hard work and dedication of those who support these goals. The following awards will be presented during the event:

• The Danielle Beverly Courage Award – awarded to an individual who personifies an incredible amount of strength and determination. This award is named after Danielle Beverly, a courageous member of Off Field who battled and beat breast cancer twice. • The Off the Field Woman of Impact Award – awarded to an individual who has shown continued dedication to causes committed to community service and philanthropy.

This incomparable event offers you and your company the opportunity to be involved in hosting an exciting and unique football-related event during Super Bowl weekend. You’ll be invited to mix and mingle with celebrity guests, models and athletes while promoting your company and/or products to event attendees. Local and national media outlets have featured this high profile event. Most importantly, you will be involved in supporting and promoting the initiatives of Off the Field helping us make a positive difference in the lives of women and children. Some past charities Off the Field has supported are St. Jude Children’s Research Hospital, Peyton Manning’s Children Hospital at St. Vincent, Phoenix Children’s Hospital Center for Cancer and Blood Disorders, 34 Ways Foundation and GEMS Girls Educational & Mentoring Services.

The event features NFL wives and special guest model the latest fashions on the catwalk. Highlights of this glamorous event include a delightful brunch, an awards ceremony, and silent and live auctions featuring unique items like sports memorabilia, jewelry and spa packages. Don’t miss out on the opportunity to be involved in the glitz and glamour of the Super Bowl while promoting your company and supporting a great cause!

Off the Field was founded in February 2006. Our membership is comprised of active and retired NFL players’ wives. Off the Field strives to use our network of women to enrich the lives of those in surrounding communities where we live. We are dedicated to raising funds and creating awareness for local and national charitable organizations that strengthen the family unit. For more information, visit www.offthefieldpwa.org.

We have designed several basic sponsorship packages. If you want to purchase multiple packages, or an in-between solution, we will work with your organization to customize a package that meets your specific needs. Proceeds from sponsorships and donations are tax deductible in accordance with IRS rules and regulations.

OTF and Sponsor agree to the following terms and conditions:

Sponsor hereby agrees to participate as a sponsor at the Annual Super Bowl Awards Fashion Show (OTFEvent) pursuant to the Sponsor Kit Guidelines, which are incorporated herein by reference, and termsand conditions of this Agreement. In the event of a conflict between the Sponsor Kit Guidelines andthis Agreement, the terms of this Agreement shall take precedence.

Upon request from OTF, Sponsor shall remove any element thereof that, OTF Event’s sole discretion, isdeemed objectionable, unsafe or detracts from the Event because of noise, method of operation, orany other reason. Sponsor agrees not to display nor offer for sale at the Event any products, services,or promotional materials that (a) infringe the intellectual property or other rights of OTF and in its solediscretion. OTF reserves the right to remove any such Sponsors (or element thereof) in its sole discretion.In any case, OTF shall not be liable for any refund or for other expenses incurred by Sponsor.

Payment Terms: Upon execution of this Sponsor Agreement, Sponsor agrees to pay the Sponsorship Fee

(Cancellation) below. OTF will invoice Sponsor for the Sponsor Fee immediately upon receipt of a signed

agreement and payments will be due in full 30 days from Sponsor’s receipt of such invoice. After January 2, 2017

Sponsor’s Sponsor Fee is due within seven (7) days from Sponsor’s receipt of such invoice or within one week

before Event Date.

OTF reserves the right to cancel and/or re-assign Sponsorship without prior notice if payments are not received

when due.

Cancellation and Liquidated Damages: Sponsor may cancel this Agreement, without penalty, by providing

written notice of the cancellation to OTF, on or before January 2, 2017. After January 2, 2017 Sponsorship Fee is not

refundable for any reason and may be retained by OTF as liquidated damages for such cancellation.

In each provision in which liquidated damages are specified as remedies, the parties agree that actual damages

would be difficult to determine accurately and that such amounts are reasonable estimates of the actual

damages that would accrue to OTF. Such damages are in addition to other remedies that OTF may have at law or

in equity.

I understand and agree the Entitlements will be granted upon payment in full, and all deadlines have been noted.

IN WITNESS THEREOF, this agreement has been duly executed by both parties as of the latest date set forth.
