    Exercise, Why Do We Need It?One of the dangers of learning new information is that sometimes it requires a major shift in ones thinking. Many people resist

    this and continue to think in their old thought patterns even though in the back of their minds, they know their thinking is flawed.Over the last few months this has happened to me and Im doing my best maintain my new information rather that sliding back intothe old thought, but it has made counseling people on diet, nutrition, and exercise very difficult.

    We have recently learned that the Apo E Gene controls how the body transports fats. We also know that each of the six ApoE genotypes is influenced by different types of food and different types of exercise. This means that many chronic diseases arecaused by a mismatch between the bodys genotype preference and the actual food intake and exercise activity that a personhas been doing.

    So this means that counseling patients on life style changes can be very precise if we have the Apo E Genetic information. ButI found myself feeling handicapped when I do not have that information. So I have had to revert back to some sort of genericdirections that will have to be satisfactory until a person gets their genetic test.

    Exercise has been a topic that I have tried to ignore for most of my career. This was not based on the unimportance of thebenefits of exercise, but my own dislike for what I thought was useless expenditure of energy and a waste of time. However, withsome new understanding, I am changing my thinking.

    There are two factors. I recently attended a conference on chronic illness and chronic liver disease. Some research waspresented that showed a Frailty Score that determined survival probability. The most important feature of evaluating the FrailtyScore and survivability after an accident, acute illness, or surgery was the amount of lean muscle mass a person carried. The exactmechanism of this is not yet clear. But, this means that if we can build and maintain muscle mass we can reduce frailty and enhancehow we deal with chronic illness, acute illness, injuries and surgery.

    The second factor has to do with normal body weight. We know that resting muscle uses only stored fat as its fuel source.Working muscle uses glucose as its fuel. If we do not eat for three hours, we begin to break down muscle for energy. Some timeago, I wrote an article, It is Good to Feel Hunger You are Burning Stored Fat. It is apparent now thatthis concept is not accurate. True biological hunger tells us it is time to eat. But it is important to feed ourbody fuel, not junk. Each meal and snack must contain some fats, proteins and carbohydrates. If we remainhungry and do not eat, the body will begin to feed on itself. But it feeds on our lean muscle. The downside of this is that reducing the total muscle mass ultimately reduces the consumption of stored body fat,reducing our ability to eliminate excess fat. So, these two things go together. We need to exercise to buildmuscle and eat every three hours to maintain that new muscle. Only then can our weight normalize.

    We need both aerobic and anaerobic exercise. Aerobic exercise is hot, sweaty, fun activities that get ourheart rate up and get the blood pumping. Anaerobic exercise is stretching and strengthening. Both buildmuscle. I will explore next week what this means and how much we should do of each kind of exercise. Inthe meantime get moving. We were designed for motion and mobility, not sitting and riding.
