Cancer TransitionsCancer Transitions is a 4-week evidence based program that has been proven to help survivors feel a significant decrease in health worry, and feel a significant increase in quality of life around physical, mental and social functioning. The program is offered to anyone in our community who has recently completed active treatment for cancer and/or a caregiver!
Thursdays – June 6th, 13th, 20th, 27th5:30 pm - 8:00pm
(dinner provided starting at 5:00, programming to go from 5:30-8:00)

The focus of this program is to empower survivors and caregivers with the information they need to maximize their health and well-being after cancer. We will cover nutrition, physical activity, emotional coping and follow-up care. Expert speakers will include cancer rehabilitation exercise experts, a board certified specialist in oncology nutrition, licensed professional counselor, registered nurse and cancer survivors.

Thank You for Your SupportThank you for being a part of our community of caring, working to insure excellence each day in our mission to help cancer patients, their families and our community. In 2018, we accomplished the following:
• 4,538 people facing cancer were assistant by our agency with a variety of needs including physical and social support
• Over 38milliondollars in medication and financial assistance was provided and secured for our clients
• Provided over 714 educational sessions encouraging healthy eating and active living through multi-week programs
• 1,660 programs were utilized by survivors to enhance well-being
• For every$1 of direct financial assistance provided, $448 of in-kind financial assistance was secured.
