General InformationThe conference will be held Sept. 28 – 30, 2012, on the first floor of the Leonard and Madlyn Abramson Pediatric Research Center at The Children’s Hospital of Philadelphia. The registration fee is $435 for physicians and $275 for other healthcare professionals. The registration fee includes continental breakfast, breaks, lunch, parking (Friday, Saturday and Sunday), and syllabus material.

Registration InformationAttendees may register by mail, fax or online. Please see the registration form for the mailing address, fax number and Web address.

Pre-registration is required; the registration deadline is Friday, Sept. 14, 2012. Payment must accompany the registration form for it to be processed. Once registration has been processed, a confirmation letter will be mailed. Single-day registration is available. If you have any questions about this conference, please call 215-590-5263.

Cancellation and Refund PolicyThe Children’s Hospital of Philadelphia reserves the right to cancel or postpone any course due to any unforeseen circumstances. In the event of cancellation or postponement, The Children’s Hospital of Philadelphia will refund any registration fees, less a $25 service charge, but is not responsible for any related costs, charges or expenses to participants, including cancellation charges assessed by airlines/travel agencies. In order to process refunds for course withdrawals, cancellation must be received in writing by Friday, Sept. 14, 2012. No refunds will be issued thereafter.

The registration fee is $435 for physicians and $275 for other healthcare professionals. The registration fee includes continental breakfast, breaks, lunch, parking (Friday, Saturday and Sunday), and syllabus material.

Cancellation and Refund PolicyThe Children’s Hospital of Philadelphia reserves the right to cancel or postpone any course due to any unforeseen circumstances.
