MILLION MILES AWARD PROGRAMMidland Transport’s Million Mile Award Program will recognize P&D Owner

Operators for every 10 years of accident free service with the company. Each

recipient of the Million Mile Award will be presented with the following at the

annual Million Miles Award Banquet:

• Cash Award for each 10 years of accident free service

• Custom Leather Jacket

• Plaque/Certificate

• Pin

• Special Personalized Hat

OWNER OPERATOR/DRIVER OF THE YEAR AWARDTwo Awards are given each year: one Highway Owner Operator/Driver and one

City Owner Operator/Driver. Recipients receive a cash award and a plaque.

YEARS OF SERVICE RECOGNITION PROGRAMCertificates are awarded for 5, 10, 15 and 20 years of service. Owner

Operators with 25 years of service are invited to the annual 25 year banquet.

QUARTERLY SAFETY BONUS PROGRAMAll P&D units are entitled to receive a quarterly Safety Bonus. To receive this

bonus, the unit must comply with the following:

• Have no preventable accidents for the quarter.

• Comply with the company uniform policy for the quarter.

• Have at least 12 paid days per month to qualify for the bonus.

SAFETY PIN PROGRAMSafety pins are awarded to full-time Owner Operators for each year of

accident-free driving with Midland. These numbered pins are awarded on the

Owner Operator’s anniversary date each year.

SAFE INSPECTIONEffective September 1, 2013, each time an Owner Operator is given a clean

DOT Inspection Report, they will receive a gift card. Simply turn in the

inspection report to Driver Services and you will receive the gift card.

OWNER OPERATOR GROUP INSURANCE

Midland Transport offers Owner Operators and their drivers the option of a

Group Benefit Plan.

Group plans do not require any medical underwriting and cannot decline

coverage if coverage is applied for within 30 days of contract date. The

Owner Operator and their family get the protection they need. The cost of

the group plan is an average of 15% less than getting personal coverage with

Blue Cross.

DOWNTIME INSURANCE PROGRAMThe Downtime Insurance Program provides protection for all Midland

Transport Owner Operator units. There are no inspections or rejections.

DISCOUNTED TIRES

Midland Owner Operators are eligible for a

discount on Michelin Tires. This discount is 15-

20% less than the retail price. This is an average

savings of $150 per tire.

In order to receive this discount, a purchase

order must be obtained from Owner Operator

Relations at Midland Head Office. Tire

purchases can be deducted from the biweekly

settlement per the purchase order guidelines.

There is no interest charged on Michelin tire

purchases, and only a $10 handling charge.

DISCOUNTED PARTS AND LABOUR

Midland has a partnership with Universal Truck & Trailer. Midland Owner

Operators are eligible for a discount on both parts and labour. This discount

is an average of 10-20% off the regular retail price.

AIR MILES — Universal Truck & Trailer now offers Air Miles reward miles. Earn

1 Air Miles reward mile for every $40 purchased in parts and service and 1 Air

Miles reward mile for every $100 in new truck sales.

RENTAL EQUIPMENT

In the event that the Owner Operator’s equipment is not in service, Midland

Transport may have equipment available to rent at an all inclusive rate per

mile or per hour. Subject to availability of Midland equipment.

PURCHASE ORDER PROGRAM

Midland Transport will assist Owner Operators by providing a Purchase Order

for emergency parts or repairs to their units. Purchase orders are deducted

from the biweekly settlement over a set period of time. Handling charge and

interest are calculated and applied by Owner Operator Settlements.

RRSP’s

RRSP’s can be contributed to by deduction

from your bi-weekly O/O Settlements

payment. As this is a Group plan,

Owner Operators benefit from lower

management fees. Spousal RRSP’s and

Tax Free Savings Accounts are also

available.

Group Retirement Services will mail RRSP

tax receipts to the Owner Operator to be

used when income tax returns are filed.

An RRSP is a retirement savings plan that you establish and to which

you contribute. Deductible RRSP contributions can be used to reduce

your tax.

CANADA SAVINGS BONDS

Canada Savings Bonds are available

by deduction from Owner Operator

Settlements.

DECALS

All decals are provided for Owner Operator units up to a maximum of $500.

TOLLS

Tolls are 100% paid by Midland Transport. Owner Operator units are provided

with transponders that cover almost all toll charges. Any toll charges that

must be paid out of pocket are reimbursed by Owner Operator Settlements.

Any necessary fax charges or trailer washes are also 100% paid by Midland.

WORKER’S COMPENSATION

100 % paid by the Company for Owner Operators not required, by Workers

Compensation, to purchase and maintain his/her own WCB coverage.

For those Owner Operators required to purchase and maintain his/her own

WCB coverage, he/she must provide evidence of such coverage. Quarterly,

these Owner Operators will be reimbursed at the Midland WCB rate up to

the amount remitted to WCB by the Owner Operator.

UNIFORM ALLOWANCE

Midland Transport provides uniforms for the Owner Operators. Uniforms

consist of a variety of items to pick from. Uniform Allowance is $350.00 per

year per unit.

FUEL COSTS

Fuel purchased at authorized fuel stops designated by the

company will be charged to the Owner Operator as follows:

IRVING TISSUE PURCHASE PLAN

Once a year, Owner Operators are able to purchase four (4) cases per truck of Irving

Tissue products at a reduced cost. The purchase is deducted through Owner Operator

Settlements.

GM DISCOUNT PROGRAM

Midland Owner Operators are eligible for a GM discount on new vehicles.

FORD DISCOUNT PROGRAM

Midland Owner Operators are eligible for a discount on the purchase or

lease of a Ford, Lincoln, and Mercury vehicles in Canada.

NISSAN-INFINITI DISCOUNT PROGRAM

Midland Owner Operators are eligible for a discount on Nissan/

Infiniti vehicles in Canada.

GOOD LIFE FITNESS DISCOUNT PROGRAM

Since 1979, GoodLife Fitness has been educating, motivating, and helping

Canadians reach their fitness goals. Midland Owner Operators are eligible

for a discount of up to 15% on memberships.

DELL DISCOUNT PROGRAM

Midland Owner Operators are eligible for a discount of up to 30% on Dell

TD/THE PERSONAL INSURANCE DISCOUNT PROGRAM

Midland Owner Operators are eligible for a discount on their personal

vehicle/home insurance.

TRANSPORT TO THE FUTURE SCHOLARSHIP FUND

Any student, who will have

successfully completed a Grade

12 or equivalent and will be

commencing a selected program

no later than one day before their

25th birthday, can apply to the

scholarship fund.

Students must be a “child” of one

of the following: a current full-time

permanent employee or a Highway

Owner Operator/City Owner Operator

of Midland Transport Limited. A “Child” is defined as natural, adopted or

step-child.

Applications will be reviewed by a Scholarship Committee. Students will

be assessed based on previous academic work at high school or post-

secondary school, extra-curricular and community involvement, and on

letters of recommendations from those who know them well.

Two (2) Scholarships are awarded annually at $2000 each.

OWNER OPERATOR SAVINGS

There is the option to contribute to an Owner Operator Savings account

which is deducted from the bi-weekly settlement. This is a flat dollar amount

selected by the Owner Operator to be put into an account with Midland. The

funds in this account are available to the Owner Operator upon their request.

Owner Operator’s

ADVANCES

Advances are available upon approval to assist Owner Operators in

emergency situations. All requests for advances must be obtained through

the Owner Operator Relations Department at Head Office.

VOLUNTARY INCOME TAX REMITTANCEOwner Operators have the option of voluntarily making payments to CRA

for Income Tax. The amount of their choice is deducted from the bi-weekly

settlement and remitted to CRA on the Owner Operator’s behalf.

REFERRAL PROGRAMEffective September 1, 2013, Midland introduced an Owner Operator Referral

Program.

• Simply refer a new Owner Operator/Company driver to the Driver Services

Department who is interested in joining Midland.

• If the referred candidate is successful in getting hired, you will receive an

initial $500.00.

• If the referred candidate is still with Midland after 3 months, you will receive

an additional $500.00.

SIGN ON BONUS

Effective September 1, 2013, Midland introduced an Owner Operator Sign-On

Bonus Program to help fill difficult to fill positions in our fleet. Each Owner

Operator vacancy that is eligible for the sign on bonus will be identified as

Sign-On Bonus Eligible.

All Owner Operators successful at bringing on a Sign-On Bonus Eligible unit

with Midland will receive $2500.00 with their first settlement payment. If this

Owner Operator unit is still with Midland after 3 months, the O/O will receive

an additional $2500.00.

This includes existing Owner Operators adding an additional unit to the

Midland Fleet.

Eligibility Requirements• The program does not apply to an existing Owner Operator adding

additional units to the system.

• The referred Driver or Owner Operator must be filling an existing need in

the system.

• The candidate must identify you as the individual who referred them on

their application.

• There is no limit on how many referrals you can make. You will receive

$1000.00 for each successful referred candidate.

• Employees working in Human Resources, Driver Services, and Recruiting

are not eligible to participate.

SUBLEASE PROGRAM

Midland Transport offers a full service Sublease Program to Owner

Operators. This program offers late model Freightliner Tractors.

Here are some of the benefits of Leasing:

• Leasing is a quick and easy transaction to complete

• There’s no down payment or credit check required

• Zero start-up costs for your trucking lease

• No long-term commitment

• No loss due to normal depreciation

• Many tax advantages

The leased vehicle can be returned and the agreement terminated with just

30 days notice. There is no long term lease to sign.

Fixed Charge for Use of Truck deducted at twenty-six bi-weekly payments.

The vehicle lease payment will be deducted from the bi-weekly Owner

Operator Settlement.

There will be a mileage charge associated with the lease of the vehicle that

will be deducted from the bi-weekly Owner Operator Settlement.

Midland is responsible for repairs and maintenance:

• Provided the Owner Operator operates the unit in a careful, non abrasive

manner and not beyond its capacity, Midland will maintain and repair the

leased vehicle;

• Midland will provide oil, lubricants, tires (due to normal wear and tear),

tubes and other operating supplies necessary for the proper operation of

the vehicle.

LICENSINGAll pro-rated Licensing costs for Canada are the responsibility of the Operator.

The annual cost of this Canadian Licensing will be deducted from the Operator

over a 12 month period from the bi-weekly settlement.

INSURANCEOperator will pay a mileage premium for insurance, which will be deducted

from bi-weekly settlement on the paid miles.
