Enclosed, you will find samples of printable documents from My Dental Hub practice growth software.

1. Welcome to the Practice We have custom made a document in the Practice Portfolio, Practice Showcase section. Your practice can create an unlimited number of text with photo documents which are saved in the software. You will also find a sample Welcome to the Practice letter in the text documents section of Practice Portfolio. The text is fully customizable and can include different fonts, different colored text, etc. You can include your office logo or a photo.

2. Patient Photos This document is printed from uploaded patient images. You can choose from 6 different templates (to print from 1-‐6 photos). Using “drag and drop” technology, photos are easily placed into the template. Text can also be added for each photo. The forms are customized with the practice and patient information. This is all done in your Patient Management section.

3. Graphic Forms These are the printable version of the Image Documents and Slides. They explain dental procedures in simple terms to patients. These forms are printed with the practice information and can be personalized to include the patient’s name.

4. Text Forms These are found in Practice Portfolio. The text forms contain more detailed explanations about various procedures. Existing forms may be edited to suit your office. The practice can also create an unlimited number of their own text documents, which are saved for future use.

5. Consent Forms Also found in the Practice Portfolio text documents section are office policy forms, consent forms, and post op instructions which are all fully customizable.

My Dental Hub recommends that practices create similar booklets for their patients, which are specific to the treatment recommendations. Fees for services can also be included within the booklet, providing the patient with a comprehensive book, containing all the information they need to make an informed decision, thereby increasing case acceptance.

Treatment Plan Information

As a result of our clinical and radiographic examination, along with a full mouth analysis, the following enclosedtreatment options are recommended to correct the problems that currently exist with your teeth and oral tissues. All reasonable options, along with their advantages and disadvantages will be discussed with you. It is yourdecision as to which treatment option you wish to pursue. In addition to the recommended treatment options,there is an estimate of the fees involved. If a treatment option is not chosen in its entirety, the fees may bereassessed. The estimated fees are valid for 120 days from the date of the estimate and any treatment whichprogresses after 120 days may be subject to increased fees.

The cost of treatment is only an estimate due to other variables involved. If your treatment proceeds without anyalteration, the final cost will be close to the estimated cost. However, it is important to keep in mind that duringthe course of treatment, additional unforeseen treatment may be requiredto properly treat your dental condition.In this event, the cost of the additional treatment will be added to the overall cost of your treatment. We willinform you of any of these circumstances should they arrive.

If the additional treatment involves the placement of small restorations, the additional cost will be moderate. Insome circumstances however, the cost of additional treatment may be substantial if the treatment planundergoes significant change. These changes will only occur with your consent. Fes for laboratory costs areseparate from your treatment and have been estimated for you. This fee includes the cost of fabricating yourprosthesis including metals, plastics, teeth, laboratory labor and materials in general. The fee cannot bespecifically established until the work is complete, but an estimate is included for you.

If you have dental insurance, you will be supplied with a dental treatment estimate (predetermination) form at thetime of your consultation. You can submit this predetermination form to your insurance carrier to receive aconfirmation of your dental coverage for the proposed treatment. At the completion of treatment, you will besupplied with an insurance form to claim your dental reimbursement from your insurance company. We do notdeal directly with your insurance company. Fees that are related to your insurance company have not beenincluded in our estimate of treatment fees. Should your insurance company require further reports or duplicateradiographs (x-rays), additional costs may be incurred. In this event, an insurance form will be issued to you tosubmit to your insurance company.

The practice of Dentistry is not an exact science and therefore we cannot ethically guarantee results. It isimportant for you to understand that treatment may or may not be successful and that with dental treatment,there is a risk of possible complications. We are unable to predict whether a specific treatment plan on anindividual will be successful or whether complications will arise. Due to the wide variety of use and abuse ofdental restorations by patients, we are unable to determine how long a restoration will survive in an individual andshould the restoration or prosthesis required repair or replacement, there will be a fee charged.

Implant Procedure - Diagram

Implants are used to replace the roots ofmissing teeth. They are a fixed solution to asingle missing tooth or an entire jaw ofmissing teeth. After an implant is placed intothe jaw bone, it is allowed to heal forseveral months to become integrated withinthe bone. After healing, the implant is readyto restore. Impressions of the mouth aremade, and models are made upon whichthe restorations are fabricated. A post isattached to the implant using a screw. Thediagram shows how the implant, post andscrew attach together.

Once the post has been attached to theimplant and secured into place, the implantsupported restoration can be completed. Acrown is made to attach to the post. Thecrown is made using metal and/or porcelainand is made to closely replicate the shapeand color of the natural tooth. Once thecrown has been adjusted, it is attached tothe post using a screw, or it can becemented onto the post. The diagramshows how the crown fits over the post.Implant therapy can take a few months andseveral appointments to complete, however,the final restoration can be almost likehaving your own tooth back!

Single Esthetic Implant

This young individual was in an accidentwhich resulted in the loss of his front tooth.A single tooth implant is the best choice toreplace this missing tooth. A bridge is notthe best choice. The teeth on either side ofthe space do not have any fillings in them.Shaving these teeth down for the placementof a bridge would permanently damagethem, and these teeth may require othertreatment.

This x-ray shows the implant positioned inthe jaw bone. A post will be attached to theimplant, upon which the final crown will beplaced to replace the missing tooth.

The final crown (cap) has been fabricatedand cemented onto the post which isattached to the implant. A small bondedrestoration was also placed on the otherfront tooth to provide a balanced estheticresult. This implant supported crownprovides a beautiful esthetic replacementfor the missing tooth.

Effects of Loss of Teeth - Female Side View

When we maintain our teeth, we alsomaintain the integrity of our facial structure.This middle aged lady has all her teeth andwe can see how the muscles in her face areproperly supported and her facial height(distance from chin to nose) appears normal.

When the teeth are lost, we canimmediately see the effects on the facialstructures. The muscles in the face are notsupported and as such, they sag and droop,creating deep facial lines. The jaws movecloser together, causing the chin toprotrude, since there are no teeth present tomaintain facial height. All these facialchanges speed up the aging process,making the individual appear much older.Although this lady is the same age in bothpictures, she appears much older with theloss of her teeth, due to the changes in herfacial structure.

Benefits of Implant Treatment

Tooth loss takes a terrible toll in the overall health and well-being of people. A missing front tooth draws instantfocus to the mouth due to the unaesthetic smile. Multiple missing teeth create a situation where the other teethwear prematurely and become crooked. When all of the teeth are lost most people have trouble wearing theirdentures as they hurt, move while eating, slip during speech. All of these problems only get worse with agedepriving many people of a good quality of life in their senior years.

Single Tooth Loss

When a single tooth needs replacing an implant is often the treatment of choice. The implant, which is an artificialtooth root, is placed in the bone and a crown is place on top of the implant. This restoration often looks andfunctions just like a real tooth. The implant restoration stops the teeth around it from drifting and becomingcrooked and it also helps the bone and the gum from collapsing as it stimulates the tissue around it in a similarfashion to a natural tooth.

Multiple Tooth Loss

Implants can serve as artificial tooth roots in any situation where enough bone is present for their placement. Oneimplant can be placed per missing tooth and individual crowns attached. In other situations implants can beplaced for some of the missing teeth where the roots used to be and bridgework can then be placed on theimplants. The great benefit of this type of treatment is that the artificial teeth often look like, feel like and functionlike natural teeth. Any implants placed provide the added benefit of resisting bone and gum loss which isimportant in resisting aging of the mouth.

Total Tooth Loss

The loss of all teeth can be a tragedy. Severe bone loss over time, as a result of tooth loss makes a person lookolder than they are due to wrinkling around the mouth. Dentures become painful to wear and many people giveup trying to eat with them. They prefer to remove their dentures at meals. As a result they become houseboundand cannot experience the social pleasure of dinning in restaurants or eating with friends. Dental implants canhelp almost all patients who have lost all of their teeth. It is better if the patient seeks treatment before most of thebone is lost, however, with modern techniques almost all patients can be helped. If bone is plentiful, implants canbe placed where the roots of the teeth used to be and crown and bridge style teeth can be made.These look like, feel like and function like natural teeth. When there is considerable bone loss, multiple implantscan be placed to support artificial teeth that resemble dentures. The difference between denture like teethsupported by implants and dentures is that they function better and move less than dentures. Eating becomes apleasure again and the teeth stay in place providing comfort, function and increased confidence. Life is wonderfulagain.

Treatment Consent Form

What you are being asked to sign is a confirmation that we have discussed the nature and the purpose of dentaltreatment, the known risks associated with dental treatment, and the feasible treatment alternatives, and that youhave been given an opportunity to ask questions and all your questions have been answered in a satisfactorymanner to your understanding. Please read this form carefully before signing it and ask about anything that youdo not understand. My signature on the bottom of this form certifies that:

1. I have been informed and understand that the practice of dentistry is not an exact science; no guarantees orassurance as to the outcome of prosthetic treatment or surgery can be made due to the uniqueness of everyindividual clinical situation. In most instances, the outcome of treatment is most satisfactory.

2. I understand that unforeseen conditions or circumstances may arise during the course of treatment and thatadditional treatment not specified in my treatment plan may be necessary. I will be advised of any additionaltreatment and estimated costs should the need arise.

3. I understand that the estimate given to me is for normal and usual treatment. I understand that if my treatmentrequires extra time, additional procedures or additional laboratory work, there will be additional fees related to theadditional time and treatment. Normal and usual treatment consists of 1 or 2 try ins of the restoration and up to 5post operative insertion adjustments.

4. I understand that Dr. Ken Hebel has carefully examined my mouth. Alternatives to the chosen treatment havebeen explained. I have been informed and I understand the purpose and the nature of the dental procedure. Iunderstand the procedures that are necessary to accomplish completion of the dental treatment and fabricationof the prostheses.

5. I have been informed of the possible risks and complications involved with surgery, drugs and anesthesia thatinclude but are not limited to the following: pain, swelling, infection, discoloration, inflammation of a vein, injury toteeth present, bone fractures, sinus penetration, delayed healing and allergic reactions to drugs or medicationsprescribed. Numbness of the lip, tongue, chin, cheek or teeth may also occur, for which the exact duration maynot be determinable and may be irreversible.

6. I have been informed of the possible risks and complications involved with dental treatment that include butare not limited to: root canal therapy, fracture of teeth or roots, fracture of porcelain or acrylic, loss ofcementation, decay around restorations and possible loss of teeth. I understand that these complications maynecessitate further treatment.

7. I understand that if nothing is done, any of the following could occur: loss of teeth, loss of bone, gum tissueinflammation, infection, decay, sensitivity, looseness of teeth followed by the need for extraction, fracture of teethand/or roots, difficulties in chewing and/or speech. Also possible are temporomandibular joint (TMJ) problems,headaches, referred pains to the back of the neck and facial muscles, and tired muscles when chewing.

8. Dr. Ken Hebel has explained that there is no method to accurately predict the outcome of dental treatment dueto large variations in teeth, gums, bone, chewing forces, and oral hygiene. It has been explained to me that insome instances dental treatment may not be successful.

9. I agree to follow the home care instructions provided to me. I agree to report to Dr. Ken Hebel for regularexaminations as indicated and I understand that this office will monitor my progress unless I have been advisedto return to my general dentist for dental care.

10. To my knowledge, I have given an accurate report of my physical and mental health history. I have alsoreported any prior allergic or unusual reactions to drugs, food, insect bites, anesthetics, pollens, dust, any bloodor body diseases, gum or skin reactions, abnormal bleeding or any other conditions related to my health.

11. I consent to photography, study models and radiographs of the procedure to be performed for use in teachingdentistry and other graphic purposes.

12. I understand that with any dental treatment, my teeth, gums or bone can be damaged by bacteria and I mustdo my utmost to remove the bacterial plaque off all the surfaces of all my teeth and/or implants every day. If I donot clean my teeth and/or implants properly, I may get decay and/or gum disease and my treatment may fail. Ihave been fully informed of the nature of dental treatment along with possible risks and complications and herebyconsent to treatment.
