Patient-Centered Primary Care Homes are health centers that have been recognized for their commitment to quality and coordinated care. At its heart, this model of care fosters strong relationships with patients and their families. Health centers improve care by catching problems earlier, focusing on prevention, wellness and management of chronic conditions
CCO
A Coordinated care organization, or CCO, is a network of all types of health care providers who have agreed to work together in their local communities to serve people who receive health care coverage under the Oregon Health Plan (Medicaid).
