What you need to do
•Identify the two factors which are of greatest concern- where your actual score is a long way from the potential score. These are the areas that will require work.•Identify the two factors which are the most positive – where your actual score is nearest to the potential score. These are the areas you should celebrate, maintain and learn from.

Initial thinking
•Work through the Model for Improvement, Aim, Change and Measure. •Predict what you think is right for each of these. •Use Capture as a baseline…..what is the experience now•What are the additional data that you can also use for example; number of complaints, number of falls, waiting time, hospital readmissions?

Questions? Questions?
You have a maximum of 39 days
Between today and the next call based on the following:I have been generous and allowed for 2 weeks holidayBut I have calculated on a five day working week so if your input is less than five days a week you will have less days. For example if you work on this around 2 days a week you have around 16 days, if you work on this 1 day a week you have around 8 days.
