Phase :

1.First : Exudative phase

2.Second : Fibroexudative phase

3.Third : Organized phase

1.Exudative phase

Pathophysiology : ดดซม pleural fluid กลบนอยลง แต inflammation กระตนใหมการสราง effusion มากขน ท าใหมการสะสม fluid ใน pleural

space.

Finding : Sp gr. ต า ม wc นอย LDH ต า Sugar และ pH ใกลเคยงใน serum.

gram stain และ Culture negative

2.Fibropurulent phase

Pathophysiology : ม bacteria เขามา colonized ท าใหม wc response และม fibrin deposit.หากเปนมากขนกจะเรมม loculation

Finding : Sp gr. สงขน wc สง LDH สง sugar และ pH ต าลงเมอเทยบใน serum

Bacteria gram stain หรอ culture หากม infection รนแรงมากกจะพบวา positive ได

3.Organized phase

Pathophysiology : fibroblast เขามารวมปฎกรยามากขนท าใหเกดความแขงแรงของ loculation หากควบคมการกระจายโรคไดโดยตวเอง fluid กอาจจะกลายเปน

-Sterile fluid จะมการ organized กลาย เปน fibrothorax ในระยะตอมา แตหาก

-Infected fluid จะมwc สะสมจนพบเปน empyema thoracis ในทสด

Post-operation 21 %

Post- trauma 6 %

Esophageal perforation 5 %

Spontaneous pneumothorax 2 %

Post thoracocenthesis 2 %

Bacteriologic study

Aerobic organism>anaerobic organism

Gram Positive >Gram negative 2 เทา

Aerobic bacteria มกพบวาเปนการตดเชอเดยวๆแต Anaerobic bacterior มกพบวาเปนการตดเชอทพบรวมกน

หลายๆชนด

Staph aueus 36 %

Strep pneumoniae 35 %

E. Coli 30 %

P. Seudomonas 25 %

Klebiella 21 %

H. Influenza 12 %

Bacteroides 20 %

Peptostreptococcus 20 %

Fusobacterium 14 %

Classification of Parapneumonic

pleural effusion according to line of Therapy

เพอชวยในการล าดบขนตอนการรกษาไปตามสงทตรวจพบ

Class I : nonsignificant pleural effusion

พบม nonloculated pleural effusion เมอ CXR lateral decubitus ใหขางทม fluid ลง พบวา fluid สงไมเกน 1 cm. จาก chest wall

RX:-ให antibiotic และ ตดตาม CXR เปนระยะ -ไมควรเจาะ Thoracenthesis หรอใส ICD

-Thoracocenthesis กรณท 1. มไขแตไมสามารถหา source of infection ทอนได หรอ 2. มปรมาณ effusion มากขนจากการตดตาม CXR

กรณ nonloculated pleural effusionทมากกวา 1 cm.จาก Chest wall

ขนตอนขนกบปรมาณ effusion และ อาการ

หากมปรมาณมากจน trachea shift หรอ มอาการ dypnea ควรใส ICD เพอลดอาการและสงตรวจ fluid

หากมปรมาณไมมากพอ และไมมอาการ dyspnea ควรท า Thoracocenthesis เพอด fluid profile และ หา organism.

Fluid ตรวจหา gramstain, C/S, pH, wc, sugar, อาจสง LDH, Protein

Class II :Typical parapneumonic pleural effusion

CXR ทา lateral decubitus แลวพบวาม effusion สงกวา Chest wall เกน 1 cm.(ยงไม loculated) fluid finding พบวา pH> 7.2 , Sugar> mg./dL.

LDH< 3*upper normal limit,

gram. Stain , C/Sไมพบเชอ

Rx: ให Antibiotic และตดตามไขและ CXR.เปนระยะ

Class III : Borderline complicated pleural effusion

หาก fluid finding

ม 7.0 <pH< 7.2

LDH>3*upper limit of serum level

gram stain หรอ culture พบเชอ

Rx : ให antibiotic และ - ตดตาม CXR และ serial Thoracocenthesis หรอ - ใส ICD เลยในกรณทม immunocompromised กไดแ

Class IV : simple complicated pleural effusion

Fluid profile: pH<7.0 sugar <40 mg./dL. Gram stain หรอCulture positive for

bacterial infection

Rx. : ให antibiotic และใส ICD ตดตาม CXR หาก lung ไม full expansion กแนะน าผาตด

Class V : Complex complicated pleural effusion

กรณทเปน multiloculation แต effusionยงไมเปน pus

Rx.: ให Antibiotic และแนะน าผาตด

Class VI : Simple empyema

กรณทเจาะหรอใส ICD แลว content เปน pus แตยงไม loculated

RX: ให antibiotic และตดตาม CXR หาก lung expansion ไมด คอยพจารณาผาตด

Class VII : Complex empyema

พบเมอเจาะแลวพบfinding เปน pus และมการ loculated

Rx : ให antiotic รวมกบการผาตด
