IDENTIFICA TION
This crash was reported at 1:01 a.m. on Friday, February 12, 2010. The collision
site was located on Lake Worth Road at the intersection of 120thAvenue South. The site
was located in the incorporated city limits of Wellington, Palm Beach County, Florida.
This crash involved a collision between two motor vehicles. One person was fatally
injured and one person received minor injuries. The driver, of one of the vehicles, left the
scene of the collision.
WEATHER
At the time of the collision the following weather conditions existed:
Temperature: 52 degrees.
Dew point: 40 degrees.
Winds: 5 miles per hour from the North West.
Barometer: 29.92 millibars.
Visibility: 10 miles.
Relative humidity: 63 percent.
The National Weather Service, located at the Palm Beach International Airport
recorded these conditions. These conditions may have varied from those found at the

collision, as the recording site was approximately 8.6 miles northeast of the scene.
HIGHWAY
Lake Worth Road, at the intersection of 120thAvenue South, is a three lane divided
roadway east of the intersection and a two lane undivided roadway west of the intersection.
The roadway runs in the east and west directions. Westbound Lake Worth Road consists
of one through lane, one designated left turn lane, and one designated right turn lane.
White painted lines prior to the intersection separate the one westbound through lane from
the turn lanes. The turn lanes are marked with white painted arrows. The left turn lane for
westbound traffic on Lake Worth Road allows traffic to turn left and travel south on 120th
South or to make a U-Turn to travel eastbound on Lake Worth Road. The right
turn lane for westbound traffic on Lake Worth Road allows traffic to turn right to travel north
on 120thAvenue South. A raised concrete median divides the east and westbound lanes
of traffic east of the intersection. The limits of the outside travel portion of the lanes, or fog
lines, consist of single white lines and concrete curbs. The roadway is relatively straight
and level with minimal crowning to facilitate normal water run off and it was free of defects.
It is composed of an asphalt material.
Eastbound Lake Worth Road consists of one through lane and one designated left
turn lane west of the intersection and two through lanes east of the intersection. White
painted lines, prior to the intersection, separate the one eastbound through lane from the

turn lane. The turn lane is marked with white painted arrows. The left turn lane for
eastbound traffic on Lake Worth Road allows traffic to turn left and travel north on 120th
Avenue South or to make a U-Turn to travel westbound on Lake Worth Road. A median,
painted with yellow lines, divides the east and westbound lanes of traffic west of the
intersection. The limits of the outside travel portion of the lanes, or fog lines, consist of
single white lines and concrete curbs. The roadway is relatively straight and level with
minimal crowning to facilitate normal water run off and it was free of defects. It is
composed of an asphalt material.
120thAvenue South, at the intersection of Lake Worth Road, is a two lane undivided
roadway north of the intersection. 120thAvenue South consists of one through lane for both
southbound and northbound traffic north of the intersection. Dual yellow lines divide the
north afid southbound lanes of traffic north of the intersection. The limits of the outside
travel portion of the lanes, or fog lines, consist of single white l ines. The roadway is
relatively straight and level with min imal crowning to facilitate normal water run off and it
was free of defects. It is composed of an asphalt material.
120thAvenue South, south of the intersection, is an unmarked dirt roadway wide
enough for two lane traffic. The roadway is relatively straight, level and free of defects.
At the area of the collision, there were no visual obstructions. This was a nighttime
collision and the weather conditions were clear and cold. There was no precipitation at the
time of the coll ision.
The intersection is controlled by stop signs located on the south and north sides of

the intersection. The stop sign for southbound traffic on 120thAvenue South, north of Lake
Worth Road, is located on the northwest corner of the intersection and was erected on
11 /16 /09 , by the Village of Wellington. The sign is 6' 10.5" from the ground at the bottom
of the sign, 4' 9" from the west edge of the roadway and 36' north of the intersection. The
stop sign for northbound 120thAvenue South is located on the southeast corner of the
intersection.
There was one street light located at the northwest corner of the intersection of Lake
Worth Road and 120thAvenue South. The area was also lit by ambient lighting, from a
nearby school and residences. In general, the area was dark.
Westbound Lake Worth Road, in the area of the collision, is governed by a posted
45 miles per hour speed limit sign one mile east of the intersection. The sign was erected
on 09/15/06, by the Village of Wellington. The sign is 8' 6" from the ground at the bottom
of the sign and 9' from the north edge of the roadway.
Southbound 120thAvenue South, in the area of the collision, is governed by a
posted 35 miles per hour speed limit sign .9 miles north of the intersection. The sign was
erected on 8 /24105 , by the Village of Wellington. The sign is 7' 6" from the ground at the
bottom of the sign and 9' 9" from the west edge of the roadway.
VEHICLE V-1
V-1 was a black, 2007 Bentley GTe convertible, vehicle identification number
bearing Texas tag 380WBJ. V-1 is registered to John Goodman

Dr. Michael Bell, M.D., of the Palm Beach County Medical Examiner's Office,
determined the cause of death to be: Drowning.
BACKGROUND
On Friday, February 12, 2010,0-2 was traveling from Orlando where he lived and went to
college on his way to his mother's house in Wellington. 0-2 was westbound on Lake
Worth Road through the intersection of 120thAvenue South where the collision occurred.
John Goodman (0-1) invoked his right to counsel and did not provide me with a
statement. However, background information on 0-1 was established through witness
statements and physical evidence gathered during the investigation. According to witness
testimony John Goodman had attended a fund raising benefit at the White Horse Tavern,
3401 Equestrian Club Drive, Wellington, FL 33414, from around 7:00 p.m. until 10:00 p.m.
Goodman drank alcoholic beverages and ate dinner during the fund raiser. Goodman
then left the fundraiser and drove to the Player's Club, 13410 Southshore Boulevard,
Wellington, Florida 33414, where he continued to drink alcoholic beverages. Goodman left
the Player's Club alone. He drove his black Bentley GTC south on Southshore Boulevard
and turned left onto Pierson Road to travel eastbound. Goodman then drove to 120th

Avenue South where he turned right to travel southbound. Goodman then traveled
southbound on 120thAvenue South approaching the intersection of Lake Worth Road.
Goodman entered the intersection, failing to stop for the posted stop sign when the
collision occurred. Goodman walked away from the scene of the collision and did not call
911 until 1:55 a.m., 54 minutes after the collision was reported.
CRASH ANAL YS/S
The dynamics of this crash involve V-1 traveling southbound on 120thAvenue South
approaching the intersection of Lake Worth Road. V-2 was traveling westbound on Lake
Worth Road approaching the intersection of 120th Avenue South. As V-2 entered the
intersection, V-1 entered the intersection and did not stop for a marked stop sigh. This
caused a violent collision with the front ofV-1 impacting into the passenger side ofV-2. As
the collision ensued V-2 began to slide sideways in a southwest direction and V-1
continued southbound rotating in a clockwise rotation. V-1 then pushed V-2 sideways in a
southwest direction through the intersection and off the roadway south of Lake Worth
Road. V-1 and V-2 entered the dirt shoulder south of Lake Worth Road and both vehicles
began to furrow into the dirt. V-1 and V-2 continued in a southwest direction toward the
canal south of Lake Worth Road. V-1 came to rest facing southwest north of the canal. V-
2 continued southwest sideways and rolled down the bank of the canal and into the water.
V-2 came to rest submerged in the water upside-down.

INVESTIGA T ION
Sheriffs Office Vehicle Homicide Investigations Sergeant John Churchill notified me
of this collision at 3:10 a.m. on 02 / 12110 . At 3:18 a.m., I arrived on the scene; Palm Beach
County Sheriffs Office Deputies had previously secured the scene by blocking eastbound
and westbound traffic on Lake Worth Road and north and southbound traffic on 120th
Avenue South. The traffic crash investigation (reference companion case number 10-
037895) was assigned to Investigator Robert Stephan # 7240 and Sergeant John Churchill
# 3848 assigned the Vehicle Homicide Investigation to me.
arrival on scene, I met with DUI Enforcement Deputy Kenneth Noel # 7660
who arrived on scene at 2:38 a.m. Noel told me 0-1 had left the scene of the collision and
called 911 later stating he was driving the Bentley and had pulled in front of someone. D-1
was later transported to Wellington Regional Medical Center. Noel also told me 0-1 was
located at an address south of the collision scene on 120thAvenue South. Noel then told
me there was a witness at the address on 120thAvenue South, who spoke to 0-1. (Refer
to Noel's supplemental report). I then spoke to Deputy Mitch Rieger # 2820, the first
responding Deputy, who arrived on scene at 1:12 a.m. Rieger stated he responded to the
scene of the collision and saw a damaged black Bentley on the side of the road and a
vehicle upside-down in the canal. Reiger stated the witness, Nicole Oroco, had already left

the scene prior to his arrival. He said he called for a tow truck (at 1:38 a.m.) to recover the
vehicle because Fire Rescue Personnel told him the vehicle in the canal was not occupied.
Rieger said the driver of the Bentley was not on sce.newhen he arrived. He then told me
the driver of the Bentley called dispatch (at 1:55 a.m.) and gave his location south of the
collision scene. Rieger said Deputy Safford (# 8885) responded to pick up the driver
identified as John Goodman. He said Safford brought Goodman back to the scene and
Palm Beach County Fire Rescue Personnel placed him in the back of a rescue rig. Rieger
said he spoke to Goodman and smelled a strong odor of an alcoholic beverage coming
from his person and the back of the rescue rig. He said Goodman told him he stopped at
the stop sign and then hit something, but did not know what it was. Rieger said Sister's
Towing responded and removed the vehicle from the canal and found the driver wa s s till
belted into the driver seat and appeared to be deceased. Rieger said he then called for the
Vehicle Homicide Unit. (For further information refer to Deputy Rieger's supplemental
report.)
On 02 /12 /10 at 3:25 a.m., I conducted a walk through of the scene and observed a
Black Bentley GTe convertible south of Lake Worth Road facing southwest. The Bentley
had extensive front end damage and the driver side steering wheel and knee bolster
airbags were deployed. I also saw a tan Hyundai Sonata four-door connected to a large
Sister's Towing tow truck. The Hyundai had extensive passenger side damage, water plant
life inside the vehicle and the vehicle appeared to have been in the water. 0-2 was also
still belted in the driver seat. I also identified gouge marks and tire marks in the roadway
and in the dirt leading to the final rest of V-1 and the canal. I then saw a set of boot prints

leading away from V-1 and traveling southbound on 120thAvenue South. The boot prints
lead to the address of 4193 120thAvenue South where 0-1 was found, approximately 1038
feet from the collision scene. The prints were the shape of cowboy boots with a square
toe.
On 02/12 /10 at 3:33 a.m., I departed the collision scene and responded to
Wellington Regional Medical Center to conduct an impairment assessment on 0-1. Upon
my arrival, 0-1 was in radiology having X-ray's taken of his injuries. I made contact with 0-
1 and saw he was wearing a pair of square toe cowboy boots and the boots had sand on
them. The sand matched the sand from the roadway the boot prints were found on. I saw
his eyes were bloodshot and glassy. He also was slow with his movements and slurred his
speech while he was speaking. There was a strong odor of an unknown alcoholic
beverage corning from his breath as he spoke. 0-1 was on a hospital bed and I made no
observations of his balance or ability to walk. I then asked 0-1 if he would give me a
statement about what happened in the collision and he stated he wanted to talk to his
lawyer. I was advised 0-1 's lawyer was in the lobby of the emergency room. I made
contact with his attorney, Wade Byrd, and told him 0-1 wanted him present while I spoke to
him. I then walked Byrd back to the room where 0-1 was located. Byrd briefly spoke with
0-1 and he advised me 0-1 would not give me a statement. I then asked Byrd if 0-1 would
allow me to obtain a blood sample from him consensually. Byrd stated he was not sure 0-
1 wanted to give a sample. At that time I advised 0-1 I had probable cause to conduct a
nurse Cecilia Betts, a nurse in the emergency room of Wellington Regional Hospital, to

forced blood draw from him and he did not have the right to refuse. I instructed registered

conduct a blood draw from 0-1. I supplied her with a sealed unexpired blood kit and
observed her draw blood from 0-1 's left arm using a butterfly needle. The butterfly needle
was supplied by the hospital and Betts told me she was more comfortable utilizing the
butterfly needle. The first blood draw was conducted at 3:59 a.m. and the second was
conducted at 4:00 a.m. Betts turned over the blood vials to me and I inverted the tubes
several times. I sealed the vials and labeled them with the case number and 0-1 's
information before placing them into the box supplied in the kit. The kit was sealed with
evidence tape and Betts signed a certificate of blood withdrawal form.
After obtaining consent from 0-1, I then photographed 0-1 and his injuries.
On 02/12/10 at 3:40 a.m., Investigator Robert Stephan arrived on scene. Stephan
and OUI Enforcement Oeputy Noel located on scene evidence and roadway markings.
Investigator Stephan took photographs of the scene, victim and vehicles involved. (Referto
Investigator Stephan's supplemental report).
On 02/12/10 at 4:16 a.m., I reviewed a copy of the 911 call recording from the
Palm Beach County Sheriffs Office Oispatch Records. 0-1 called 911at 1:55 a.m., 54
minutes after the collision was reported. 0-1stated "I just had a wreck. And, urn, I, uh,
urn, walked over and my phone was dead and someone -- where am 1 7 Lake Worth
and 120th. And, uh, um, what direction is, uh, 120thAvenue?" 0-1 then stated "And,
urn, I just actually pulled out in front of someone." He also gave the location of where
he was. (For further information refer to the transcribed dispatch recording.)
On 02/12/10 at 4:22 a.m., I responded back to the scene of the collision and met
with Investigator Robert Stephan. Investigator Stephan told me he was documenting the

boot prints and photographing them. He advised me that the family of 0-2 was at the
scene and wanted to talk to me. I made contact with the Medical Examiner Investigator
Lidia Littlefield on scene and she told me they positively identified 0-2 by his Florida Driver
License. I then made contact with 0-2's father William Wilson and made next-of-kin
notification. I turned the blood kit over to Deputy Noel and he transported the kit to the
Palm Beach County Sheriffs Office Crime Lab, where he placed it into the crime lab
refrigerator.
On 02/12/10 at 5:00 a.m., I met with Lisa Pembleton, an independent witness, at
4193 120thAvenue South, Wellington, Florida 33414. I told Pembleton I was investigating
the collision and wanted to talk with her about it. Pembleton agreed to provide me with a
recorded swom statement. The following is a brief synopsis: Pembleton told me she was
awakened in her trailer by a knock on the door. She said a man walked into her trailer and
asked to use the phone. The man told her he was involved in an accident and needed to
call someone. She said she gave him her cellular phone and he called a lady in Georgia.
Pembleton said the male then asked her if he looked impaired and if he should call 911.
She told him to call 911 and he did. She said the male stayed on the phone with 911 until
the police arrived and took him. (For further information refer to the transcribed statement.)
Pembelton agreed to let me look at her cellular phone and record the calls made by
0-1. I saw that 0-1 called at 1:52 a.m., 911 at 1:55 and 1:56, and the
number lied incoming at 2:10 a.m. There was also a message left on the
voice mail. The message was a Hispanic male that said, "This is Carlos give me a call
back."

On 02/12/10 at 7:45 a.m., I took aerial photographs of the area of the collision
scene.
On 02/12/10 at 8:15 a.m., Investigator Robert Stephan, Karly Gallart # 7347 and I
completed a forensic mapping of the scene utilizing the Sokkia Mapping Station. The
measurements of this scaled diagram were utilized to illustrate the dynamics of this
collision and the documentation of the roadway.
On 02/16 /10 at 1:00 p.m., I responded to the Palm Beach County Courthouse and
met with Judge Frank Castor to sign a search warrant for oral mouth swabs from 0-1 for
DNA testing and the cowboy boots 0-1 was wearing the night of the collision. Judge
Castor reviewed the search warrant and search warrant application and agreed to sign the
search warrant. I then contacted Mark Shapiro, 0-1's attorney, and advised him] had a
search warrant for the items. His attorney stated a search warrant was not needed and 0-
1 would consent to provide me with anything that was necessary. I then made an
appointment to meet with 0-1 and his attorney to retrieve the requested items.
On 02/16 /10 at 5:35 p.m., I met with Facundo Paredes, the valet at the Players
Club, at 13410 Southshore Boulevard, Wellington, Florida 33414. I told Paredes I was
investigating the collision that occurred at the intersection of Lake Worth Road and 120th
Avenue South and wanted to talk with him about it. Paredes agreed to speak with me and
the following is a brief synopsis: Paredes stated he was working the night before the
collision and was parking cars at the Players Club. He said he saw Goodman arrive at the
club and park his own car in front. He advised Goodman left the club alone at around one
hour before the club closed. He said Goodman was only at the club for one to two hours

and the club closes at 2:00 a.m.
On 02/16/10 at 6:15 p.rn., while at The Players Club, I met with Manger Paul Griffo
and asked him which employees were working on 02/11/10 through 02/12/10. Griffo told
me he had just notified their attorney, Craig Galle, upon my arrival and Galle would be
coming to the club. Galle responded to the Players Club and told me he would set up an
appointment for interviewing employees on 02/18/10 at 4:00 p.m. I also told Galle I was
requesting the bar receipts for 0-1 and the people drinking with him at the bar the night
before the collision. I told Galle I would provide him with a subpoena. Galle then told me
he also represents the White Horse Tavern and I could contact him with any questions I
had for that business. I told Galle I would like the bar receipts for 0-1 the day before the
collision. I told him I would also provide him with a subpoena for the receipts.
On 02/16/10 at 8:00 p.m., I drove from the Players Club to the intersection of 120th
Avenue South and Lake Worth Road utilizing the route V-1 took leading up to the collision.
The distance recorded was three miles and took 4 minutes and 35 seconds driving at the
speed limits of the roadways traveled.
On 02/16/10 at 8:15 p.m., I walked the route 0-1 took after leaving the scene of the
collision. This was in an attempt to account for the 54 minutes between the reporting of the
collision and 0-1' s call to 911. I walked at a steady pace from the final rest of V-1 to the
fifth wheel trailer located on the property at 4193 120thAvenue South. The route took
seven minutes to walk.
On 02/17/10 at 10:00 a.m., I responded to the Palm Beach County State Attorney's
Office and obtained subpoenas for the telephone records of John Goodman's cellular

phone.
On 02/17/10 at 2:40 p.m., I received the telephone records for John Goodman from
AT&T wireless.
On 2/18/10 at 12:59 a.m., I conducted roadway friction tests utilizing a Vericom
VC4000 accelerometer. The test was conducted using a 2001 Chevrolet Impala, four door,
police vehicle, vehicle identification number 2G1WF55K619335943, bearing Florida tag
33948. The test revealed the roadway friction in the area of the collision to be a .74 drag
factor.
At the time of the testing the following weather conditions existed:
Temperature: 48 degrees.
Dew point: 38 degrees.
Winds: 12 miles per hour from the North West.
Barometer: 30.04 millibars.
Visibility: 10 miles.
Relative humidity: 68 percent.
The National Weather Service, located at the Palm Beach International Airport
recorded these conditions. These conditions may have varied from those found at the
collision, as the recording site was approximately 8.6 miles northeast of the scene.
On 02/18/10 at 4:00 p.m., I met with Galle at the Players Club. I gave the subpoena
to Galle and he provided me with the bar receipts for 0-1, Kris Kampsen and a male only
identified as Stephan. He also told me there were no receipts from the White Horse

Grey Goose, and five Patron Silvers. 0-1 's receipt was opened on 2/11/10 at 11:23 p.m.
and closed out on 2/12/10 at 12:52 a.m. Kris Kampsen purchased $ 87.00 in alcoholic
drinks including two bottles of beer, eight Baileys 8, two Baileys, and one Capitan Morgan.
Kampsen's receipt was opened on 02/11/10 at 11:52 p.m. and was closed on 2 / 12110 at
12:19 a.m. Stephan purchased one Jack Daniels, one bottle beer, and eight Kahlua 8 Up.
Stephan's receipt was opened at 2:07 a.m. and closed at 2:55 a.m. on 02/12/10.
On 02/18 /10 at 4:10 p.m., I met with Cathleen Lewter, the bartender at the Players
Club, at 13410 Southshore Boulevard, Wellington, Florida 33414. I told Lewter I was
investigating the collision that occurred at the intersection of Lake Worth Road and 120th
Avenue South and wanted to talk with her about it. Lewter agreed to provide me with a
recorded sworn statement. The following is a brief synopsis: Lewter told me she was the
bartender at the Players Club the night before the collision. She said she saw Goodman
arrive at the club at 11: 15-11 :30 p.m. and served him at her bar. She said she saw
Goodman hanging out with Stacey Shore and cashed his tab out between 12:00-12:30
a.m., but did not see him leave. Lewter stated she received a call from Stacey Shore on
Saturday the 13thasking if she heard what happened and Stacey told her Goodman tried to
get her to go get cocaine with him and she refused. (For further information refer to the

On 02/18 /10 at 4:37 p.m., I met with Matthew Barger, the manager of the Players
Club, at 13410 South Shore Boulevard, Wellington, Florida 33414. I told Barger I was
investigating the collision that occurred at the intersection of Lake Worth Road and 120th

duty manager and did not see when Goodman arrived at the club. He said he was busy
with the restaurant and did not observe Goodman except when Goodman slipped on the
floor. He said he closed the bar down at 2:15 a.m. and Stacey Shore was still at the bar.

On 02/18 /10 at 5:35 p.m., I met with Facundo Paredes, the valet of the Players
Club, at 13410 South Shore Boulevard, Wellington, Florida 33414. I told Paredes I wanted
to get a recorded interview with him about the information he provided on 02/16 /10.
Paredes agreed to provide me with a recorded sworn statement. The information provided

On 2119/10 at 1:00 p.rn., I received a Toxicology Report from Dustin Tate Yeatman,
Toxicology Manager of the Palm Beach County Sheriffs Office. The toxicology report
showed John Goodman's blood results. Yeatman stated Goodman's blood test revealed a
blood alcohol level of .177% wlv ethyl alcohol, Hydrocodone at 42ng/mL, and
Dihydrocodeine at less than 10 ng/mL.
On 2/19 /10 at 8:49 p.m., Investigator Karly Gallart met with Deputy Ricardo Safford,
(who initially transported 0-1 back to the collision scene), at the Palm Beach County
Sheriffs Office Wellington Substation. Deputy Safford agreed to provide Investigator
Gallart with a recorded sworn statement. The following is a brief synopsis: Safford stated
he was one of the first responding deputies and arrived at 1:12 a.m. Safford stated

Goodman was not at the scene when he arrived. Safford stated while on scene he was
advised Goodman called 911 and he responded to 4193 120thAvenue South, transporting
Goodman back to the scene for medical treatment. Safford stated while he was
transporting him he smelled the odor of an alcoholic beverage and noticed Goodman's
speech was slurred. Safford stated after he turned Goodman over to Fire Rescue he had
to drive around with his windows down to get the smell of the alcoholic beverages out of his

On 02 /22 /10 at 2:04 p.rn., I met with Stacey Shore, who was in the Players Club with
D-1 on the date of the collision, at 400 Executive Center Drive Suite 201, West Palm
Beach, Florida 33401. I told Shore I was investigating the collision that occurred at the
intersection of Lake Worth Road and 120thAvenue South and wanted to talk with her about
it. Shore agreed to provide me with a recorded sworn statement. The following is a brief
synopsis: Shore told me she was at the Players Club and met up with John Goodman.
She said she was hanging out with Goodman and drinking. Shore said Goodman asked
her to go home with him. She stated she did not go home with him because he had a
girlfriend, he had been drinking, and he wanted to get cocaine. (For further information

On 02 /23 /10 at 1:00 p.m., I met with 0-1 and his attorneys at 2101 Corporate
Boulevard Suite 300, Boca Raton, Florida 33431. 0-1 agreed to consent to provide me
with two samples of his DNA, utilizing mouth swabs, and his boots he was wearing on the
morning during the collision. 0-1 signed a consent to search form and provided me with
the samples. The samples were sealed in an evidence bag and placed into evidence at

the Palm Beach County Sheriffs Office Crime Lab. D-1's attorney told me the boots were
at his other office in West Palm Beach and I could go and retrieve the boots from his office.
On 02/23/10 at 2:35 p.m., I responded to 515 North Flagler Drive, West Palm
Beach, Florida 33401, and met with Margaret Wright. Wright turned over the boots and I
verified they were the boots 0-1 was wearing the morning of the collision.
On 02123 /10 at 3:15 p.m., I inspected the boots and found a series of numbers hand
written on the inside of the boots and the manufacturer's stamp. I recorded the number 89-
22-H and the manufacturer as ML Leddy of Texas. I then contacted Joselyn an employee
at ML Leddy and asked about the serial numbers in the boots. Joselyn stated the numbers
are for a customer's measurements and are used in case the customer wants another pair
of boots. I gave the number to Joselyn and she advised 1Tlt3l1e numbers came back to a
pair of boots custom made in 2005 for John Goodman of Houston Texas. She stated the
boots were the only pair made and described the boots in my possession. The boots were
then placed into evidence at the Palm Beach County Sheriffs Office Evidence Division.
On 03/01/10 at 1:30 p.m., I contacted Misty Lynn of the Palm Beach County
Sheriffs Office Serology Department. I requested the mouth swabs taken from 0-1 be
compared to the driver side airbags removed from V-1. Lynn advised me the comparison
would be assigned and completed.
On 03/01/10 at 5:00 p.m., Investigator Sean Ramsey contacted Palm Beach County
Traffic Engineering to obtain a traffic count for traffic traveling eastbound and westbound
on Lake Worth Road in the area of 120thAvenue South.
On 03/02/10 at 10:15 a.m., I received the traffic count report from Robert Patane of

Palm Beach County Traffic Engineering. The report showed the count came from site #
3445 and was 1500 feet east of Southshore Boulevard. The count was taken on January
19,2010. The report also showed the traffic count for eastbound traffic at 1:00 a.m. was
eleven and the count for westbound at 1:00 a.m. was six.
On 03/03/10 at 10:00 a.m., I contacted Michael Proenza the district service manager
for Bentley Motors. I asked Proenza if he could provide me with information on the ability
for Bentley Motors to download crash data from the airbag control module for V-1.
Proenza told me he would contact David Goldsmith, Customer Service Manager, and ask if
they could obtain an answer to my question.
On 03/03/10 at 10:45 a.m., I met with Commercial Vehicle Enforcement Officer Jerry
Seeley in the Palm Beach County Sheriffs Office impound lot Seeley weighed both
vehicles involved and found V-1 weighed 5600 pounds and V-2 weighed 3600 pounds.
During the weighing of the vehicles, the vehicles were placed together and photographed in
relation to the maximum engagement of the vehicles at impact. I also conducted vehicle
inspections on V-1 and V-2. No known defects that contributed to the collision were found
on either vehicle.
On 03/03/10 at 3:00 p.m., I obtained, from the Palm Beach County Sheriffs Office
Communication Division, a copy of the 911 recordings, on disc, of calls made the morning
of the collision
On 03/04/10 at 2:20 p.m., Investigator Victorio Fazzino met with Robert Perreault, a
firefighter for Palm Beach County Fire Rescue, at Palm Beach County Fire Rescue Station

statement. The following is a brief synopsis: Ocoro stated she witnessed the Bentley on
the side of the roadway and called 911 at 1:01 a.m. She said she did hot see anyone
around. Ocoro stated she stopped at the scene of the collision and called out asking if
anyone needed help. She stated no one answered. Ocoro said he provided the
dispatcher with information on the Bentley and dispatch told her it was ok to leave. (For

On 03/04/10 at 2:35 p.m., Investigator Victorio Fazzino met with Scott Mock, a

occurred at the intersection of Lake Worth Road and 120thAvenue South and wanted to
talk with him about it. Kampsen agreed to provide me with a recorded sworn statement.
The following is a brief synopsis: Kampsen stated he was at the Whitehorse Tavern and
saw Goodman. He said he saw Goodman drink and eat dinner at the Whitehorse Tavern.
He said he then went to the Players Club and saw Goodman there. Kampsen said he had
a few drinks and left early with his girlfriend. (For further information refer to the

On 03 /10 /10 at 4:46 p.m., I met with Carlos Pravas at 400 Executive Center Drive

morning of the collision. He said he did not answer the phone and there was no voice mail.
He said he then received a phone call from Goodman's girlfriend, Heather Colby. He said
Colby told him to call Goodman and gave him the phone number to call. He said she also
told him Goodman was in an accident that happened at Lake Worth and 120thAvenue.
Pravas said he called the number Colby gave him and left a message. Pravas said he
then went to the scene of the collision and saw Goodman in the rescue truck. (For further

On 03/11 /10 at 2:00 p.m., I receive a letter from David Goldsmith, Customer Service
Manager for Bentley Motors. The letter stated V-1 was not manufactured with any event
data recorder, which he referred to as the "black box."
On 03/22 /10 at 1:00 p.m., I responded to the Palm Beach County State Attorney's
Office and obtained subpoenas for the telephone records of Heather Colby's, Lisa
Pembleton's and Carlos Pravas' cellular phone records.
On 3/30/10 at 2:00 p.m., I conducted speed calculations utilizing vehicle damage
and physical evidence taken from the scene. Results of these calculations revealed, V-1
was traveling at 63 miles per hour, at the time of the collision, and V-2 was traveling at 44
miles per hour, at the time of the collision.
On 04/14/10 at 5:00 p.m., I received the telephone records for Heather Colby from
AT& T wireless.
On 04/30/10 at 10:59 a.m., I met with Heather Colby at 2300 Vista Parkway, West

Courthouse to obtain an arrest warrant for John Goodman. Judge Hafele signed an arrest
warrant for John Goodman (warrant number 01-10-037896.)
On 05/19/10 , Agent Nicholas Lentini from the Palm Beach County Sheriffs Office
Tactica l Unit, in conjunction with the United States Marshals Service, located John
Goodman at the Four Seasons Hotel, 1435 Brickell Avenue, M iam i, F lo rid a 33131. Lent ini
took him into custody without inc ident. Goodman was then booked into the Palm Beach

INVESTIGA riVE SUMMARY
This investigator, given all of the facts, is compelled to conclude:
1. John. Goodman (0-1) was operat ing a black Bentley GTe (V-1). Goodman was
traveling southbound on 120thAvenue South approaching the intersection of Lake Worth
Road. Scott Wilson (0-2) was operat ing a tan Hyundai Sonata (V-2) westbound on Lake
Worth Road approaching the intersect ion of 120thAvenue South. As Wilson entered the
intersection, V-1 entered the intersection through a stop sign. The front ofV-1 struck the
passenger side ofV-2 in the intersect ion and V-1 pushed V-2 into a canal southwest of the
intersection. V-1 was traveling in excess of the posted speed limit at the time of the
collision. 0-2 died as a result of the collision.
2. The area of the collision is controlled by a posted and unobstructed forty-five miles
per hour speed limit sign for westbound Lake Worth Road and a thirty-five miles per hour
speed limit sign for southbound 120thAvenue South. These speed limit signs were erect
and free from obstruction at the time of the collision.
3. According to physical evidence, 0-1 failed to stop for a posted stop sign and collided
with V-2 at the intersection of Lake Worth Road and 120thAvenue South. 0-1 was the only
occupant of the V-1.
4. According to speed calculations, V-1 was traveling at 63 miles per hour at the time
of the col l ision and V-2 was traveling at 44 miles per hour at the time of the collision.
These speeds are consistent with the physical evidence and damage to both vehicles.

revealed a blood alcohol level of .177% w/v ethyl alcohol, Hydrocodone at 42ng/mL, and
6. John Goodman, while operating V-1, caused the this collision and made no attempt
to assist the driver of V-2 post collision, thus the driver of V-2 drowned.
7. Witnesses who stopped at the collision as early as 1:01 a.m. stated Goodman was
not present at the scene. Goodman made no attempts to flag down any vehicles for help.
Goodman traveled down a dark roadway away from the collision scene and did not make
an attempt to summon aid for 54 minutes after the first witness reported the collision.
8. Goodman drove past his residence prior to the collision and walked in a direction
opposite from his residence in relation to the collision scene.
9. Scott Wilson's death was determined by the Palm Beach County Medical
Examiner's Office due to: Drowning as a result of a motor vehicle collision.
Therefore, based on the aforementioned information, this investigator
concludes that John Goodman was operating the 2007 black Bentley GTC at a high rate of
speed while intoxicated and struck Scott Wilson's 2006 Hyundai Sonata causing it to roll
over into a canal where Wilson drowned. Goodman then left the scene of the collision on
foot and did not stop to render aid to Wilson. Therefore, Scott Wilson drowned as a result
of his vehicle coming to rest in the canal. The collision occurred before 1:01 a.m. and
Goodman did not call 911 until 1:55 a.m., and only after he called his girlfriend and was
told he should call 911 by Pembleton.

sufficient probable cause to support the following violations of Florida State Statue by John
Goodman:
1. One count of Driving Under the Influence Manslaughter Failure to Render Aid in
violation of Florida State Statue 316.193 (3)(c)(3)(b).
2. One count of Vehicle Homicide Failure to Render Aid in violation of Florida State Statue
782.071 (1)(b).

On February 12, 2010, at approximately 2:50 a.m. I was notified of this traffic fatality
by Sergeant John Churchill of the Palm Beach County Sheriffs Office. I was directed to
respond to the Palm Beach County Sheriffs Office Traffic Division to pick up and drive
the Vehicle Homicide Incident Command Truck to the scene of the crash which was
located at 120th Avenue South and Lake Worth Road, Wellington, Florida 33414.
At approximately 3:19 a.m. I arrived at the Palm Beach County Sheriffs Office Traffic
Division. I retrieved the incident command truck and responded to the crash scene.
At approximately 3:40 a.m. I arrived at the crash scene. Investigator Troy Snelgrove of
the Palm Beach County Sheriffs Office directed me to photograph the scene. After a
brief scene inspection, I began to document boot prints, within the loose, sandy, dirt road,
which led from the final resting position of the black Bentley (involved in the crash) to a
private horse farm located at 4159 120th Avenue South, Wellington, Florida 33414. I
took numerous photographs of the boot prints. These boot prints were the only prints
leading away vehicle to the residence. Further, they were distinguishable by the
distinct flat heel and sole combination and flat front toe (riding boots, cowboy boots,
etc.). Also, several measurements were taken of the boot prints. The prints were
approximately eleven and one-half inches from the heel to the toe and were documented
within several photographs. The path of travel was also documented. I took yellow
numbered triangular signs and placed one numbered sign just prior to the heel print of
each foot step. It should be noted, the numbered signs were reused after photographs had
been taken as the footsteps progressed down the roadway.

Deputy Kenneth Noel of the Palm Beach County Sheriff's Office assisted me with the
placement of the yellow numbered triangular signs.
At approximately 7:19 a.m. I removed the steering wheel airbag from the black Bentley
bearing a Texas registration plate of 380 WBJ. The vehicle identification number
(V.LN.) was I utilized a sterile scalpel and gloves while
removing the airbag. Next, the airbag was immediately placed into a brown evidence bag
which was immediately sealed with evidence tape and then labeled with the time
removed (7:19 a.m.).
At approximately 7:23 a.m. Subsequent to the steering wheel airbag being removed, I
removed the driver's frontal knee airbag. I utilized a sterile scalpel and gloves while
removing the airbag. Next, the airbag was immediately placed into a brown evidence bag
which was immediately sealed with evidence tape and then labeled with the time
removed (7:23 a.m.) .
.At approximately 8:15 a.m. I began assisting Investigator Snelgrove with the forensic
mapping of the roadway and crash scene.

INVESTIGATIVE REPORT SUPPLEMENT
On February 12, 2010 at approximately 0238 hrs, I responded to the intersection of LakeWorth Road and 120thRoad North in reference to a traffic crash involving a vehicle in a
canal. This area is located within the Village of Wellington in Palm Beach County, State
of Florida. Itshould be noted that I was already on duty this early morning conducting
my normal duties ofDUI Enforcement. I responded to this area upon hearing that a body
was located inside the vehicle pulled from the canal.
Upon my arrival, I observed the crash scene to be locked down and properly secured. I
assisted road patrol units who were already on scene by keeping the scene secure and
awaiting the arrival of investigators from the Palm Beach County Sheriff s Office,
Vehicle Homicide Investigations Unit for further instructions. I did assist Investigator R.
Stephan# 7240 of the Vehicle Homicide Investigations Unit with locating foot prints
along the roadway. I assisted by marking and photographing these footprints. I later
assisted investigators with documenting the scene as well as following the two vehicles
involved to the Palm Beach County Sheriffs Office impound lot located at 3228 Gun
Club Road, West Palm Beach, Florida 33406, Lot "C". I also turned in a blood kit
containing two vials ofthe suspects blood previously turned over to me by Investigator T.
Snelgrove# 5475 to the toxicology unit located on the second floor of the Palm Beach
County Sheriffs Office, 3228 Gun Club Road, West Palm Beach, Florida 33406. This
concludes my involvement in this case.
