New Submission

• Enter your Personnel Number – Your personal info will be pulled from IRIS

• Click Next after answering questions

New Submission• Continue filling out though page 4• Sign your name • Enter in the name and email of your

Supervisor or Department Head– This will route the Form to the next stage

• Click Submit

New Submission

• Institute for Public Service– After clicking “Submit,” you will be prompted to

agree with the “IPS Outside Consulting Policy”

– Check the “Agree” box and click “Submit”

Supervisor/Department Head

• Review the form by clicking “Review Employee Data”

• Sign Electronically

Supervisor/Department Head

• If the form needs additional approval before it goes to the CBO/Designee, click “Yes”

• Enter in the name and email of the next person

• Click next

Supervisor/Department Head

• Institute for Agriculture Submissions: – After clicking “Submit” you will be prompted to

pick which Dean to route your form to.

– If your dean is not listed (or you need to route to other people not in the list) click the check box “My dean is not listed” and enter the name and email of the next approver in the table.

Additional Approval (Optional)

• Review the form by clicking “Review Employee Data”

• Sign Electronically• Click Submit to route

the form to the CBO for review

Ag Dean Review

• Review the form

• Click “Mark as Seen”

• Enter any comments you might have

• Click “Submit” to route to next approver

CBO

• Click the buttons across the top to review data

• Enter in required data

• Next Slide will discuss the committee review

• Submit

CBO

• To send to committee for review, click “Yes”• The form will open as a PDF – save this and click

exit.• An Email will be sent with a link to access the

form for submission.

HR

• Click the buttons to review form data.

• Click “Save as PDF” to generate a PDF to attach in IRIS

• Click Attached HR Record to close the form.

Things to Note

• To avoid “locking” the form, always use the Red X ( ) in the upper right corner of the form or one of the “Exit” buttons.
