Safety Alert Number: 0000001 Issued on September 24 2020 by Peter McClare, Chief Inspector, Department of Labour and Advanced Education OTIS 12C SAFETY Legal information Issued pursuant to the Elevators and Lift Act Summary To All Class "A" Contractors: During routine testing of an Otis type 12C safety plank it was discovered that the safety wedges tended to stick in the applied position. Upon closer inspection, it was discovered that the metal plate fastened to the back of the safety wedges had become damaged and fractured preventing the wedge from moving freely in the roller cage. See attached photos for details of damage. ACTION TO BE TAKEN Safeties on 12C safety planks shall be disassembled and checked for damage. Elevators with defective safety equipment or systems shall be removed from service until repairs are completed. Inspection and findings from this bulletin shall be reported to this office by email to ELA@novascotia.ca under the heading Otis 12C safeties. For tracking purposes include Nova Scotia Installation Number (NSIN) and location of the device.
