• The Frequent Player Card (FPC) is valid from April 1, 2019 through March 31, 2020.

• The FPC is valid for, and may only be used by, the individual whose name appears on the card.

• The FPC must be presented with a photo ID at the point of sale to receive discounts and benefi ts.

• The FPC cannot be reproduced, copied, sold, transferred or tampered with in any way.

• FPC discounts and specials are not transferable or pro-ratable. Fees must be paid in full as specifi ed in the golf course fee schedule.

• The FPC can be purchased and used at any of the fi ve Palm Beach County operated golf facilities.

• The FPC discount is not available in conjunction with any other rate specials, coupons or discounts unless clearly specifi ed.

• FPC holder must give the FPC ID number when reserving a tee time.

• FPC holders may make a tee time up to ten (10) days in advance for a maximum of four (4) players. FPC holder must play in tee time booked.

• FPC holders and participants must abide by all rules and regula-tions established by the PBC Parks and Recreation Department.

• The Parks and Recreation Department reserves the right to cancel a customer’s FPC and revoke the holder’s benefi ts for violation of FPC rules and regulations or abuse of the card and/or golf course (including NO-SHOWS).

• FPC holder must report lost or stolen cards. One replace-ment card may be requested through the golf course manag-er where the card was originally purchased.

• FPC refunds will only be issued within 30 days of purchase date.• FPC holder must present the guest pass and play in the same group

as guest(s) for non-FPC golfer(s) to receive discounted price.

For and in consideration of the rental of golf carts, par-ticipation in golf programs anduse of County golf facilities at any Palm Beach County golf course, I acknowledge that the use of golf cart, participation in golf programs and the use of the County golf facility involves risks of serious bodi-ly injury, including death, and property damage. I hereby assume these risks and agree to hold Palm Beach Coun-ty, its agents, designees, employees, elected offi cials and JCD Caribbean, Inc. and its independent contractors and managers free and harmless at all times from and against claims, liability, expenses, losses, costs, fi nes, damages or causes of action of every kind and character, including at-torney’s fees and costs, whether at trial or appellate levels or otherwise, due to their acts, errors or omissions resulting in bodily harm, including death, or damage to my property incident to or in connection with my rental of Palm Beach County golf carts, participation in golf programs or use of County golf facilities. I, the renter and participant, agree to defend, reimburse, indemnify and hold Palm Beach Coun-ty, its agents, designees, employees, elected offi cials, JCD Caribbean, Inc. and its independent contractors and man-agers free and harmless at all times from and against all claims, expenses, losses, costs, fi nes, damages or causes of action of every kind and character, including attorney’s fees and costs, whether at trial or appellate levels or oth-erwise, arising during, as a result of, in connection with my use of Palm Beach County golf carts, participation in golf programs or use of County golf facilities. I promise to follow the Frequent Player Card rules and return any and all rented golf carts immediately following the completion of the number of holes paid for each occa-sion. I agree that the rented golf cart will be in as good con-dition as when I received it, normal wear being expected. I promise and agree to pay for any and all damages that are sustained or caused as a result of my use of the golf cart other than normal wear, while the same is in my posses-sion. I further acknowledge that this agreement is effective on and expires March 31, 2020.
