The Equality Fund uses an online application portal called SurveyMonkey Apply to collect

Organisational Profiles. This Help Guide provides step-by-step instructions in creating an account,

completing the Eligibility Quiz, and submitting an Organisational Profile.

PLEASE READ THROUGH THIS GUIDE AT LEAST ONCE!

In order to complete the Eligibility Quiz, prospective applicants must first create a

SurveyMonkey Apply account. Before taking this step, please be sure to fully review our

general eligibility criteria found on pages 3 & 4 of the Guidelines for Equality Fund Call for

Applications to see if your organisation meets our requirements.

While profiles must be submitted through our online form (except in exceptional

circumstances – see next bullet point), we know many groups prefer to first collaborate on

applications offline. A copy of the Organisational Profile form is available HERE.

In exceptional circumstances, we are able to accept Organizational Profiles by email. Please

email [email protected] to inquire about this option if you need it.

Create your Account

1. Go to https://equality-fund.smapply.io/.

2. Click on the “Register” button in the top right corner of the screen (Screenshot 1).

3.You will be directed to first create an

account (Screenshot 2), and then

provide your organisation’s name

(Screenshot 4).

a) Our portal allows for Single Sign On,

which means you can register and sign

into our online portal with an existing

Facebook, Twitter, or Google

account– simply click on the Facebook,

Twitter, or Google icon and follow the

instructions (Screenshot 2).

b) If you do not have a Facebook,

Twitter, or Google account or prefer

not to use Single Sign On, then fill in

the name, email, and password fields,

and select “I’m not a robot”

(Screenshot 2).

**IMPORTANT – Make a note in your

records about which email address you

use to create your account. **

After you select the “I’m not robot” option, a pop-

up box (Screenshot 3) will then instruct you to

select specific images and click “Verify” to prove

you are not a robot. When successful, the box will

disappear and then click the “CREATE ACCOUNT”

button at the bottom to continue (Screenshot 2).

c) Once you set up the account using option a) or b), you will be directed to provide your

organisation’s name (Screenshot 4). You do not need to fill out the optional information, this will be

requested in the Organisational Profile.

Verify your email address Before you can submit your Organisational Profile, you will need to verify your email address. The

eligibility quiz, however, can be completed without verifying your email address.

You will be automatically sent a verification email when you create your account. The verification

email will be sent from [email protected]. Open the email and click on the “Confirm

email address” button.

Note: If you don’t receive the verification email, check your spam folder. If you have a

Gmail account, it may be in your “Promotions” emails. To resend the verification email, click

on the “Send verification link” button at the top of your screen (Screenshot 5) – this button

will continue to reappear until you complete the verification process.

Complete the Eligibility Quiz 1. Once you have created your account and are logged in, click on the “View programs” button OR

the “Programs” button at the top of the screen (Screenshot 7).

2. Then click on the “Complete eligibility profile” button (Screenshot 8).

3. Respond to each of the 10 quiz statements to determine if your organization meets the general

eligibility criteria listed in the EF Application Guidelines document. After question 10, at the end of

the page, click on “NEXT” (no screenshot).

4. On the next page, you will see your Eligibility Quiz result. Click the "Save my profile" button at

the bottom of the screen (Screenshot 9).

5. If you are not eligible, you will be automatically returned to the

“Programs” page, but the page will be blank and there will be no programs

to select. You will not be able to continue with the application process. Thank you for your interest in the Equality Fund, and we wish you the best of luck in your funding

journey.

6. If you are eligible, when you are automatically returned to the

Programs page, there will be one or more granting programs visible

(Screenshot 10). Select the “Catalyze” program by clicking on the “More>” button.

7. Then click on the “Apply” button (Screenshot 11).

Start your Organisational Profile

8. After you click “Apply” in the previous step, you will be brought directly to your application. Click

on the “Organisational Profile” task to begin filling out your form (Screenshot 12).

9. As you complete the Organisational Profile, move to the next page by clicking on the “NEXT”

button at the bottom of the screen.

TIP 1: The system is set up to save your answers as you type them in – however, we recommend that

you regularly click on the “SAVE & CONTINUE EDITING” button at the bottom of the page.

TIP 2: You cannot continue to the next page until you complete all the answers on your current page.

However, you are able to go back to previous pages and edit answers. For this reason, we provide an

offline copy of the application so that you can complete your answers offline in collaboration with your

team, and then copy and paste your answers into the online application form.

TIP 3: You can increase the size of the text boxes by clicking and dragging the bottom right corner of the

boxes (Screenshot 14).

Sign into (and out of) your account, and continue with your application

You can sign in to and out of your account as you need, saving your application to complete later.

1. Go to https://equality-fund.smapply.io/

2. Click on the “Log In” button in the top of

the screen (no screenshot).

3. If you created your account using your

Facebook, Twitter, or Google account,

click on the corresponding button to login

with those account credentials (Screenshot

15).

If you did not use your Facebook, Twitter,

or Google account to register, type in your

email address and password and click the

“LOG IN” button (Screenshot 15).

4. Navigate to your My Applications page, then click on the green “CONTINUE” button on your

application (Screenshot 16).

5. Click on the “Organizational Profile” task to continue working (Screenshot 17).

6. To sign out again, click on your name in the top right corner, then click “Log Out” (Screenshot 18).

Submit your Organisational Profile

1. Once you have inputted all your answers and are ready to submit the Organisational Profile,

scroll to the bottom of the screen on the last page and click on the “Mark as Complete” button

(Screenshot 19).

2. You will then be automatically taken to a page where you will be able to read over the entire

Organisational Profile (Screenshot 20).

a) If you would like to change one of your answers before submitting your Organisational Profile,

click on the three dots in the top right corner and select “Edit”;

b) To download a copy of your Organisational Profile, clicking the three dots in the upper right

corner and select “Download”;

c) When you are ready to submit, click on the “SUBMIT” button on the left hand side of the screen.

3. The final step is to click on the “SUBMIT” button on the pop-up screen (Screenshot

21).

4. Once you have clicked “SUBMIT”, you will receive an email confirming your Organisational

Profile was successfully submitted. If you did not receive a confirmation and would like to follow

up, please email [email protected]. Your Organisational Profile will not be considered if

it is not FULLY submitted.

Change the language of the online portal

The online portal is available in three (3) languages: English, French and Spanish.

1. To change the language, click on the language button at the top right of the screen, and select

your preferred language (Screenshot 22).

If you’re having problems using the online application portal

1. If you’re having problems accessing or navigating the online portal, SurveyMonkey Apply offers

technical support. You can reach them by clicking on this link (https://equality-

fund.smapply.io/helpdesk/ ) or clicking on the “Info” icon at the top of your account page

(Screenshot 23), and fill out their support request form. They will be in contact within 24 hours or

less.

2. If they are unable to help, or you require direct assistance from the Equality Fund, please contact
