Stroke is the fourth leading cause of death in Hong Kong and the most common cause of permanent disability in adults. The number of deaths and the rate of disability from stroke are expected to increase in Hong Kong as the population rapidly ages in the coming decades.

Theterm“stroke”referstothesuddenonsetofneurologicalimpairmentduetovascularcauses.Therearetwotypesofstrokes:Ischaemic,whichaccountsforabout70percentofcasesinHongKongandhaemorrhagic,accountingfortheremaining 30 percent. They strike individuals regardless of age, race, or gender. During an ischaemic stroke, a blood vesselinthebrainbecomesblocked,disruptingthebloodflowandpreventingoxygenfrombeingdeliveredtotheaffectedarea of the brain. The section of brain tissue deprived of oxygen becomes “stunned” and no longer functions properly, resulting instroke symptoms. (Fig 1)

Acuteischaemicstroke(AIS)isa serious medical emergency. Permanent neurological damage is more likely to occur the longer a strokegoesuntreated.Iftreatmentis delayed, further damage to critical braincellswilloccur,knownasinfarction,whichwillreducethepotential benefits of treatment. Brain cellswillsufferirreversibleinjuryordeath if a stroke is left untreated for threehours.ThisiswhythrombolytictherapywithrtPAisgivenonlywithinthree hours of the onset of stroke. Thus, timely treatment is absolutely critical to increase the probability of a good recovery

Stroke Revisited

The old concept of cerebrovascular accident and the acceptance that therewillbegradualneurologicaldeterioration and final state of disability, or even death, have changed.Thenewterm“brainattack” implies that a stroke is not an accident. Just like a heart attack, a stroke is something preventable,

treatable, or even curable, and may leavepatientswithoutanyneurologicaldamage. Strokes can be tackled in a similarwayasheartattack.Vascularneurosurgeons prefer to perform PercutaneousCerebralIntervention(Neuro-PCI)withineighthoursaftersymptomonset.IntravenousrtPAwhenperformedwithinthegoldenthree-hourwindow,isconsideredtobesecond-linetreatment,offeredonlywhenNeuro-PCIisnotavailable.Withthisnewperspectiveonstroke, patients suffering from acute ischaemicstrokesinthefuturewillbeless miserable.

Percutaneous Cerebral Intervention (Neuro-PCI)

Neuro-PCIhasvariousadvantagesover intravenous rtPA including a longertherapeuticwindow,effectivesalvage of brain cells, minimizing the area of infarction, significantly less contraindications in patient selection criteria, etc. Depending on the patient’s causative vascular occlusive lesion, the vascular neurosurgeon maychoosetouseintra-arterialthrombolysis (ia rtPA), mechanical thrombectomy, intravascular stenting,

or a combination of the above procedures.InjectionofiartPAatthe site of the thromboembolism improves outcomes in acute ischaemic stroke, and significantly reduces the combined odds of death and dependency. During mechanical thrombectomy (Fig 2), clot retrieval devices are passed up to the occluding clot under the fluoroscopic guidance of cerebral angiograms, and the thrombus or embolus is removed by the device through mechanical breakage and aspiration. Intravascularstents(Fig3)maybedeployed if concomitant vascular stenosis exists.

Screening for Stroke

Themajorityofstrokesarepreventable and thus prevention continues to be a key aspect of reducing the burden of stroke for individualsandthewidercommunity.This can be achieved through screeningindividualsforknownriskfactors for stroke and implementing management of modifiable risk factors.

Hong Kong Adventist Hospital (HKAH) cardiologists Dr Chris Wong,DrBoronCheng,andDrRyanKoarethefirsttodeliverMitraClip treatment in China, providing an advanced and minimally invasive procedure to treat heart valve disease. To date, the Hong Kong Heart Center has successfullyimplantedfivepatientswithMitraClips,andtheyareallrecoveringwell.

Inastringoffirsts,HKAH’s Heart CenterwasthefirstprivatehospitalinthecitytolaunchTAVIandtoday,achieves another milestone in its HeartProgramwithMitraClip.

Small Clip, Big Difference

Existing treatments of mitral regurgitation include medication to manage the symptoms and invasive open-heartsurgery,atreatmentthatmightnotbeofferedtopatientswithsevere mitral regurgitation and other significantcomorbidities.

The MitraClip system, a percutaneouscatheter-basedtherapy, reverses this adversity. MitraClipisasmallclipthatallowsthe mitral valve to close more completely, and therefore helps restorenormalbloodflowthrough

Hong Kong Adventist Hospital’s Heart Program Launches its First MitraClip Treatment for Severe Mitral Regurgitation Patients

Forthepastfewyears,76-year-oldMsChanhas had bouts of heart failure. She has a history ofcoronaryarterydiseasethatwastreatedwithcoronary artery stenting. An echocardiogram also showedadilatedleftventricle,poorejectionfractionat 20% and severe functional mitral regurgitation. Shewaslateradmittedtohospitalagainfordecompensatedheartfailure,aswellasdyspnoea,orthopnoea and bilateral pedal edema. After admission,shewastreatedwithoxygensupplement,intravenous diuretic, and inotropic support. Her conditionfluctuatedduringhospitalization,andafter her assessment by cardiologists at Hong KongAdventistHospital,shewasrecommendedtoundergo MitraClip therapy.

Three experienced interventional cardiologists, an echocardiographer, and an anesthetist carried out hertherapy.MsChanunderwentgeneralanesthesiaandafterafour-hourprocedure,aMitraClipwassuccessfully implanted. The functional mitral regurgitationwasreducedtominimal(Fig1and2).

After her MitraClip therapy, Ms Chan’s clinical symptoms of dyspnoea and pedal edema improved. Therewasspontaneousdiuresisafterthetreatmentandshewasdischargedthreedaysafterhertherapy.

Insummary,theoutlookforseverefunctionalmitralregurgitationwithpoormyocardialfunctionistypicallypoor.Open-heartsurgeryisnotalwaysanoptionasthesurgicalriskisveryhighandthemid-tolong-termresultsmaynotbesatisfactory.MitraCliptherapy is an alternative therapy for this group of high-riskpatientsasillustratedbyMsChan.

This minimally invasive procedure demonstrated an acute procedural success rate of 98 percent during medical trials, and effectively reducedre-admissioncasesby45percentwhencomparedtooptimalmedical therapy that treated high-riskpatientswhowerenotcandidatesforopen-heartsurgery(1).

MitraClip Closes Treatment Gap The advantages of MitraClip includeasmallwoundsize(assmall as six millimeters) and the fact that a patient can be dischargedfromthehospitalwithinonetotwodays.Whileopen-heartsurgery is still the golden standard of therapy at this moment, it is not suitable for some patients such aselderlypatientswithpoorheartfunctionwhoareoftenforcedtogiveuptreatment.WithMitraClip,thereisnewhopeasitoffersanalternative to current treatments.
