Practice history Pre 1997 No GP practice in Iwade. 1997 A locum GP set up a satellite practice. Operated three days a week from the village hall building, and subsequently from a house. 2004 The locum retired. 2004 Iwade Health Centre was built and became operational. 2018 Providers put several locum practitioners in place but lack of continuity of patient care was a concern to patients and ultimately saw the present Patient Participation Group (PPG) mobilise and the Care Quality Commission intervene. DMC management results 2018 Feb DMC was contracted the management of Iwade due to inadequate CQC ratings. 2018 June Iwade had earned a ‘Good’ rating from the CQC. 2018 July DMC established Community Services centre in building 2018 August Contracted local workmen to freshened up the building. Improved the experience and quality of service for patients. Created wellness awareness campaigns. Established a good working relationship with the PPG. PPG engagement PPG meet monthly with practice manager, with public meetings every six months. Collaborate on waiting room exhibition piece celebrating local history and introducing DMC to patients. Digital service DMC set up a new, user friendly practice website through which patients could easily access vital contact and hours info, health resources, as well as book appointments and order repeat prescriptions. Environment: We want patients to be as comfortable and assured as possible while in the waiting room. We painted the walls, replaced outdated info displayed with more welcoming imagery and helpful health information.
