Overview§ The new Office of Long-Term Living (OLTL) Services and Supports Directory (SSD)
allows individuals receiving OLTL services, individuals interested in OLTL services, family members, Service Coordinators and the general public to access timely, up-to-date information on providers and services being offered in their area
– A complete catalog of providers and services can be downloaded and printed for individuals when necessary
§ Information in the OLTL SSD search results and catalog of providers comes from provider information in HCSIS
§ Users can search for providers or services by entering an address, zip code or county– Search results are delivered in random order; a maximum of 60 results are returned with
each search
§ The OLTL SSD allows the user to complete an Advanced Search which provides additional search criteria
§ The OLTL SSD can be accessed directly through the link below or through a link on the OLTL homepage

OLTL Services and Supports Directory HomepageThe SSD Homepage allows the user to access important information about the Office of Long-Term Living.

1. Access a complete list of OLTL Service Definitions here.
2. Begin your search; search by Geography or by Provider/Service.
3. Click Learn about HCBS Waivers to navigate to the DPW site that provides information about the 12 HCBS waivers DPW administers.
4. A complete catalog of all OLTL providers grouped by county can be downloaded here.
§ The file is refreshed once a week; users may download and print from the PDF file if a hard copy is required.
Search by Provider or Service CategoryEnter search criteria on this screen.

1. To search by provider name, enter at least four letters of the provider’s name (not necessarily the first four letters).§ For example, if the user enters
“heal” the search will return all providers with the word “health” in their name (i.e., Health Group, Berks County Health Group etc.)
2. To search by service category, click the name in the Service Category list box and click [Select].§ All services can be selected by
clicking [Select All].
§ Services can be removed from the Selected Service Category list box by clicking the category name and clicking [Remove].
§ Click [Continue] to review the search results.
3. Click [Advanced Search] to access additional search criteria. (See screen on page 6)

1. Click a Service Category from the Service Category list box.
2. When the user selects one or more Service Categories, the Service Name list box is populated with services in the category(s) selected.
3. Review the services in the Service Name list box and select one service or select all of the services by clicking [Select All].§ Services can be removed
from the Selected Service Name list box by clicking the service name and clicking [Remove].
§ Click [Continue] to review the search results.
4. Select one or more Program/Waiver to view services provided through the Waiver or Program.
Geographic Search CriteriaUsers can enter geographic search criteria on this screen.

Search by Address, Zip Code or County by clicking the correct tab. When the user enters an address or Zip Code it is converted into a county and the search results contain a list of all providers in that county.

Users can select up to three counties by selecting the county on the state map and clicking [Select].

Create a custom list of preferred providers by clicking the checkbox next to the provider’s information and clicking [My Selections].
Search results include the provider’s name, main office address, website (if available), provider contact title, phone number, and counties served. Each search returns a maximum of 60 providers.
Click the provider’s name to view detailed information for that provider.
These links allow the user to modify the search, return to the Search Criteria screen to begin a new search, or print search results.
My Selections

My Selections functionality allows the user to create a custom list of preferred providers. This list can be modified or printed from this screen.
Return to the search results or print the My Selections list here.
Modify the My Selections list by clicking select next to the provider’s name and clicking [Remove].
