or another dementia through services tailored to meet individual needs.

Our 3 programs include: Free door to door transportation Breakfast, lunch and snack Oversight by: medical director, pharmacist, occupational therapist, speech therapist, dietician, nurses andsocial workers Coordination with primary physician and caregiver(s) Medication monitoring &
