Nursing Unit

Based on the vision and mission of University of Dammam, Nursing College & the undergraduate program, the Vision and Mission of Obstetrics & Gynecologic Nursing Department will be as follows:

Vision:

Providing the Saudi community with high qualified professional nurses in Obstetrics & Gynecologic Nursing care who are unique on National and International levels. Sharing in an enhancement of Women's health through the application of nursing education and researches in Obstetrics & Gynecologic Nursing field.

Mission:

The Obstetrics & Gynecologic Nursing Department, College of Nursing, University of Dammam (UOD) prepares the Saudi undergraduates and postgraduates to be a competent, skillful and creative in delivering the needed high quality nursing care for healthy and high risk women throughout pregnancy, labor & delivery, postpartum, newborn and women with gynecologic problems in different Obstetrics & Gynecologic health settings. These are derived from recent National and International nursing strategies within the framework of Islamic belief and moral values.

Guidelines of OBGYN Ng. Skill Lab

ObGyn Skills Lab Guidelines The ObGyn nursing skills lab at the nursing college provides nursing students with an additional learning resource to meet their educational goals. The lab is equipped with instructional videos and DVDs, supplies and equipment, as well as practice manikins including Full body Pregnancy Simulator, obstetric manikin, pelvic models, Ultrasonography simulator (Biometric and Gynaecology) as well as high fidelity childbirth simulator. The lab provides students with a realistic, simulated clinical environment to practice and demonstrate competency of selected nursing skills. Open lab times are posted at the beginning of each semester. Please keep in mind that the following guidelines should be followed during your attendance in the ObGyn nursing skills lab: • No eating or drinking in the lab. • Please turn your cell phone off or on vibrate before entering the lab. • PLEASE clean up after yourself. • Keep all beds in appropriate positions with linens straightened before leaving. • Students are required to return clean equipment to its proper place after use. • All simulated practice needles are to be disposed in sharps containers. • Needles and syringes are not to be discarded in trash containers. • If you are accidently stuck with a non-sterile needle or your skin comes in contact with contaminated material, you must immediately go to the Lab Coordinator. • Lab resource books, movies and supplies MUST NOT leave the lab. • Please return all media to proper storage areas or to lab personnel after use.

• Please save equipment obtained in labs for practice during open lab times. • Report any incidents or malfunctions to the Lab Coordinator immediately. • Do not move mannequins. If a mannequin needs to be moved, please, contact lab coordinator. • Keep personal belongings with you during the lab session, please take them with you when leave.

*If a latex or betadine allergy exists or is suspected, please notify your clinical instructor or lab coordinator.

Obstetrics & Gynecologic Skill Laboratory “I hear, I forget; I see, I remember; I do, I understand” – Confucius

Goal The goal of the OB/GY Nursing Skills and Simulation Laboratory is to promote

safe, knowledgeable and effective nursing care by demonstrating and reinforcing

the highest level of performance and readiness.

Introduction/Philosophy The OB/GY Nursing Skill and Simulation Laboratory is here to make the student’s

clinical experience educational and enlightening and to promote empowerment of

the student. Scenarios and simulation experiences relate to the OB/GY Lab

objectives. Students will be oriented to simulations instructions prior to running a

scenario. If the student is not comfortable, learning will not take place and scenario

objectives may not be met. Simulations and case scenarios mimic the clinical

setting and are designed to help the student develop problem-solving and decision-

making skills. Simulations include all environmental factors to make students’

learning realistic and authentic. These simulations help students think on their feet

and help the transition from lab to clinical. For an enhanced learning experience,

students must come to the lab prepared. The faculty will provide debriefing and

positive feedback; students will self-analyze their performance and use critical

thinking during the reflection process.

General Information The OB/GY Nursing Skill and Simulation Laboratory is located in Dammam

University, College of Nursing, C3. Room # has an Interactive Child Birth

mannequin/Simulator (NOELLE Maternal and Neonatal Birth simulator),

LUCINA (High fidelity childbirth simulator), Biometric & pelvic ultrasonography

simulator and a Full Body Pregnancy Simulator. The NSSL simulates a hospital

care setting. The OB/GYN Skill Lab is fully equipped to practice all OB/GYN.

nursing clinical skills served both undergraduate and postgraduate students. A

variety of task trainers are also accessible as well as the ability to view a variety of

media. Mentoring and tutoring are also available.

The OB/GN. lab is opening 5 days a week (Sunday to Thursday) according to the

students’ schedule and either they are undergraduate, post graduate & bridging

students.

The lab schedule is subjected to change: The calendar for the OB/GY Nursing Skill and Simulation Laboratory is posted on

OB/GYN lab. Bulletin.

Confidentiality All simulation scenarios practice sessions involving students and/or recordings are

considered confidential. All mannequin accessibility should be treated as a real

patient (including inappropriate viewing). Discussion of scenarios or information is

considered a violation of Practical Nursing Program privacy policy. All students

OB/GY Nursing Skill Laboratory’s Objectives: Upon completion of the laboratory rotations the students will be competent in performing different procedures through using different models & simulators available in the OB/Gyne skill lab and they will be able to:

x Identify the anatomical parts in both male & female reproductive system

through using models (bony pelvis & soft tissues) and video tapes.

x Identify different stages of fetal development using different fetal

development models.

x Perform complete physical assessment for women during antenatal period

through using full body simulator and other lab resource as CTG, video

tapes, (plasma screen) and posters.

x Assess the parturient physical condition & the progress of process of normal

labor through using interactive childbirth simulator, and different models

available in the lab.

x Perform complete physical assessment for women during post partum period

through using full body simulator & childbirth simulator.

x Prepare delivery set and instruments and equipments needed during labor.

x Use the full body pregnant simulator to demonstrate the different fetal

positions attitudes, lies, and presentations.

x Use different interactive childbirth simulator to help students to formulate

nursing care plan according to each scenarios.

x Perform physical examination & resuscitation for the newborn using

interactive childbirth simulator.

x Assess any deviation from normal during labor using interactive childbirth

simulator.

x Interactive child birth simulator ultrasound scanning can be used to detect

any deviation from normal during pregnancy.

x Assess female genital system to help students dealing with women having

any gynecologic problem.

x Identify different instruments used for gynecologic women.

x Apply universal infection control precautions & safety measures while

providing care to the women during antenatal, labor & postpartum,

Newborn, as well as women who have any gynecological problem.

Schedule of Occupancy & Evaluations in OBGYN Skill lab for

Undergraduate Students

x As regards clinical training of undergraduate students in the 2nd semester, At the

beginning of the semester the lab was occupied for the 1st 3 weeks as

incorporation between theory followed by application of variable procedures

related to the same lecture for demonstration by using models for training

(anatomy models), the full-body simulator, interactive childbirth simulator,

CTG, postpartum procedures and Gyn. procedures as a preparatory phase for

interaction with women in the actual health care settings (hospital & or centers)

followed by continuous lab training through clinical lab rotation for all students

groups according to their schedule throughout their semester.

x Continuous evaluation was held for each student to be competent while

performing procedures in the skill lab.

x At the end of each lab rotation, clinical exam. was held through using checklists

& simulation assessment to evaluate their performance, through application of

different procedures using the simulators and models

x At the end of the semester, OSCE. Will be held through using variable

stations in the OBG/YN, Pedia and Newborn Skill labs for each student to

evaluate students' performance, through application of different procedures

using the simulators and models. Also clinical scenarios will be held in the

OBGYN Skill lab, through applying the scenarios on the simulator, and

formulate nursing care plan according to each scenarios and other stations

that measure students' ability for taking history and communication skills.

1 Prepare the woman by: Explaining the procedure and Instruct her to empty the bladder then

instruct the woman to lie on her back, with knees flexed slightly (dorsal recumbent

position), place a small pillow or rolled towel under client’s right hip.

2 Close the door or close the curtains. Properly drape the patient.

3 Perform abdominal palpation (Determine fundal level)

Stand at the foot of the bed facing the face of the woman and measured in centimeters from

the top of pubic bone to the top of fundus,correllates with the current weeks of pregnancy.

4 First maneuver (fundal grip ) (content of the fundus) ( determine the fetal lie and

presentation)

Stand at the foot of the bed, facing the patient and gently place both hands flat on the

abdomen palpate upper abdomen with both hands (Use palms not fingertips). Palpate gently

but with firm motions ,determine if the mass palpated is the head or buttocks by observing

the relative consistency ,shape ,and mobility

5 Second Maneuver ( lateral maneuver) (Determine the fetal lie, presentation , position and

attitude)

Facing the woman place the palmar surfaces of both hands on either side of the abdomen

and apply gentle but deep pressure, Hold one hand still while using the flat surface of the

fingers on the other hand to gradually palpate the opposite side from the top to the lower

segment of the uterus, palpate the fetal outline.

6 Third Maneuver (1st pelvic maneuver) (determine degree of engagement)

Gently grasp the lower portion of the abdomen, just above the symphysis pubis, between

the thumb and the fingers of one hand, Press thumb and fingers together in an attempt to

grasp the presenting part.

7 Fourth Maneuver ( 2nd pelvic maneuver) (determine how fare fetal head is flexed)

Stand Facing the client’s feet, Place tips of first three fingers on both sides of the midline

about 2 inches above the Poupart’s ligament, Exert pressure downward and in the direction

of the birth canal, moving skin of the abdomen downward along with the fingers. Slide

fingers of other hand as far as possible.

8 Use the doppler with ultrasonic gel, locate and determine the FHS.

Wipe the extra gel in the abdomen with tissue, redress her and asset her down of the bed.

9 Report any deviation from normal.

10 Record all pertinent information. lie, whether presenting part is flexed or extended, engaged or free or floating ,presentation, position, FHS, vaginal discharge , and report any abnormal findings

1 Welcome the woman, introduce yourself.

Place the woman on the examination couch and explain the procedure to her.

2 Drape the woman and keep the doors and curtain closed.

3 Wash your hands.

4 Inspection:

a) On sitting position: Ask the client to sit in comfortable position facing the examiner

x With arm relaxed at sides. x With arms held over head. x With hands on hips ,pressing in to contract the chest muscles b) On the supine position: Ask the client to assume the supine position and put

her right arm over her head and inspect the right breast (Reverse this step for the left breast).

5 Palpation :examine the right breast on the supine position:

x Put right hand behind head. Use pads of fingers of left hand, held flat together, gently press on the breast tissue using small circular motion, imagine the breast as a face of a clock. Beginnings at the top (12 O’ clock position) make a circle around the outer area of the breast.

x Move in one finger width, continue in smaller and smaller circles until you have reached the nipple (cover all areas including the breast tissues leading to the axilla)

x Reverse the procedure to the left breast.

Labor and delivery Scenario

Labor Scenario:

Noelle is 24 years old female. Weighing 150 pounds (68 kilograms). Her OB history shows a para 1, a gravida of 2 and that she is currently 39 weeks pregnant; she has had 0 spontaneous abortion, 0 elective abortions, and 0 stillbirth.

Labor and delivery lab role-play:

First step: Student will be divided into 2 scrubbing nurses and 1 circulating nurse.

Second step: The scrub nurse will be responsible for delivery set preparation with the consideration of which type of set should be prepared and at what time.

Third step: the second scrubbing nurse will conduct the delivery of the fetus and the placenta and monitor the uterine contraction during labor process.

Fourth step: the circulating nurse will be able to examine the placenta after delivery. The students should work together as a team exactly as if working in the delivery room.

Fifth step: Each student must document her role in the nursing record and answer some questions related to the scenario.

Finally, at the last half an hour debriefing started and each student will gave her feedback and the instructor will respond to students' questions and for explanation.

After reading the scenario carefully each student will pick up one paper each has

the task that will be conducted: (delivery set preparation, monitoring of uterine

contraction during labor and placental examination).
