SCHOOL OF NURSINGOFFICIAL IMMUNIZATION RECORDS
All students must provide official immunization records and verification of a physical exam within the last 12 months from their health care provider(s).

VERY IMPORTANTPlease read prior to starting immunizations!!!
Prior to administration of vaccines, please discuss with healthcare provider all vaccinations needed in order to begin clinical. The following items need to be taken into consideration when receiving vaccinations:
Live vaccines can be administered on the same day, if you were to have an MMR one day and then attempt to have the Varicella a day or two later the healthcare provider would not be allowed to administer the later vaccine. Student would need to then wait 45 days before the additional vaccines could be administered.
Although TB test can be placed on the same day as receiving live vaccines, TB test can not be placed a day or two after receiving a live vaccine. If this occurs student would need to wait 30 days before having the TB test placed and later read.
IMMUNIZATION EXPIRATIONDATES
Students must be current on all immunizations and CPR certification for the duration of the semester. Month/day/year are required for all immunizations. Immunizations cannot expire during a semester.
Immunizations with expiration dates must be good through the upcoming semester. Fall admits – good through early December Spring admits – good through early May FastBacc Summer admits – good through mid August
Immunizations must be updated prior to the beginning of each semester if expiration falls during the semester.
The above specifications are set forth by contracts between Baylor University School of Nursing and clinical agencies.
TB TESTS
TB test are good for one year. Documentation to include: Date placed Date read Results – Negative/Positive Induration (i.e. 0mm, 2mm, …)
If you have had a positive TB test, the following documentation is required along with documentation from the positive reading. Chest X-Ray report
HEPATITIS B
Student must have one of the following: HEP B, 3 part series Titer confirming immunity
Series must be completed by 9/5/2017
MMR(Measles/Mumps/Rubella)
Student must have one of the following: MMR, 2 part series (must be complete by 9/5/2017) or Titer confirming immunity to all three diseases – Measles, Mumps, Rubella
If a student is unsure whether they have had the immunization or disease, a blood titer can be drawn to verify immunity.
TDAP(Tetanus/Diphtheria/Pertussis)
Good for 10 years.
Tdap As long as it has been at least 45 days since an individual received
a Td, the Tdap can be administered.
VARICELLA(Chicken Pox)
Even though you may have had chicken pox, if you were born after 1979, students are required to provide documentation of one of the following: Varicella Titer showing immunity Varicella vaccine (completed 2 part series by 9/5/2017)
If born before 1980 and have not had chicken pox you will need to provide one of the following: Varicella Titer showing immunity Varicella vaccine (completed 2 part series by 9/5/2017)
FLU
Flu Vaccine is required for all students.
Current Flu Vaccine being administered as of August of the current year. Fall Admits – 2017-2018 Flu Vaccine due by 10/2/2017
(allowing students to receive vaccine administered beginning late June – early August)
Documentation submitted must include: Date of vaccination Lot # Manufacturer name
NURSING HEALTH FORM
Physical Confirmation Examination form: Confirming physical within the past 12 months
Physicians office will need to complete form and sign
If student had a physical within the past 12 months, in lieu of having physicians office complete form, documentation of the physical from physician’s office can be attached to form.
In place of physician’s signature on form, student will write “See Attached”.
CPR CERTIFICATION
American Heart Association Basic Life Support for Health Care Provider
Certification must be current through each semester and is best if it runs through the entire time at School of Nursing.
Student will receive Certificate from agency confirming completion. Some agencies are still using the CPR card – if student receives a card they
will need to sign the back and upload the front and back of card. Certificates do not need to be signed and can be uploaded as received.
PERSONALHEALTH COVERAGE
Students are required to have personal health coverage. Copy of insurance card/documentation uploaded to CastleBranch account NOTE: if students name is not listed on card – will need to include verification
confirming student is covered under insurance If card or document has Social Security numbers listed other than the students,
make a copy and black out socials before uploading – CastleBranch cannot accept documents with another persons Social Security number listed.

MENINGOCOCCAL(Meningitis)
This is a requirement by the State not clinical sites – Meningococcal documentation is to be sent to the Universities Health Center – The School of Nursing does not need a copy.
“A Texas State law states ALL new students under the age of 22 must show proof of meningococcal vaccination within the last 5 years and at least 10 days prior to the first class day.”
Students MAY NOT register or attend class without providing proof of the Bacterial Meningitis Vaccination or an appropriate exemption form to the Universities Health Services Office in Waco.
Therefore, any student, under the age of 22 on the first day of class, who is new to Baylor University or has had a lapse in attendance during a long semester (fall or spring) will be required to show proof of the Meningococcal Vaccine within the last 5 years.
