Background: Building of Partnerships
• Initially established in collaboration between NSW Ambulance and The Children’s Hospital at Westmead where paediatric palliative care plans were put in place to support children in their home
• 2012/13 piloted adult palliative care plans at Veronica RACF Central Coast and across community patients in Southern NSW under the care of the Amaranth Foundation
The Purpose
NSW Ambulance Authorised Care Program was set up to: •Support paramedic decision making in meeting the needs of individual patients with specific medical conditions
•Respecting palliative wishes when an ambulance responds to where an Authorised Care Plan is in place.

Accessing the Plans
Requests for Palliative and General Authorised Care Plans can be obtained through NSW Ambulance by:
•NSW Ambulance Intranet site
•Request via email [email protected]
•Telephone 9779 3808 or 9779 3821
Completing the Plans
Processing of the Plans
•The Plan is clinically reviewed •Discussed with treating clinician if required •Forwarded for endorsement •Once endorsed notification uploaded on
computer system •Letter of reply sent to patient and treating
clinician with endorsed plan enclosed
Statewide Availability
Information and plan templates have been widely distributed: Local Health Districts Medicare Locals – Including uploading on Health Pathways Age Care Provider Groups Individual RACFs A GP Information Booklet has been developed and distributed via the Medicare Local
Current Status
Since inception1,100 plans have been registered Currently we have: • 414 Adult Palliative Care Plans in total (299 regional and 115 metro locations) • 76 General Authorised Care Plans in total (21 regional and 55 metro) • 38 Paediatric Palliative Care Plans in total (10 regional and 28 metro)
