issued on producing hard copy from the School Office

(Senior Building) from 12th November, 2018 to 14th

November, 2018 from 10:30am to 12:30pm.

Documents Required for issuance of Form:

The Birth Registration Certificate in original (Hard Copy

only).

COST OF FORM : Rs. 500/-

Form Submission Date : 16.11.2018

(Timing : 10:30am to 12:30pm at Senior Building)

Documents to be submitted :

1. Xerox copy of Birth Certificate.

2. Xerox copy of Aadhar Card.
