We are pleased to have added new providers to your Preferred Provider Directory. With preferred

providers available, you can save yourself and your Trust Fund money by selecting preferred

providers whenever you need medical, dental or vision care.

REMEMBER Your Network is in a constant state of change. New Providers may enter the

Network. Currently listed providers may no longer participate. Before services are rendered,

please verify Preferred Provider status by calling the Healthcare Services Department at your

Trust Fund Office, (775) 826-7200, before making an appointment with a provider you have not

seen before or to verify that a current PPO contract is in effect. It is your responsibility to verify

that your choice of provider and/or location of office/facility are in fact a Preferred Provider.

Failure to do so may result in reduced benefits and higher out-of-pocket expense to you.

Just because a specialist is listed in your Directory, does not mean that all services provided are

covered benefits. READ YOUR BENEFIT BOOK CAREFULLY. If you do not understand it or are

uncertain if a service is a covered benefit, call the Trust Fund Office for assistance.

PRESCRIPTIONS Whenever possible use PPO pharmacies and always show your ID card when

filling prescriptions. To file a prescription claim, please tape prescription receipts in date order on

a blank piece of paper and then attach to a completed claim form. You must use a separate claim

form for each family member. Our mail department THANKS YOU!

PRE-CERTIFICATION When obtaining any of the following types of care or treatment; it is

strongly recommended that your provider contact the Trust Fund Office to obtain a pre-

certification and to confirm that the facility or other health care provider is a preferred provider:

Hospital Admission:

Same-day surgeries(unless done in a physicians office):

Outpatient radiology testing: MRIs, MRAs CT Scans, PET Scans, DEXA Scans,

Nuclear Medicine

Any biopsy (done in an outpatient setting)

Paracentesis/Thoracentesis

Inpatient treatment for alcohol or substance dependency **(MUST be approved

by ARP before services are rendered)**

Inpatient treatment for mental/nervous conditions

Home Health Care / Home I.V. Therapy / Home P.T.

Chemotherapy and Radiation Treatments

Notification is recommended for the following types of care:

Medical Equipment: Purchase or Rental

Under this Plan, prior approval of services is NOT REQUIRED and benefits will not be reduced if

pre-certification is not obtained. Pre-certification confirms the medical necessity for the medical

services; it however, DOES NOT guarantee that the claim will be paid by the Plan.

**Claims will NOT be paid if prior authorization was not obtained from ARP prior to services being

rendered.**

I. DENTAL SERVICES

DENTISTS - GENERAL If you use one of the following Preferred Provider dentists, the Northern Nevada Operating Engineers Health & Welfare Trust Fund will reimburse 90% of a set discounted fee schedule for preventative & basic services, 60% for prosthodontic services, not subject to a deductible, up to a calendar year

maximum of $2,500.00 per eligible person (Calendar year maximum does not apply to dependent children to age 26). All other Plan limitations apply for covered charges.

Orthodontic Benefits: $2,500 lifetime maximum, children to age 26 (Through age 25) Banding fee limited to $500.00.
