Search and Recruitment Interim PlacementCompatibility Assessment

Strong knowledge of Hospice/Homecare regulations and Medicare/Medicaid reimbursement.

Demonstrated ability to develop, lead and support staff to include mentoring, leadership development and alignment with program and hospital roles across the clinical care continuum.

Successful Strategic Planning/Business Development within Hospice Care.

Demonstrated successful engagement, partnering and alliance building with a broad range of constituencies.

Proficiency in cultivating excellent physician & staff relations.

The incumbent needs to be a credible leader within hospice/homecare operations, planning and business development. The qualified candidate will have a Master’s degree in Health Care, Business Administration, or related field. BSN is required. A minimum of two to five years of healthcare leadership experience, and a minimum of two to five years of hospice/homecare experience required. Must be able to obtain an AZ Nursing License.

Northern Arizona Homecare & HospiceNorthern Arizona Hospice, which was founded in 1984, is the only nonprofit hospice organization in the greater Verde Valley and Sedona area. Northern Arizona Homecare is licensed by the state of Arizona and is Medicare certified. They are affiliated with Flagstaff Medical Center and Verde Valley Medical Center. Their services cover a wide geographic area including Flagstaff, Williams, Winslow, Munds Park, Verde Valley, Village of Oak Creek, Sedona, Cottonwood and Jerome. They strive to provide their homecare patients high-quality healthcare with a personal touch. Their goals are to: 

Help the patient get well or maintain health at home. Provide care that respects the independence, choices and needs of the

patient and their families. Provide health education to the patient and their family. Provide end-of-life care so the patient can live each day to the fullest.

Care may be provided by registered nurses, nurse aids, physical therapists, speech therapists, occupational therapists and clinical social workers as ordered by the patient’s own attending physician. All disciplines are licensed or certified to the highest level in their field.

Northern Arizona Homecare Services

Home Infusion I.V. antibiotics, nutritional infusions and other special-need infusion therapies are performed at home by a registered nurse.

Case Management Nurse case managers assist in coordination of care to help patients and families handle complex healthcare issues.

Skilled Nursing Registered nurses and home health aides provide post-surgical care, prenatal, complex wound care, diabetic management and many other home-based services.

Skilled Therapy Professionals in physical therapy, occupational therapy, and speech and language therapy help patients work toward rehabilitation without leaving home.

Social Services Clinical social workers help locate community resources, counsel patients and their families, provide financial counseling and help with planning for the future.

Personal Care:Bathing, personal care, light housekeeping and meal preparation are among the personal services available to patients who need them.

Northern Arizona Hospice Home

Northern Arizona Healthcare now operates the Verde Valley's first hospice house. The home is staffed 24 hours a day, seven days a week by compassionate, personal caregivers who are trained by Northern Arizona Hospice, a service of Northern Arizona Healthcare. Care teams, which include board-certified physicians, nurses, social workers, pharmacists, therapists and volunteers, develop a care plan for each patient and his or her family to help them face the challenges of life's last days with dignity, peace, and comprehensive support. The team provides a variety of services and care options to meet the physical, emotional and spiritual needs of residents and their families.

The 12,000-square-foot home features 10 patient rooms, each with a private bath, outdoor patio and views of the red rocks. It has a large common kitchen and gathering area, as well as a spacious sitting room for patients and families to spend time together. There is a meditation room for prayer and reflection. In addition, the home hosts a physician consultation room along with a nurse's station and office.

Infusion Therapy Services Northern Arizona Homecare Infusion Pharmacy is the only independently state licensed homecare infusion pharmacy in Northern Arizona. Their pharmacists and technicians offer personal, knowledgeable and friendly service. Patients receiving

infusion services may or may not include nursing or therapy services, depending on the care needed. Available infusion services are:

Adult or child nutritional feedings, such as TPN Hydration therapy IV antibiotics for systemic infections Special needs infusion therapies for treatment of illnesses, such as Multiple

Sclerosis and illnesses of immunocompromised patients
