It has been noted that some physicians lack knowledge related to the most effective

treatments for diabetic patients as well as awareness of the importance of the collabora-

tive approach to care and its impact on patient quality measures (ADA 2017). Physicians

should demonstrate competence in identifying optimal therapeutic interventions to pre-

vent cardiovascular and other complications of diabetes. They should develop and im-

plement appropriate evidence-based treatment strategies and assess the effect of care

on various quality measures. Physicians should effectively identify and discuss expected

and unexpected patient outcomes as well as the adverse effects of diabetes therapies

and identify additional practice gaps that should be addressed in the future. They should

function effectively in a collaborative care environment. To address this professional prac-

tice gap, we propose to develop and present a performance improvement based 1½ day

CME program for primary care physicians and other allied health professionals, e.g. nurs-

es and dietitians. Recent published work shows that performance improvement CME pro-

grams in diabetes have a positive and measurable impact on the performance of primary

care physicians treating diabetes.

Target Audience

For this symposium, the target audience is Physicians, Physician Assistants, Fellows/Resi-

dents, Nurse/Nurse Practitioners, and Dietitians. We will update health care providers with

state of the art information regarding evaluation and treatment of patients with diabetes.

Physicians Texas Heart Institute is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

Texas Heart Institute designates this live activity for a maximum of 11.25 AMA PRA Category 1 Credits™. Physi-cians should claim only the credit commensurate with the extent of their participation in the activity.

Nurses CHI St. Luke’s Health–Baylor St. Luke’s Medical Center is an approved provider with commendation of con-tinuing nursing education by the Texas Nurses Association – Approver, an accredited approver with distinc-tion, by the American Nurses Credentialing Center’s Commission on Accreditation.

Up to 11.25 contact hours will be awarded for successful completion of this activity. To receive a certificate of completion with awarded contact hours, you are required to sign-in at registration each day, attend the entire program, and return the completed evaluation form for each day.

Dietitians Up to 11.0 CPEUs (Level 2) have been pre-approved with the Commission on Dietetic Registration (CDR) for Registered Dietitians (RDs) upon completion of this program.

At the conclusion of this symposium, participants should be able to:

• Describe novel and optimal therapies for treatment of type 1 and type 2 diabetes. • Discuss and generate evidence based treatment strategies for management of type 2 diabetes based

on recently published clinical trials regarding cardiovascular and renal outcomes with SGLT-2 inhibitors and GLP-1 Receptor agonist.

• Describe the management of hyperglycemia in hospitalized patients. • Incorporate effective communication techniques within a multidisciplinary team in weight

management in obese patients with diabetes. • Develop and implement effective intervention strategies to reduce diabetes risk in patients with

prediabetes.

Ground Transportation: Taxi service to the Texas Medical Center (TMC) hotels from Hobby Airport averages $35 one way. Taxi service from Bush Intercontinental Airport averages $50 one way. Uber is also an option. The drive to the TMC is approximately 30 minutes from Hobby Airport and approximately 45 minutes from Bush Intercontinental Airport.
