NON-ADMITTED PATIENT DATA COLLECTION: CLASSIFICATION AND CODE STANDARDS FOR
REPORTING SERVICES PROVIDED FROM 1 JULY 2017 IN AN EDWARD EXTRACT FORMAT
PURPOSE
The purpose of this Information Bulletin is to inform NSW Health service providers and source system administrators of changes to the classification and code set standard for reporting non-admitted patient services provided from 1 July 2017 in an EDWARD extract format.
KEY INFORMATION
As part of the transition from WebNAP to EDWARD extract formats LHD/SHNS will need to align their source system classifications and code set builds to the latest EDWARD classification and code set standards and ensure they have all of the classification categories available to source system end users.
Changes have been made to the initial EDWARD classification and code standards and many source system builds have aligned to WebNAP rather than EDWARD classification and code standards. Therefore, services reporting in an EDWARD extract format will need to undertake a full review of classification and code standards in order to identify compliance gaps and then take appropriate action to resolve them. It is expected that this review and realignment should coincide with the cessation of WebNAP extract format reporting and the transition to reporting in an EDWARD extract format.
The changes are of the following type:
1. Some new categories become effective
2. Some existing categories expire
3. Some continuing/common categories have descriptive label changes
4. Some continuing/common categories have different code values
5. Some data elements in WebNAP have split to multiple data elements in EDWARD.
The following fields having classification and code standard changes from 1 July 2017:
1. Individual Provider Discipline / Specialty (~ WebNAP “Provider Type”)

3. Primary Service Setting Type Code (~ WebNAP “Setting Type”)

The following fields have different classification and/or code standards for reporting via an EDWARD extract format compared to reporting via a WebNAP extract format:

EDWARD requires the following technical fields to enable the processing of data:

IMPLEMENTATION
The classification changes must be implemented for non-admitted patient activity reporting of services provided on or after 1 July 2017 where they are reported via an EDWARD extract format.
These changes will require LHD/SHNs to:

Modify local source system classifications / pick lists as appropriate
Map the local source system categories to the appropriate EDWARD alias code values.
Where a source system is shared between multiple LHD/SHNs, are compliant with a State Based Build, and / or are subject to application support services provided by eHealth NSW, it is the responsibility of each LHD/SHN to ensure the technical implementation of the modified reporting requirements are raised through the appropriate application support mechanisms. This includes:
The LHD/SHN Application Advisory Group (AAG) representative ensuring that the change requirements are on the AAG meeting agenda, discussed at the AAG meetings and are approved within a timeframe that will enable the implementation due date to be met.
Directing and authorising eHealth NSW to make the application build change by raising the request for change on the State-wide Service Desk and tracking the change through to its delivery.
CLARIFICATION ADVICE
The NSW Ministry of Health will provide clarification advice regarding the changed reporting requirements outlined in the attachments. 1 Overview
This attachment outlines new and modified EDWARD reporting requirements for reporting non-admitted patient services provided on or after 1 July 2017 in the EDWARD extract formats.
As outlined in the ‘Non-Admitted Patient Data Collection Transition from WebNAP to EDWARD Reporting’ Guideline (GL2015_012) non-admitted patient services are expected to be reported in an EDWARD extract format from 1 July 2017.
The codes presented in this information bulletin refer to EDWARD code standards.
The codes for services reported in the WebNAP extract format are available at:

2 Minimum mandatory fields
The list of data elements in the sections below describe the minimum mandatory fields that must be reported to EDWARD. The fields indicated with a # are new mandatory fields for reporting activity from 1 July 2017 via an EDWARD extract format.

This is to be indicated via the Consultation Liaison Indicator field (as “Y”), and the Financial Class Code of “NAN.12” (Non-admitted: Service to Emergency Department Patient) or “NAN.09” (Non-admitted: Service to admitted patient) must be reported. Service Type codes 258 and 259 should not be expired until Financial Class codes NAN.09 and NAN.12 have been implemented.
3.3 Primary Service Setting Type Code

Note 1: The term “Telehealth” has been removed from code “C” and “P” to align NSW Health to the national “Service Delivery Mode” classification standard’s descriptive label for this code (Videoconference). Note 2: The Independent Hospital Pricing Authority definition of the “Telehealth – Patient Location” code in the national Tier 2 Non-Admitted Services Classification is defined as a clinical / therapeutic service delivered using “information communication technology”. From 1 July 2017, NSW Health has interpreted “information communication technology” to mean services provided by telephone or videoconferencing. Changes to the derivation of the “Telehealth – Patient Location” Tier 2 code in the Non-admitted Patient Data Mart will be made to include “Telephone – Patient Location” services from 1 July 2017. Note 3: Despite the change of description of code “Telephone” to “Telephone –Consultant End” there is no change to the code definition. This code has only been used for reporting the consultant end of a telephone non-admitted patient service to date. The patient end of a telephone non-admitted patient was not counted or reportable under any circumstances prior to 1 July 2017. Videoconference is defined at HIRD data domain: http://hird.health.nsw.gov.au/hird/view_domain_description.cfm?ItemID=9437 Note 4: The new code “Telephone – Patient End” is only for use when the patient receives a clinical / therapeutic service delivered via the telephone and where the client / patient is located on a NSW Health Service campus in a setting that is not their usual residential location, and in the presence of a clinician at the patient end who will make an entry in the patient’s health record at the patient end. Where a telephone call is made to the patient / client in their home or any setting other than a NSW Health Service campus only the consultant end service may be reported, and the Service Contact Mode will be “Telephone – Consultant End”. Note 5: The new code for “No Client Contact – Other” has been introduced because some NSW Health Services record services that have no client contact for activities other than case conferences and case planning and review to support costing and other reporting requirements. As activities where the client / patient does not participate do not qualify as a national service event, they therefore do not qualify for a Commonwealth NWAU. These services must be properly identified as having no client contact so that they can be excluded from the National Service Event and NWAU allocation process. Reporting activity under this code may be mandatory for selected clinical streams (e.g. certain mental health service activities). This code may also be used for reporting planned non-admitted patient services where the client / patient has failed to attend and

a significant amount of clinician time (indirect time) has been used. This is particularly relevant for services planned to be delivered in the patient’s home where a clinician spends considerable time travelling to the patient’s home and then finds the client / patient is not at home and the planned service cannot be delivered. This may result in a clinical note being made to this effect. Such data may be useful for costing. Clarifying reporting requirements
1. Where a patient is provided a non-admitted patient service via telephone or video conferencing to his / her home and is not supported by a NSW Health staff member at his / her home during the service, only the consultant end of the non-admitted patient service is to be reported. The Service Contact Mode will be either “Telephone – Consultant End” or “Videoconference – Consultant End”.
A non-admitted patient service must not be reported for the patient end – this includes situations where the patient’s home is a NSW Health operated residential care setting.
2. Where a patient is provided a non-admitted patient service via telephone or video conferencing to his /her home and is supported by a NSW Health provider at his/her home during the service, that clinician is to report a service with a modality of “In Person”. The remote clinician is to report a non-admitted patient service with a modality of “Telephone – Consultant End” or “Videoconference – Consultant End”.
If both the clinicians provide a service that contains clinical / therapeutic content, and both make a note in the patient’s health record, both non-admitted patient services may attract NWAU.
3. Where a patient is provided a non-admitted patient service via telephone or video conferencing and the client / patient receives the service while on a NSW Health Service campus, the consultant end of the non-admitted patient service is to be recorded and the modality will be either “Telephone – Consultant End” or “Videoconference – Consultant End” and the patient end of the non-admitted patient service must be reported as either:
“Telephone – Patient End” or “Videoconference – Patient End” if the client / patient is not supported by a clinician at the patient end; or
‘In Person’ where the patient is supported by a clinician at the patient end.

The Health Service Campus Location Identifier is a new data element that enables analysis of types and number of services provided at each NSW Health campus location. The data will assist with health service planning and improve the accuracy of reporting on the health services available to local communities.
This data element has become necessary because the registered address of the base location of the service unit and health organisation is no longer a reliable indicator of where a non-admitted patient service was provided with the increase use of outreach services and mobile teams.
Health Service Campus Location Identifiers are issued by HERO. Local Health District HERO administrators are responsible for registering each NSW Health Service Campus Location and maintaining the characteristics of them over time.

The CHIRP program will change the CHIME application and the EDWARD extract to enable reporting of the Financial Class categories outlined in this information bulletin.
For iPM it is a local source system administrator responsibility to ensure the “ADCAT” domain values align to the Financial Class categories outlined in this information bulletin and are aliased with both the EDWARD Financial Class code and the Billing Category code. 6 Reporting arrangements for specific clinical streams
6.1 Services that must report the core minimum data set to EDWARD
All non-admitted patient services must report the core minimum data set to EDWARD except those provided by the following:

8 New technical data elements
The submission of data to EDWARD requires additional ‘technical’ data elements to enable the processing of the data.
The new technical fields include primary key fields which, as a set, uniquely identify each non-admitted patient service provided by NSW Health Services.
The other new technical fields enable EDWARD to identify whether a record that it receives needs to be aged and replaced, aged and not replaced, or inserted as a new service not previously reported.
Typically technical data elements are not ones that the end user is asked to complete, but are set in the back end of the source system or on creation of a data extract.
8.1 Control and audit fields
8.1.1 Source Create Date/Time
The Source Create Date/Time is the date and time that the record reported in the file / table was first created in the source system.
Note: Where data for one record in EDWARD is sourced from multiple records / tables in the source systems, the earliest record create date/time should be reported.
8.1.2 Source Modified Date/Time
The Source Modified Date/Time is the last date and time any data element in the file / table was modified in the source system.
Note: Where data for one record in EDWARD is sourced from multiple records / tables in the source systems, the latest record modified date/time should be reported.
8.1.3 Record Source System Code
The Record Source System Code indicates the instance / database of a source system that supplies the data to EDWARD, i.e. the source system in which the non-admitted patient service was recorded, extracted and reported to EDWARD. The Record Source System Code must include an alpha suffix where there are multiple extract packages for the same source system build.

8.1.4 Container Sequence Number
The Container Sequence Number is a unique sequential identifier allocated to the set of files in a single submission of data to EDWARD.
The container sequence number must be unique for each data stream for each record source system code. It must be generated by the source system and never re-used, unless a data container is being resupplied as a result of a rejection.
Containers are processed in strict sequential order. If a container does not have the next expected Container Sequence Number it will not be processed until the matter is resolved with the EDWARD Application Support Team.
8.1.5 Action Type
The Action Type indicates the action to be taken for a record reported to EDWARD.

U Update A record with the same primary key has previously been reported to EDWARD, however a different value is being reported for one or more of the non-primary key data elements. Note: A record may be modified as part of the process of correcting errors, or completing additional fields as information becomes available.

The Service Event Type Code is an indication of the type of service event, e.g. non-admitted, admitted or emergency etc. It is a primary key field for the non-admitted patient service record and is a technical field that must be hard coded in the EDWARD extract.

The Service Encounter Record Identifier is a primary key field for the non-admitted patient service record intended to indicate a phase of care or treatment. The same Service Encounter Record Identifier may be reported for many non-admitted patient services provided on different days over a course of weeks or months to the same patient by the same non-admitted patient service unit.
8.2.4 Service Event Record Identifier
The Service Event Record Identifier is a source system assigned unique identifier for a record representing a client / patient’s continuous contact with a health organisation. One encounter may have one or many service events, each of a different or same service event type. For non-admitted patient service contacts the service event record identifier is intended to indicate one patient’s contact with one service unit on one day.
It may represent the patient seeing:
1. One individual service provider only, or
2. Multiple individual service providers at the same time in the same service contact or appointment, or
3. Multiple individual service providers in succession within the same service contact or appointment where the individual service providers are associated with the same service unit.
This item is a primary key field for the non-admitted patient service contact record.
Note: Where clients / patients receive a service in a group session a unique Service Event Record Identifier must be reported for each client for each group session.

9 Data Submission Methodology
9.1 Direct submissions to EDWARD
Direct, automated nightly submission to EDWARD is the strategic objective and preferred method for reporting non-admitted patient activity.
Automated submission requires the source system to create the container header file and transfer the data securely to the EDWARD drop zone.
9.2 mLoad Data Submission Portal
mLoad converts WebNAP Extract files into the EDWARD Extract file formats. It also creates the container header files, tracks the container sequence numbers and transfers the files to the EDWARD drop zone for processing. The due date for source systems to submit data in an EDWARD extract format is 30 June 2017; however the functionality to load non-admitted activity in the WebNAP extract format will be supported during 2017/18. mLoad also accepts EDWARD extract non-admitted flat file extract formats without a container header. It will apply rudimentary validation of the data, create the container header and transfer the file to the EDWARD drop zone for processing.
10 Forward Plan for 1 July 2018
10.1 Service Activity and Interventions
There is a strong demand for data pertaining to the actual services undertaken and the interventions patients receive. There are also recommendations to simplify classifications for data elements that have mixed concepts.
If supported through the NSW Health Data Governance Work Plan the Ministry of Health will, during 2017/18:
review existing service activity and intervention classifications in NSW Health source systems and in existing data sets for specialist clinical streams
undertake consultation with key stakeholders
develop a classification for inclusion in the core minimum data set.
The following mixed concept data elements will be considered during this review and enable the following outcomes:
Removal of service activity and intervention categories (i.e. case conference and case planning and review) from the “Service Contact Mode Code”

1 Overview
1.1 Purpose of this document
This document provides the NSW Health State classification and code standards applicable to services provided from 1 July 2017 for data elements in scope of the Non-admitted Patient Data Collection Core Minimum Data Set.
The NSW Health State classification and code standards applicable to services provided and in scope of a Non-admitted Patient Data Collection Data Set Extension for specialist service units are published in separate Information Bulletins.

4.1 Overview
For clients who are registered in iPM or the Cerner HNA Millennium eMR, and whose characteristics are maintained with effective date/time stamps in those master client registration systems, the Client Identifier Type Code and Client Identifier Issuing Authority Code must be reported. EDWARD will then use these two codes with the service event date/time to look up the client details from the client characteristics reported to it via the iPM or Cerner HNA Millennium eMR systems.
