We are proud to offer ourSDS Membership that covers
100% of ALL preventative careincluding routine cleanings, exams
and x-rays.
Members also receive 20% off of ALL treatment
completed within 12 months of enrollment, including but not
limited to fillings, crowns, implants, even bleaching!

Co-payments for services provided are due in full at the time of service. We do not
send out statements or finance.
Cash or check only. A 5% fee will be added to services paid by credit card.
Membership is valid for twelve months from initial enrollment. Benefits expire at
the end of twelve months. Benefits are not transferable.
Membership cannot be combined with any other discount or insurance plan. If you
have traditional dental insurance, you are not a candidate for SDS Membership.
It is the responsibility of members to maximize benefits and schedule the
appropriate appointments within twelve months.
