Machine: Nissan Fork Lift

I. Shut Down Procedures:(a). Notify all affected employees that a lockout or tagout system is going to be utilized and the reason for its

application. The authorized employee shall know the type and magnitude of energy that the machine or equipment utilizes and shall understand the associated hazards.

Warning: This machine has a manufacture's established written Shut Down procedure, which should be followed to avoid damage to the equipment or possible injury prior to commencinglockout procedures.

Warning: Position the powered industrial truck in a location that does not present a hazard. Lower the forks to the full down position or install safety braces to support the mast.

(b). Electrical:Rotate the "IGNITION" switch to "OFF" to de-energize the machine & remove the key.

II. Isolation Procedures:(a). Electrical: E1 (Positive) Located under the drivers seat.

Disconnect the battery cable to isolate the electrical POWER.E2 (Negative) Located under the drivers seat.

Disconnect the battery cable to isolate the electrical POWER.

(b). Gas: LPG1 Located on the rear of the machine.Rotate the main gas valve to the “CLOSED” position to block the LPG service.

(c). Hydraulics:Note: Hydraulic pressure automatically bleeds down when POWER is removed.

Note: Refer to the Material Safety Data Sheet (MSDS) for information regarding the physical and chemical hazards and personal protective equipment requirements for this machine.

(d). Temperature:Allow the lift trucks motor to cool to a safe temperature.

III. Lockout Procedures:(a). Electrical:

Lockout the (2) battery cables with cord plug canisters, interlocking hasps, padlocks, and tagout tags.

(b). Gas:Lockout the gas service valve using a gate valve lockout, interlocking hasp, padlock, and tagout tag.

IV. Verification Procedures:(a). Electrical:

“Try” the machine start controls (e.g. by switching the "ON/OFF" switch to the “ON” or “START”position and observing that the machine does not operate) after lockout/tagout to make sure the correctisolation device has been secured and that the device is in the open or safe position. Switch the"ON/OFF" or “START” switch to the “OFF” position.

(b). Gas:Verify isolation and dissipation of the gas service by observing the gas pressure gauge indicates "ZERO".

(c). Hydraulics:Warning: Maintenance personnel noted that residual hydraulic pressure may still exist, consult

operator’s/maintenance manual for correct procedure to relieve pressure.

(d). Temperature:Verify cool down of the motor by observing the temperature gauge or probe indicates a lowering of the temperature to safe levels.

V. Returning to Service:(a). Check the machine and the immediate area around the machine to ensure that nonessential items have

been removed and that the machine components are operationally intact.

(b). Check the work area to ensure that all employees have been safely positioned or removed from the workarea.

Warning: This machine has a manufacture's established written Restart procedure, which should be followed to avoid damage to the equipment or possible injury.

(c). Electrical:Remove the tagout tags, padlock, interlocking hasps, and cord plug canisters from the (2) batterycables and reconnect the cables to the battery.

(d). Gas:Remove the tagout tag, padlock, interlocking hasp and gate valve lockout from the gas service line and rotate the air service valve to the “OPEN” position.

(e). Blocks & Braces:Remove Safety Blocks or Braces if installed.

(f). Notify affected employees that the maintenance is completed and the vehicle is ready for use.
