Better healthcare for local peopleNHS North Durham Clinical Commissioning Group (CCG) is the new body responsible for planning and commissioning (buying) local NHS healthcare for people living in the North Durham area.

This booklet explains who we are and how we will work with people who live in North Durham and with local organisations to tackle some of the main health issues and improve services for our communities.

Who we areNorth Durham CCG took over responsibility for buying healthcare for its communities on 1 April 2013. All 31 GP practices in the area are members of the CCG and together they make decisions about new and existing health services. The CCG serves around 243,000 people living in Derwentside, Durham and Chester-le-Street.

As doctors and health professionals we see patients and providers of health services every day. We now have a great opportunity to use the knowledge gained from these relationships to make changes that will improve the health and experiences of local people. We work closely with hospitals and other healthcare providers, Durham County Council, the Health and Wellbeing Board (a joint group that plans how health and social care services will work together better) and the newly established Healthwatch County Durham. We are also committed to working with our patients, those who live in the area and with voluntary organisations to ensure that as many people as possible are involved in decisions made about local health services.

Health challengesPeople who live in the North Durham area have significant health challenges and problems. They are also more likely to die sooner than those living in other parts of the country. The main causes of early death include high levels of cancer and diseases of the heart or blood vessels.

With an ageing population, we will also experience greater demand for hospital services and an increase in illnesses related to older people such as stroke, long-term conditions and dementia. The large student population in Durham City results in a demand for sexual health, alcohol and harm reduction services.

Other key challenges facing North Durham include:

• Poorlifestyleissuessuchassmoking,alcohol,obesity

• Economicinequalityrelatedtounemploymentandlowincomes

• Peoplewithdisabilitieshaveworsehealththanthosewithout

• Children’shealthandlifestylesarepoorerthanelsewhereinthecountry

What are we trying to achieve? Our Vision and Aims

North Durham Clinical Commissioning Group’s vision is:

‘Better Health for the People of North Durham’ where:

health services meet the needs

of patients

the health of our community is

improved

the gap in health inequalities is

reduced

we obtain value for money

and efficiency from available

resources

To deliver our vision, we will focus on four key aims:

Our aims have been developed from the ground up with significant contributions from GPs, member practices and engagement with stakeholders and patients. We have also learned much from our pathfinder projects that have crossed a range of care pathways. Through feedback and engagement from constituent member practices we now have a greater understanding of the key issues and challenges for our population.

Improving the health status of the population.

Addressing the needs of the changing age profile of the population.

Commissioning clinically effective,

better quality services closer to

home.

To make best use of public funds to ensure healthcare meets the needs of patients and is safe and effective.
