• From within primary care we will build continuous relationships with citizens andpartners to allow a greater level of local coordination of resources.

• We will make the patient experience as personal, supportive and appropriateas we would wish for ourselves.

• We will create the conditions for everyone to take their share of responsibilityfor health & well being.

• We will create a community team that reflects our ambitions; to extend theexisting team to include other community, hospital, partner resources.

• We will create a richness of trust and understanding between citizens and theNHS through regular, meaningful conversation.

• NUKA model seen as an influence, not the answer in totality

• strong cross system/partnership/government support

• Three key components emerging - Deep Community Engagement, GPs as co-ordinators of care, Redesign of general practice
