You have been referred to our service for help and support.

To find out how we can help, you will meet with one of our staff.You will discuss what you find difficult at the moment, as well asthe things that might help.

Care planning

Together, we will make a care plan describing the type of help,support and treatment you will receive. The care plan will say whowill work with you and what they and you will do to help.

This leaflet gives you some more information about how youshould expect us to work with you.

l Treat you with courtesy, kindness and respect, as staff wouldexpect their family and friends to be treated

l Listen to you and take notice of your preferences

l Offer choices about your care and treatment

l Be reliable and do what we agreed to do

l Offer care and treatment in buildings that are safe and clean

l Ask if you would like other important people in your life to beinvolved

l Respect your confidentiality and share information withothers only with your permission, or when we have a duty todo so

l Help you to be safe, and treated fairly in all areas of your life

l Let you know who to contact if things get difficult.
