1.0 Introduction

This is the revised version of the NHIS Medicines List (ML), which takes effect from 1st April, 2020. The

original list of medicines in this schedule was developed using a process that involved evaluation of

evidence for the management of health problems commonly seen at health facilities in Ghana. A

Specialists Group comprising medical doctors, pharmacists and a midwife reviewed the evidence for

the management of the health problems and selected the appropriate medicines. A Core Group of

clinicians assisted the specialists in the selection of medicines and did the final review of the selected

medicines. The current reviewed Medicines List contains a total of Five Hundred and Thirty Two (532)

formulations.

2.0 Methodology

A national survey of medicine prices was undertaken using the methodology of the World Health

Organisation and Health Action International (HAI). Pricing data was collected from manufacturers,

wholesale distributors, private pharmacies, government, mission and private health facilities located

in all the ten (10) regions in the country.

The purpose of the survey was to capture the prices of the selected medicines as obtained in both the

public and private sectors. For each formulation, prices of the generic versions were used for the

analysis. However, for medicines still under patent protection, innovator (brand) price was used. The

output of the Framework Contract Agreement of the Ministry Of Health was also factored into the

pricing of the formulations within the contract arrangement. The prices provided are those agreed

upon by the National Health Insurance Authority (NHIA) and stakeholders as the reimbursable price

for medicines supplied under the scheme.

3.0 Stakeholders’ Meetings

NHIA has always emphasized the importance of involving stakeholders in the review of the Medicines

List. Stakeholder meetings were held where the list of medicines was presented for comments and

inputs. Stakeholders were given the opportunity to compare NHIA proposed prices with prices

captured from their own pricing surveys.

4.0 Technical Group Meeting

A technical group of experts in the pharmaceutical sector was constituted to finalise prices and

proposals presented by stakeholders. The members were drawn from the Office of the Director of

Pharmaceutical Services of the Ministry of Health, Ghana National Drugs Programme, Food and Drugs

Authority, Pharmaceutical Society of Ghana, Chamber of Pharmacy, Government and Hospital

Pharmacists Association, Community Practice Pharmacist’s Association and the National Health

Insurance Authority.

5.0 Exclusions

The development of the Medicines List concentrated on diseases covered by the benefits package

under the National Health Insurance Scheme. Medicines used for healthcare programmes are

considered as exemptions and are thus excluded from the List. These include childhood

immunizations, tuberculosis and mental health care. Some medicines used in mental health, however,

may be used in general practice as well. Such situations have been recognized and medicines have

been listed for such conditions. The list also does not include medical devices and supplies, and all

anesthetic agents because their prices are included in the tariffs for services for which they are used.

6.0 How to use this document

The medicines on this list are described in their Generic or International Non-Proprietary Names and

include the dosage form and strength. For example: Paracetamol Tablet, 500mg. The proprietary

names of very few medicines are used as the ‘generic’ name as a result of long standing common

usage, for example, Aspirin Tablet instead of Acetyl Salicylic Acid Tablet. Medicines should be

prescribed by the names on this list. Personal abbreviations of names of medicines must be avoided,

for example, P’mol for paracetamol would not be accepted on the claims form for reimbursement.

The Unit of Pricing column provides information on the basis of the price quoted in the Price column.

Examples of the units of pricing are tablet, capsule, suppository, pessary, vial, ampoule, weight (for

creams, ointments, granules etc), and volume (for liquids). It is important that prescribers indicate the

quantity of medicines to be supplied on the prescription. This is a normal requirement for prescribing

and will be very helpful in claims processing for reimbursement.

A prescription for two (2) Paracetamol Tablets, 500mg to be taken three times a day for 3 days must

indicate the quantity to be supplied as 18 (6 tablets a day for 3 days). Thus, the reimbursed amount

will be the price of one tablet of Paracetamol (0.03 Ghana Cedis) multiplied by 18, which is 18 x 0.03

those medicines used as part of procedures, the tariffs developed for the procedures include the cost

of the relevant medicines.

In calculating the cost of medicines supplied, particular attention should be paid to the unit of pricing.

Where a unit of pricing is stated as a course by volume, the price indicated is for the whole course,

not per tablet or ml. For example for AMOARTTA1, the GH¢ 3.45 price quoted below is for the one full

Course of six tablets, not per tablet.

6.1 Local codes (Nine Alpha-numeric characters)

The code generated for each medicine item was limited to a fixed length of NINE alphanumeric

characters. A system of classification based on the first six characters of the generic name of the

medicine (with variations), formulation (which is the next two characters) and strength being the last

character, was designed to provide each medicine on the NHIS Medicines List a unique identity. Where

there is more than one strength of the same formulation, the medicine’s code was arranged

numerically in ascending order of strength. In designing the codes, consideration was also given to the

practicality of filling forms within diverse health institutions (public and private); with different staff

strengths and capabilities.

For example, the code for Albendazole Tablet, 200 mg would be ALBENDTA1, with ALBEND from its

generic name, TA- indicating that it is tablet and 1 as the lower strength; as opposed to 2 for the 400

mg which is a higher strength.

Exceptions to the ‘First 6 character’ rule apply when the generic name of the medicine is made up of

two or more words. Thus where the generic name has two words, the first three characters of each

word are joined to form the first six characters of the code.

For example, the code for Magnesium TrisilicateTablet, 500 mg is MAGTRITA1.

On the other hand, where the generic name has three words the first two characters of each word are

joined for the first six letters. An example is Magnesium Trisilicate + Aluminium Hydroxide Mixture

with code MATRALMI1.

The two character designation for the dosage forms are given in Table 1.

A few of the medicines with identical codes have had the principle of the first six characters modified

in order to avoid confusion. Identical codes occur when:

• medicines appear under more than one therapeutic class

• the first six characters of a medicine name and the dosage form are identical

Below are some examples:

1. Chlorpromazine and Chlorphenamine Tablets changed to CHLPRO AND CHLPHE respectively;

2. Diethylcarbamazine and Diethylstilbesterol Tablets changed to DIECAR AND DIESTI

respectively;

3. Streptomycin and Streptokinase Injections changed to STREPT (maintained) and STRKIN

respectively and

4. TETRAC and TETCAI for Tetracycline and Tetracaine Eye drops respectively

7.0 Level of Prescribing

The level of prescribing is the lowest level of health care delivery at which a specific medicine can be

prescribed. These levels have been adopted from the latest version of the Essential Medicines List

(EML) 2017 of the Ministry of Health. The overall goals of enforcing the levels for medicines on the

NHIS Medicines List are to improve quality of care, promote rational use of medicines and contain

escalating cost of medicines to the scheme. The categorization is as follows:

Level A – Community- based Health Planning and Services (CHPS) (the lowest level)

Level M – Midwifery

Level B1 – Health Centre without Doctor

Level B2 – Health Centre with Doctor

Level C – District Hospital

Level D – Regional/Teaching Hospital

Level SM – Specialist Medicines

As indicated in the EML 2017, Specialist Medicines are restricted for use by qualified specialists who

may request for them.

With these categories a medicine assigned a level C can only be prescribed by prescribers at C level

facilities and above, i.e. at district, regional, and teaching hospitals. For example:

The level of prescribing for Midazolam Injection, 5mg/5ml is C which indicates that it can be prescribed

only at district, regional, and teaching hospitals. This means that prescribers’ at community, midwifery

and health centres cannot prescribe Midazolam Injection, 5mg/5ml.

8.0 Prescribing Guidelines

All NHIS prescriptions should be written on approved prescription forms and in approved formats.

Only medicines on the NHIS Medicines List shall be permitted for reimbursement and only

credentialed providers shall prescribe for dispensing under the NHIS. The following, working in

credentialed health facilities, are currently authorized to prescribe medicines: Medical practitioners,

Dentists, Midwives, Medical Assistants and Nurses in specialized fields.

All prescriptions should fulfill the following requirements:

• conformity with Ghanaian laws including the Health Professions Regulatory Bodies Act, 2013

(Act 857)

• written legibly in ink or otherwise so as to be indelible

• written by the prescriber and not left for another person to complete

• should be dated

• the full name and address of the patient should be stated

• the age and weight (in the case of children) of the patient being treated should be specified

• the diagnosis being treated should be stated

The prescription should also contain the following:

Dosage form, generic name of medication, strength, dose and dosage schedule

9.0 Dispensing Guidelines

The dispensing of all prescriptions under the NHIS must be carried out either by a pharmacist or

under the direct supervision of a pharmacist who can intervene, if necessary. The dispenser should

ensure the following:

that the prescription is legally valid, genuine and has not been altered after issuing

that the items on the prescription can be prescribed under the NHIS i.e. they are on the NHIS

Each medicine on the prescription contain the dosage form, generic name, strength, dose,

dosage schedule and quantity of medication to be supplied

the prescription is assessed for validity, safety and clinical appropriateness.

the dosage form and route of administration are appropriate according to the patient’s

gender, age and clinical condition.

10.0 Labelling

Each dispensed medication should be appropriately packaged and adequately labelled with the

following minimum information:

• Name of the patient and the generic name of the medicine

• Strength of the active ingredient and special instructions

• Quantity of dispensed product

• Complete dose regimen in written and/or graphic form

• Duration of use

• Name and address of the dispensing facility and dispenser

• Date of dispensing
