NOTICE TO RESIDENTS & FAMILY MEMBERS RE: OUTSIDE MEDICAL APPOINTMENTS FOR RESIDENTS
From time to time the Resident may be asked to attend a medical appointment outside of the facility. In most cases the
Resident will need a family member to accompany them. If a family member is not
available then an agency escort will be booked. The full cost of the escort will be charged to the
Resident. Transportation (Community Support Services van, taxi, family vehicle) can be
scheduled depending on the medical status of the Resident. All costs associated with this
transportation will be the responsibility of the Resident.
