How To Participate In The Healthy Lifestyles Wellness Program

HOW TO QUALIFY FOR INCENTIVES:Healthy Lifestyle Wellness Program Rebate:• $50: 0 – 449 points• $200: 700 – 899 points• $175: 450 – 699 points• $250: 900 + points

Rebate will appear on the employee’s paycheck 4-6 weeks after completion.

Points are accumulated throughout the year through programs, classes, events, and preventative exams. Participants (employees + adult designees) are eligible to earn points towards an annual cash rebate up to $250 and an HSA incentive up to $275.
