• CMS Web Interface Submission Period (January 2, 2020 - March 31, 2020)

- Submission Period

• The submission period for the CMS Web Interface aligns with other submission types, and closes promptly at 8:00pm Eastern Daylight Time (EDT) on March 31, 2020.

• Now, the CMS Web Interface submission period is longer and spans a three-month timeframe.

• The CMS Web Interface will automatically accept your submission at the end of the submission period.

• The CMS Web Interface is Accessible via the “Sign In” link on the Quality Payment Program website at https://qpp.cms.gov.

Reminders

• Other CMS Approved Reason skip requests must be submitted through the CMS Web Interface.

- A CMS Approved Reason is a way to skip a patient attributed to a measure during Denominator Confirmation.

• Reserved for circumstances that are unique, unusual, and not covered by any of the denominator exclusions or denominator exceptions identified in the measure specifications.

• Patients for whom a CMS Approved Reason is selected will be “skipped” and another patient must be reported in their place for the measure, if available.

• When a skip request is denied, the patient will remain incomplete and users must complete reporting on the patient.

• Submit requests for a “2019 CMS Approved Reason” as soon as possible during the submission period to allow for CMS review.

• Please review the 2019 measure specifications thoroughly before submitting a CMS Approved Reason request.

CARE-2 The specification states, “a clinician with appropriate skills and experience may perform the screening." Would a nurse outside of the office be considered a "clinician"?

Yes. A nurse with the appropriate skills and experience may perform the screening and the setting is not restricted to an office setting

HTN-2 Can a HTN diagnosis be found from an Urgent Care or emergency room record or does it need to be from a PCP office visit?

Yes. Medical record documentation from urgent care or emergency room may be used to confirm the diagnosis.

PREV-7 Can a clinician document patient refusal of the influenza vaccination by using the "Z28.21 Influenza Vaccination Declined" diagnosis code? It is not included in the CMS Web interface PREV Coding Document PREV 7 Denominator Exception Codes.

Yes. Although the particular code in question is not contained in the coding document, a patient refusal documented in the medical record would be appropriate documentation. Documentation should be during the measurement period and be specific to the flu season being reported.

PREV-10 Is the numerator met if a patient is identified as a smoker during the most recent screening, was not counseled, but had counseling previously within the reporting period?

Yes, as long as there is documentation that cessation intervention occurred within the 24-month look back period.
