Mit diesem Buch wird eine wissenschaftliche und umfassende Darstellung des heutigen Standes der Neuro-rehabilitation vorgelegt. Das Thema ist zum einen fr diejenigen Personen von grter Bedeutung, die unter den Folgen von Gehirnschden und neurologischen Erkrankungen leiden. Es ist zum anderen fr ihre Familien und fr die Gesellschaft wichtig, in der sie leben. Von dem Buch werden vor allem die verschiedenen Spezialisten, die in der Neurorehabilitation arbeiten, profitieren. Sie werden Einsichten in die stndig wachsenden Erkenntnisse der Neurowissenschaften und Neurorehabilitation gewinnen. Damit wendet sich das Buch gleichermaen an rztinnen und rzte, an Therapeutinnen und Therapeuten und Fachkrfte der Pflege.

Es geht in dem Buch nicht nur darum, Einblicke in Hirnfunktionen und ihre Strungen zu erhalten. Viel-mehr geht es auch darum, die Erkenntnisse in die klinische Praxis zu bertragen, damit sie denjenigen Personen zugute kommen, die unter den Folgen einer Hirnverletzung oder einer neurologischen Erkrankung leiden. So erfahren die Leserinnen und Leser, wie therapeutische Ziele definiert werden und welche Therapiemethoden effektiv sind. Dabei sollten die Ergebnisse der Neurorehabilitation sowohl funktionelle Verbesserungen als auch eine Frderung der Teilhabe am Leben in der Gesellschaft umfassen.

Der rote Faden, der sich durch das Buch zieht, ist die Tradition, in der die deutsche Neurorehabilitation von ihren Anfngen bis heute steht. So werden die Gedanken des Neurologen Kurt Goldstein und dessen psycho-logischen Kollegen Adhmar Gelb und sowie von Martin Scheerer aufgegriffen. Es wird auf die Theorien von Kurt Goldstein aufgebaut, der mit seinen Vorstellungen schon whrend und nach dem 1. Weltkrieg ein nach-haltiges Fundament fr die Rehabilitation hirnverletzter Personen geschaffen hat. Seine Denkweise wird von Anne Harrington (2002) so charakterisiert:

Er war nicht lnger bereit zu akzeptieren, dass die einzige Mglichkeit, einen Patienten mit einer Hirnver-letzung zu verstehen, darin bestand, einige seiner Verhaltensweisen zu einer kurzen Liste vorsortierter Symp-tome zusammenzufgen und dabei alles andere auer Acht zu lassen. . . Schon als er whrend des Krieges mit Gelb zusammenarbeitete, hatte er bemerkt, dass man nach Ganzheit suchende Organismen mit Hilfe einer medizinischen Wissenschaft und Biologie, die in atomistischen Metaphern und Methodologien gefangen war, weder verstehen noch ihnen helfen konnte.

Der Einfluss von Goldstein auf die Neurorehabilitation der 90er Jahre des vergangnen Jahrhunderts zeigt sich in einer Einladung an Yehuda Ben Yishay (ein Student von Goldstein whrend seiner Zeit an der New Yorker Universitt), einen Kurs fr Neuropsychologen in Schaufling zu leiten. Sein Konzept einer ganzheitlichen neuropsychologischen Rehabilitation wurde von Peter Frommelt und Wolfgang Khne in der Schauflinger Klinik umgesetzt. Sie gehrten auch zu den Grndungsmitgliedern der European Group for Holistic Neuropsycholo-gical Rehabilitation, zu der Zentren in Dnemark, Finnland, in den Niederlanden und das Oliver Zangwill Center in Cambridge gehren. In jhrlich stattfindenden Treffen werden Ideen und Methoden der holistischen neuropsychologischen Rehabilitation ausgetauscht und kritisch bewertet. Projekte und Forschungen werden in einer interaktiven Atmosphre prsentiert, von der alle Teilnehmerinnen und Teilnehmer profitieren.

Die Mehrheit der Autoren, die an diesem Buch mitgearbeitet haben, kommt aus Deutschland. Zwei Kapitel sind jedoch von Mitgliedern der European Group for Holistic Neuropsychological Rehabilitation die in Finnland und England arbeiten, geschrieben worden. So unterschiedlich die Autoren sind, freue ich mich dennoch feststellen zu knnen, dass sich die Gedanken einer ganzheitlichen Rehabilitation in vielen Kapiteln wiederfinden. Das Buch gibt damit einen umfassenden berblick, der von den historischen Wurzeln bis zur Dokumentation und Qualittssicherung reicht.

Ich bin berzeugt, dass das Ziel dieses Buches erreicht wird, die Rehabilitation von Menschen mit einer Hirnverletzung oder einer neurologischen Erkrankung zu strken.

Seit dem Erscheinen der 1. Auflage von NeuroRehabilitation sind ber 10 Jahre vergangen. Es schien uns daher an der Zeit fr eine Aktualisierung. Tatschlich ist in der 2. Auflage ein nahezu komplett neues Buch ent-standen, was die Entwicklungen im Fachgebiet der Neurorehabilitation widerspiegelt. Wir haben uns bemht, allen in Klinik und Praxis ttigen sowie in Ausbildung befindlichen medizinischen und therapeutischen Fach-krften ein vielseitiges Arbeitsbuch zur Verfgung zu stellen. Das Buch ist interdisziplinr ausgerichtet; daher finden sich keine auf Berufsgruppen bezogenen Kapitel.

Aus dem frheren, gelegentlich etwas chaotischen, insgesamt jedoch wohlwollenden Nebeneinander von Methoden hat sich eine konomisch konkurrierende Auseinandersetzung von therapeutischen Schulen ent-wickelt, in der es um den Nachweis von Effizienz und Wirtschaftlichkeit geht, aber natrlich auch um die Verteilung von Macht und Ressourcen. Man mag begren, dass manches nun rationaler betrachtet wird als frher, und dass hufiger Fragen nach Sinnhaftigkeit und Effizienz gestellt und auch beantwortet werden. Andererseits fehlt oft die Zeit, einen Schritt zurckzutreten und das eigene Tun zu reflektieren. Die Mglich-keiten des kollegialen Austausches sind seltener geworden. Der Einzug von Behandlungsroutinen erschwert es, auf die Individualitt der Patienten einzugehen, das Narrativ kommt zu kurz deshalb nimmt es in diesem Buch einen etwas breiteren Raum ein.

Wir hoffen mit diesem Buch einen Beitrag zu einer sachlichen Diskussion leisten zu knnen. Wir haben uns bemht, viele Stimmen zu Wort kommen zu lassen. Die evidenzbasierten Methoden z.B. der motorischen Rehabilitation werden ausfhrlich dargestellt. Gleichzeitig wird der Begriff evidenzbasierte Medizin vom Kopf wieder auf die Fe gestellt, indem dargelegt wird, dass die Erfahrung und Kompetenz des Therapeuten oder Arztes und seine Orientierung an den individuellen Patientengegebenheiten ebenso unentbehrlich sind wie die Auswahl der richtigen Methoden nach wissenschaftlichen Kriterien. In diesem Buch wird der Team-arbeit und der Zielsetzung in der Rehabilitation Raum gewidmet, vor allem aber der Persnlichkeit und Bio-graphie der Patienten und dem Narrativ in der Neurorehabilitation. Die Beitrge der Herausgeber sind in Anpassung an diese Schwerpunkte sehr persnlich gehalten. Die wichtigsten Strungsbilder der Neuro-rehabilitation werden von kompetenten Autoren auf dem aktuellen Stand diskutiert. Zum Abschluss werden Aspekte der Messbarkeit, Qualitt und Dokumentation ausfhrlich dargestellt.

Das Erscheinungsdatum dieses Buches hat sich vonseiten der Herausgeber mehrfach verzgert. Holger Grtzbach hat sehr wesentliche Vorarbeit geleistet. Ihm gilt dafr unser besonderer Dank. Er musste dann aus Zeitmangel die Arbeit an diesem Buch einstellen und den Stab an Hubert Lsslein bergeben. Wir mchten uns bei unseren Autoren fr ihre Langmut bedanken. Es war nicht einfach fr sie, ihre Beitrge wiederholt zu aktualisieren. Unser Wunsch an die Autoren, ein praxisorientiertes und gut lesbares Buch zu liefern, wurde uns erfllt. Frau Schreier hat fr den Springer-Verlag die stilistisch recht heterogenen Kapitel sehr schn in ein einheitliches Raster gebracht, was die Orientierung und die Lesbarkeit erleichtern. Die Autoren

Dr. med. Peter Frommelt ist Facharzt fr Neurologie und Psychiatrie, Psycho-therapie und Facharzt fr Physikalische und Rehabilitative Medizin. Seit 1988 leitet er die Abteilung Neurorehabilitation in der Asklepios-Klinik Schaufling, b. Deggendorf. Er ist Lehrbeauftragter an der Universitt Regensburg. Sein Haupt-interesse gilt den therapeutischen Prinzipien der Neurorehabilitation. Dazu ge-hren die Beschftigung mit Teamorganisation und Zielsetzungsprozessen und die Umsetzung der Internationalen Klassifikation der Funktionsfhigkeit, Behin-derung und Gesundheit (ICF) in die Praxis. Gemeinsam mit Wolfgang Khne, ltd. klinischer Neuropsychologe in Schaufling, hat er die European Group for Holis-tic Neuropsychological Rehabilitation gegrndet.

In den letzten Jahren hat sich Dr. Frommelt besonders mit dem subjektiven Erleben von Patienten auseinandergesetzt und sich fr einen narrativen Ansatz in Ergnzung zu einer biomedizinischen Herangehensweise eingesetzt. Sowohl in

diesem Thema als auch in vielen anderen verbindet ihn mit Holger Grtzbach, MA ein jahrelanger Gedanken-austausch und eine enge wissenschaftliche und praktische Zusammenarbeit. Ab 2011 wird Dr. Frommelt er seine Ttigkeit in Berlin in eigener Praxis fortsetzen.

Aus einer frheren kollegiale Zusammenarbeit von Dr. Frommelt mit Dr. Hubert Lsslein, ehemals Chefarzt der Klinik fr Neurorehabilitation im Bezirksklinikum Mainkofen b. Deggendorf, hat sich in den letzten Jahren eine enge persnlichen Freundschaft entwickelt. Dr. Lsslein hat sich seit Jahren mit Fragen des Managments und der Qualittssicherung in der Neurorehabiliation beschftigt. Nur Dank des Eintritts von Dr. Lsslein als Mitherausgeber konnte das Buchprojekt zu einem guten Abschluss gebracht werden. Er war pragmatisch und schnell in der Lektorierung. Trotz seiner Erkrankung an Darmkrebs hat er bis in die letzte Woche vor seinem Tod an dem Buch gearbeitet. Er hatte gehofft, das fertige Buch in der Hand halten zu knnen. Dieser Wunsch ging nicht in Erfllung. Er starb am 18. Juli 2010
