1. Get the S1 form from your home country health authority and/or bring the EHICard

EU STUDENTS

Students from European Union countries, the European Economic Area & Switzerland

CASE AIf you stay in Italy temporarily and you need to access a general practictionerthe emergency room or necessary health services at an hospital, it is sufficient to show your valid EU ID and the European Health Insurance Card (EHIC / TEAM) of the State of origin.

CASE BIf you live in Italy for over 3 months and you wish to access additional health services (e.g. be assigned with a general practitioner ad get discounted prices for medicines and therapies) you should better register with the National Health Service. If you are moving from European Union countries, the European Economic Area and from Switzerland you can temporarily transfer your health coverage to take advantage of Italian public healthcare as an Italian citizen, by obtaining S1 Form from your country of origin. If you don't have the S1 form with you, you are still eligible to subscribe for a Voluntary Registration by paying a fee. 5

TIP: Explore the available doctors

around you

You can go to this link https://www.ats-milano.it/portale/Cerca-medico-o-pediatraand see available general practictioners (look for MMG doctors) (green dot) in your area.

When doctors have reached their maximum patients capacity, the dot is red and you cannot sign up for that doctors until some patients leave.

You may also ask neighbours and friends to suggest the name of a general practictioner in the area

If you have to register with an S1 formfrom your home country health authority

2a. Find your nearest ASST branchAzienda Socio Sanitaria Territoriale (ASST) is the administrative entity that manages local health services) competent in the area where you reside. The areas are called "Municipi" – i.e. the administrative zones of Milan.

Find out in which "Municipio" you live: ask Google, your landlord, a neighbor, or any shop owner nearby... or look up to the map in the next slide:Find the location and contacts of the "Ufficio Estero”, the office handling the international procedures of your local ASST office.Alternatively, you may book an appointment with the “Ufficio Scelta e Revoca” of your ASST

3. Take an in-person appointment

The appointment for first enrollment must be done in person at Ufficio Estero / Scelta e Revoca- By phone- By e-mail- Through an online appointment booking tool (in some zone)

The office will need to know your exact address, tax code, nationality and type of request and other information useful to better serve your needs.

4. Enroll with S1 form

Go to Ufficio Estero / Scelta e Revoca counter with:- a valid ID- Tax code- one photocopy of both ID documents along with the S1 form and a self-certification of

residence / domicile.

The office will retain the first sheet of S1 and the photocopies, and enroll you in the SSN. They will also assist you to choose a local general practitioner from a list. They will issue you a form called "Allegato 1" ("Annex 1") which must be shown as a health card at the time to request any health service.

Enrollment expiration

Enrolment at the SSN is valid for one year from the date of application.

Should you stay longer you can follow a similar procedure to renew your coverage by using the online portal https://sr.asst-fbf-sacco.it .

Apart from the first enrollment that must be done in person, the greatest part of all the next operations can be done online.

You may renew every year as long as the period reflects the dates stated on the S1 form.

Please note: if you have a planned health treatment, e.g. for chronical disease, you need to ask your local authority in the country of origin for S2 form - Authorization to obtain planned treatment abroad.

Procedures for ALL Students who enroll voluntarily

Period of coverageThe voluntary registration validity period always starts on 1/1 and expires on 31/12 of the current solar year, no matter when you enroll and gives access to the general practitioner and health services only on the Italian territory.

The first application shall be done after January the 1st of the year in which you want to enroll.

Skip-the-queue tip for renewals: If you are already registered with SSN, renewals can be done in advance by repeating the same procedure and telling the officer that you are renewing the enrolment for the subsequent year.

2. Pay the Voluntary Registration

Contribution at any Post office

The voluntary registration coverage starts on 1/1 and expires on 31/12 of the current solar year, no matter when you enroll.

The subscription is never retroactive nor divisibleThe contribution must be paid in one single payment.

For students the contribution is €149.77 per solar year.

Students placed au pair: 219,49 €

Others, including students with dependent family members: 7,50% on the share of income of the previous year up to 20.658 €, plus 4% on the share of income exceeding 20.658 € and up to 51.646 € 24

enrollment in a course of study (issued by your university /institute),

Students from non-European countries already in possession of a residence permit for study:

Bring with you: passport, residence permit, tax code (both in original and one photocopy), certificate

of enrollment in a course of study, and receipt of payment of the contribution.

3b.Voluntary Registration with

Students from non-European countries holding the application receipt of their residence permit for study

will need to bring:

your passport, tax code, entry visa for study reasons, the receipt / certificate of having applied for a

residence permit (all in original, plus one photocopy), certificate of enrollment in a course of study

(issued by your university /institute), receipt of payment of the contribution

*The voluntary registration health card is issued valid for 3 months and it can be extended going again

to the office, until obtaining the residence permit for study. Once in possession of the actual residence

permit, you may go to the office and have it registered until the deadline, then, will be set to 31/12 of the

current year. 28

The voluntary registration with the SSN is valid in Italy only

and does not entitle you to:

a. Issue of the European Health Insurance Card (TEAM)

b. Issue legal certificates required by bilateral agreements in force

c. Issue of the certificate pursuant to art. 15 of Presidential

Decree 618/80

d. Issue of portable document S2 (high specialization care

abroad)

TIP: Double Check your Tax Domicile

Make sure that the address registered at Agenzia

delle Entrate on your tax code file is present and

matches with your actual address
