Goals, Objectives,and Activities

Time Task Plan: Project Goals

1. Establish a county-wide system that will identify all juvenile offenders in need of AOD treatment or other interventions designed to address substance use and delinquent behavior.

2. Improve coordination and delivery of drug, alcohol, and other services to juvenile offenders throughout Nassau County.

3. Strengthen compliance with court-ordered AOD treatment, and other services by NCJTC participants.

4. Reduce the criminal recidivism of NCJTC participants.

Project Goal

Establish a county-wide system that will identify all juvenile offenders in need of AOD treatment or other interventions designed to address substance use and delinquent behavior.

Objectives and Activities

1. Hire a Change Leader to implement Reclaiming Futures model.

Activities:

• Post Position, interview and hire Change Leader.

2. Contract with Service Providers.

Activities:

• Prepare and submit contracts to Department of Probation, alcohol and drug treatment providers.

3. Screen, assess, and place participants in treatment within one month of petition filing.

Activities:

• Eligible juveniles will be referred and screened for the NCJDC program.

4. Implement a universal screening tool designed to flag all drug involved juveniles.

Activities:

• Juveniles flagged with substance use problems will be referred for a full assessment to determine the nature and scope of their involvement.

Project Goal

Improve coordination and delivery of drug, alcohol, and other services to juvenile offenders throughout Nassau County.

Objectives and Activities1. Change Leader conducts outreach in the community.

Activities: Identify gaps in services, harness resources and coordinate delivery.

2. Convene stakeholders in a “change team” with representatives from the Judiciary, Department of Probation, Alcohol and Drug Superintendents of schools, Social Service agencies, Commissioner of Mental Health, Alcohol and Drug, and Developmental Disabilities, and youth and family members to address gaps in juvenile treatment services.Activities: Change team conducts an assessment of service gaps, and using information from a needs assessment and working with community agencies, determines which programs can best fill service gaps and how to build community support to create a complete network of service resources.

3. New juvenile services will be developed.Activities: Change team will produce a resource guide listing all relevant community-based providers and ancillary services.

4. Produce a Strategic Plan for managing a successful juvenile drug court.Activities: Using timelines, benchmarks, and clear assignments of responsibilities, change team drafts MOUs among stakeholders, articulates a commitment to a whole-system shift to improving the response to identifying and intervening with seriously alcohol and drug involved young people across all sectors of the juvenile justice system, including structuring management for change, cross-training orientation and training with partners, expanding active community partners, tracking data, process and outcome measures, and strategizing to engage in targeted communications with stakeholders and the public.

5. Complete Reclaiming Futures Learning Collaborative and core curriculum as team.Activities: Change leader will coordinate NCJTC participation in online learning

courses, phone conferences, and national meetings.

6. Change team meets at regularly scheduled intervals.Activities: Preview progress, identify barriers, and brainstorm solutions.

Project Goal

Strengthen compliance with court-ordered AOD treatment, and other services by Nassau County Juvenile Treatment Court participants.

Objectives and Activities

1. The NCJDC will hold regular post-dispositional judicial status hearings for all NCJTC participants.

Activities:

• NCJTC will schedule biweekly status hearings to verify compliance, discuss the participant’s progress, and administer a system of sanctions and rewards.

2. The NCJTC will administer a system of graduated sanctions and awards.

Activities:

• Judge will assign following discussions with team.

Project Goal

Reduce the criminal recidivism of Nassau County Juvenile Treatment Court participants.

Objectives and Activities

1. Reduce the percent of juveniles re-arrested for any new offense.

Activities: Evaluate the impact and usefulness of the NCJDC program by developing and implementing a data tracking system to demonstrate process and outcome measures.

2. Reduce the percent of juveniles re-arrested for any new drug-related offense.

Activities: Use feedback to inform program modifications

3. Record and report findings.

Activities: Produce final evaluative report.

Implementation Plan: Project Goals

1. Establish a county-wide system that will identify all juvenile offenders in need of AOD treatment or other interventions designed to address substance use and delinquent behavior.

2. Create a sustainable plan for supervision and coordinated service provision for all agencies that serve substance abusing juveniles.

3. Enhance system capacity to utilize validated screening and assessment tools

4. Improve the coordination and delivery of drug, alcohol, and supportive services to juvenile offenders throughout Nassau County.

5. Coordinate substance abuse treatment providers, youth development providers, schools and with the court to build wraparound services.

6. Engage Youth and families in planning, implementation, evaluation and modification of RF initiative in Nassau.

7. Create community and interagency support for the initiative.

8. Coordinate the RF initiative with other state and county initiatives to enhance, complement and align RF with other funded initiatives for leverage, creative partnerships and sustainability.

9. Evaluate the impact of the Reclaiming Futures Initiative.

Objectives and Activities

1. Form a Change Team

Activities:• Establish NCJTC team and RF Fellows as core leadership. • Participate in online learning courses, phone conferences, and

national meetings. • Build communication tools to share information and updates.

2. Develop governance structure for system change effort

Activities: • Create Executive Steering Committee, Clinical Advisory Board, Supportive Service Workgroup, and Educational Subcommittee • Define roles of Board Members and create mission statements for each

governing body.

Project Goal

Create a sustainable plan for supervision and coordinated service provision for all agencies that serve substance abusing juveniles.

Objectives and Activities1. Pilot the Reclaiming Futures model in the Nassau County Juvenile Treatment Court

Activities:• Formulate policies and procedures that address Reclaiming Futures goals. • Expand service hours of court, treatment and supervision. Enhance MH services.• Enhance supervision tools to include constant monitoring for drugs, alcohol and curfew.

2. Build centralized interagency tracking for Reclaiming Futures model standards

Activities:• Collect baseline data for assessment, initiation, engagement and completion of interventions. • Translate treatment process improvements into cost savings info for communication

plan.• Create baseline for local use of evidence based practices and profile of target

population.• Conduct system walk through with planners from the community.

3. Engage stakeholders in system change effort

Activities:• Build a communication system among stakeholders to identify what each stakeholder

can offer to address the target populations needs.• Engage county and state agencies to integrate RF into policy and the broader

community plan (use of GAIN and EPB’s).• Plan regular follow up for all advisory boards and stakeholders.

Enhance system capacity to utilize validated screening and assessment tools

Objectives and Activities

1. Promote universal screening and GAIN assessment

Activities:• Utilize the GAIN to assess substance abusing juvenile offenders that are participants in the NCJTC and the broader juvenile justice system.• Track the number of GAIN assessments conducted, collect and report all specific performance indicators required under the Government Performance and Results Act (GPRA) and the GAIN.• Use GAIN to supplant JUTA assessment and OASAS file requirements.

2. Build county-wide training capacity to sustain the utilization of those instruments

Activities:• Provide guidance for public agencies or not-for-profit organizations to pursue GAIN Trainer status. Disseminate the GAIN by training multiple screeners, certifying GAIN trainers and building ongoing training capacity at strategic locations (court, HHS centralized assessment, DSS Behavioral Health Unit, University, and CM function).• Write training agreements (get Kentucky MOU) that focus on implementation, clinical supervision and coaching for ongoing practice fidelity.• Distribute grant funded enhanced rates, over and above the rates the local providers receive from OASAS/Medicaid reimbursement, for GAIN assessments.

Improve the coordination and delivery of drug, alcohol, and supportive services to juvenile offenders throughout Nassau County

Objectives and Activities1. Support the delivery of evidence based practices for substance abuse treatment and supportive service providers.

Activities:• Train treatment and supportive service providers in evidence-based practices, ensure that local providers attain certification and that model incorporation and delivery are conducted with

fidelity • Distribute grant funded enhanced rates, over and above the rates the local providers receive from OASAS/Medicaid reimbursement, for EBP units of service.

2. Ensure that Reclaiming Futures standards for treatment initiation, engagement and completion are maintained.

Activities:• Document movement through juvenile system interventions. Develop and coordinate

intervention delivery plan that ensures that Reclaiming Futures standards are attained and maintained.

3. Enhance communication and coordination of all service and supervision components.Activities:• Create service coordination teams led by the NCJTC CM and Probation, to coordinate interagency service plans and monitor minimum standards. Involve family, supportive service providers and community members in service coordination meetings.

4. Develop data sharing plans and coordinated service plans.Activities:• Engage state and county agencies to share data and provide treatment system process data. Build interagency communication , service plan and reporting protocols with supervision, treatment and supportive service providers through the JUTA. • Create a one page interagency release

Project Goal

Coordinate substance abuse treatment providers, youth development providers, and schools with the court to build wraparound services.

Objectives and Activities1. Build interagency linkages to leverage existing resources, share data, build interagency system of care and enhance sustainability.

Activities:• Build wraparound service approach of both existing and nontraditional services (using mentors and natural helpers) that is family / consumer driven.

2. Conduct mapping of education stakeholders / advocatesActivities:• Build an outreach plan for targeted school districts (Principles/Superintendents/ Social Workers/ Guidance Counselors). Hold a school social workers meeting.• Engage facilitator to conduct focus groups with participants, families and community members to develop a coordinated service plan approach and resource bank.

3. Identify and mobilize community resources to match needs and strengths of youth, especially the need for pro-social activities.

Activities:• Produce a resource guide listing all relevant community-based providers and ancillary services• Utilize interns, faith based organizations and natural helpers to link to services and pro-social activities. Develop volunteer and mentoring training with focus on community service (to double the impact of involvement) and pro-social activities to engage community leaders.

Engage Youth and families in planning, implementation, evaluation and modification of Reclaiming Futures initiative in Nassau.

Objectives and Activities

1. Define engagementActivities:• Form youth and family subcommittee.

2. Create communication tools, define roles and limitations.Activities:• Utilize focus groups, community meetings and participation on planning bodies to involve juveniles, family members, community members and natural helpers. Identify and train natural helpers/mentors.

3. Encourage juveniles, family members, community members and natural helpers to be involved in planning and implementation.

Activities:• Tie quality assurance monitoring to family involvement as a prime indicator of a successful plan. Review “Family Voice,” LINK, and “Forum for Youth Investment.”

Objectives and Activities

Develop court and community education and engagement strategy (marketing strategy).

Activities:• Plan and conduct targeted community outreach by creating a press release, team outreach tools (local info, RF canned speeches and videos). Establish speaker’s bureau and media plan.• Create a communications subcommittee?• Conduct a community survey as an engagement tool.• Document all outreach accomplishments including law guardian events, public TV.• Build website and outreach tools with agreed upon message to targeted audience.• Demonstrate the power of the team approach by using the whole team for outreach when possible.• Engrain sustainability into outreach plan

Project Goal

Coordinate the Reclaiming Futures initiative with other state and county initiatives to enhance, complement and align RF with other funded initiatives for leverage, creative partnerships and sustainability.

Objectives and Activities

Engage state and county agencies to identify existing initiatives, ongoing development of screening and assessment tools and evidence based practices and service coordination

Activities:• Assemble info on all statewide agency initiatives to avoid duplication, Link all prevention, anti drug, education and enforcement to coordinate efforts and communicate issues.• Engage in county and state policy meetings with CCSI?, Disproportionate Minority Contacts subcommittee at DCJS, Juvenile Coordination Council at ?, Children’s Cabinet, Governor’s Task Force on Juvenile Justice.

Evaluate impact of Reclaiming Futures Initiative

Objectives and Activities1. Ensure all data for systems change is captured throughout the grant period.2. Build program feedback mechanisms.3. Record and report findings.

Activities:Produce final evaluative report for system change and organizational dynamics change, including:

• The extent to which the proposed service components have been provided participants are satisfied with the services and the project. • The extent to which recidivism and placements are reduced; • The extent to which CSAT training resulted in improved programming, increased knowledge concerning targeted populations, and improved ongoing training capacity.• Percentage of program participants assessed using the GAIN and served with evidence based practices• Of all program participants identified with AOD problems at screening and assessment, how many agreed to complete a service plan;• Percentage of program participants who received at least one service contact within 14 days of the GAIN assessment;• Of all program participants who agree to complete an appropriate service plan, how many initiated services as designed;• Percentage of program participants who engaged in at least three successful service contacts within 30 days of GAIN assessment;• Of all program participants who initiated a service plan, how many become fully engaged in services;• Of all program participants who become fully engaged in services, how many completed the service plan as designed.
